RESUMO

Acute thrombotic renal artery occlusion is rarely identified at premortem. Early diagnosis and treatment are crucial for salvage of renal function. We reported a case of successfully dissolved acute thrombotic occlusion of renal artery of 9 hour duration with intra-arterial infusion of urokinase in a 83-year-old man.

RESUMO

This study was performed to investigate the effects of a levan diet on the body fat accumulation and serum lipid composition of 29 Korean women over a period of up to 12 weeks (n = 13 for the control group, n = 16 for the levan group). The subjects ate an uncooked diet (6 g) with 400 ml of tap water twice a day. The mean body weight and height measurements of the subjects (levan group) at the outset were 66.0 +/- 8.8 kg and 156.7 +/- 5.3 cm, respectively. The subjects showed a significant reduction in weight, body fat mass, anthropometric measurements and skinfold thickness during the experimental period. The waist hip ratio (WHR) was 0.88 +/- 0.03 at the outset, and fell to 0.82 +/- 0.05 after 4 weeks. The intake of levan was also influenced on the levels of serum Fe, leptin, lipoprotein lipase, HDL-cholesterol, LDL-cholesterol, and triglyceride. The serum glucose levels were within the normal range during the experimental period. The initial serum triglyceride level was 121 mg/dl, but fell to 103 mg/dl after 4 weeks of levan supplementation. The current study demonstrates that a levan diet is effective in controlling weight, body fat, HDL-cholesterol, LDL-cholesterol, and triglyceride levels.

RESUMO

Wide QRS tachycardia may represent a rare proarrhythmic effect of some antiarrhythmic agents. Class Ic antiarrhythmic agents produce rate- dependent ventricular conduction slowing due to use-dependent sodium channel blockade, causing QRS prolongation in cases with an increased heart rate. The authors describe two cases of atrial flutter with 1:1 atrioventricular conduction exhibiting a wide QRS tachycardia while on therapy using flecainide and propafenone, and the difficulty in interpreting the ECG.

RESUMO

A malignant mesenchymoma is a rare soft tissue neoplasm that show at least two types of malignant mesenchymal differentiation, in addition to poorly differentiated fibrosarcomatous elements. They grow rapidly, recur frequently, and can be found in a wide variety of location, with most occurring in the extremities. A primary malignant mesenchymoma of the heart is extremely rare, with only 20 cases having been reported worldwide in the English literature up to 2002. We experienced a case of primary cardiac malignant mesenchymoma in a 59 year old female patient, who was admitted due to exertional dyspnea. This case is reported, with a review of the relevant literature.

RESUMO

Salmonellae cause a number of characteristic clinical infections in humans, including gastroenteritis, enteric fever, bacteremia, focal metastatic infection and an asymptomatic chronic carrier state. We report a case of large hepatic cyst infected with Salmonella paratyphi A. A 47-year-old woman was admitted, because of abdominal complains (fullness and pain) and fever. On physical examination, a mass was palpated on right upper quadrant of the abdomen. The computerized tomography of the abdomen showed a large hepatic cyst, but not cystic infection. Aspiration and percutaneous drainage of the hepatic cyst were perfomed for the therapeutic decompression under fluoroscopic guidance. Cultures of the cyst fluid revealed growth of Salmonella paratyphi A. Radiologic contrast media after draining of the cyst fluid revealed no communication between the cyst and bile ducts. Infected hepatic cyst appeared to result from hematogenic seeding. One month later, the patient improved and was discharged without any complication.
