RESUMO

The article is devoted to contribution of major general (rear Admiral. - navy) of medical service Nikolay Grigoryevich Ryzhman (1944-2005), the chief of the medical service of the Northern fleet (1991-1999), to development of naval medicine, who would have turn 70 years this September. The constant analysis, and experience generalization of the medical services of the fleet connections and associations and aspiration to provide fighting capacity for the fleet crew - were a keynote of Ryzhman's activity concerned development and deployment of the new forms and methods of medical support and searching of ways to increase crew efficiency. The implemented methods and forms of organization of the medical support and scientific development of the medical support are used even now in the practical activities of the medical service of the Northern fleet.

RESUMO

Violation of qualitative and quantitative composition of the microflora of the intestinal tract has a negative effect on the body and conduces to formation of pathosis. As a result of our study we have the date which confirmed the relevance between the functional status of hepatobiliary system, status of lipid exchange and violations of the microflora of the large intestine. Therefore, apparently, that the preservation and sustentation of normal microflora of the intestinal tract are the most important components determining the health status of the person and microecological approaches to the understanding of the development of the pathosis can be the starting point of the creation of new methods of treatment.

RESUMO

The aim of work was examination of peculiarities of outhospital pneumonia (OHP) among military servicemen, participants of local armed conflicts in areas of Prodnestrovie, Tadjikistan, North Ossetia, Abkhazia. There were retrospectively analyzed 139 archive patient histories of military servicemen, got pneumonia during 1990-1993. In structure of OHP of military servicemen predominating was pneumonia with not hard course of disease. Was educes a clear season dependence of appearance of pneumonia in described conditions with it's peculiarities. Each fifth patient form call-up contingent had deficit of weight. It influenced on course of disease. In 40% of cases of OHP there were coexistent diseases. Peculiarities in clinical forms of disease were not found. Using of antibacterial measures in treatment of OHP was empirical, during therapy of acute pneumonia were used 3 antibiotics simultaneously. For a more long-term of treatment had a need soldiers, participants of combat actions on the Northern Caucasus.

RESUMO

For last few years in Russia and in the Armed Forces of RF is marked a growth of number of acute intoxications by GABA-agonists and by muscle relaxants, particularly by baclofen. Manifestations of intoxication by this specimen include cerebral and somatic-vegetal disorders, in grave cases of illness could evolve psychic excitement, delusions and coma. Treatment of baclofen intoxication compares of gastric lavage using a thick pathfinder, accompanied by insertion of adsorbent and apocathartic means, maintaining of impaired vital functions of body. Hereafter these injured patients should be hospitalized in a treatment institute, where exists a possibility of effectuating of reanimation and intensive care as well as extracorporal detoxication (hemosorption, hemodialysis).

Clinical practice has been recently applied new approaches to classifying patients with arterial hypertension (AH), which are based on the determination of the summary risk suggesting the prognosis of the disease. This approach is quite efficient due to the fact that it takes into account a combination of the comparable clinical symptoms of AH, which characterize its course. The paper presents data on the values of blood pressure (BP) in AH and on daily BP profiles (DBPP) in 494 patients with different categories of a risk and on their endothelial function. It also gives the results of achievement of target BP levels in patients with AH having different categories of a risk during long-term (36-month) treatment with enapril and perindopril, as well as data on its impact on endothelial vasomotor function. The findings suggest that the higher the risk is, the more severe the hypertensive syndrome is; that the redistribution of DBPP occurs towards prognostically poor types and endothelial dysfunction (EDF) progresses. Treatment with angiotensin-converting enzyme (ACE) inhibitors results not only to the achievement of target BP levels and to the normalization of DBPP, but also to the amelioration of EDF. Moreover, the co-administration of ACE inhibitors and isosorbide-5-mononitrates causes additional antiischemic effects.
