PURPOSE: Enterocutaneous fistula (ECF) is a condition in which there is an abnormal connection between the intestinal tract and the skin. It can lead to high morbidity and mortality rates despite the availability of therapeutic options. Stem cells have emerged as a potential strategy to treat ECF. This study aimed to evaluate the effect of adipose tissue-derived stem cells (ASC) on ECF in an experimental model. METHODS: ECF was induced in 21 Wistar rats, and after one month, they were divided into three groups: control group (C), culture medium without ASC group (CM), and allogeneic ASC group (ASC). After 30 days, the animals underwent macroscopic analysis of ECF diameter and histopathological analysis of inflammatory cells, tissue fibrosis, and vascular density. RESULTS: The study found a 55% decrease in the ECF diameter in the ASC group (4.5 ± 1.4 mm) compared to the control group (10.0 ± 2.1 mm, p = 0.001) and a 59.1% decrease in the CM group (11.0 ± 4.3 mm, p = 0.003). The fibrosis score in the ASC group was 20.9% lower than the control group (p = 0.03). There were no significant differences in inflammation scores among the three groups. CONCLUSIONS: This study suggests that ASC treatment can reduce ECF diameter, and reduction in tissue fibrosis may be a related mechanism. Further studies are needed to understand the underlying mechanisms fully.

Introducción: Las personas con cáncer necesitan recibir cuidados enfocados en aspectos biopsi-cosociales considerando la experiencia con la medicación (MedExp), la evolución de esta enfermedad y la calidad de vida relacionada con la salud (CRVS). Se presentan los resultados del Compre-hensive Medication Management (CMM) implanta-do en una institución colombiana especializada en oncología. Método: Diseño cuali-cuantitativo mixto observacional, descriptivo y prospectivo. Los datos se obtuvieron mediante triangulación de técnicas cualitativas (entrevistas en profundidad y observación participante) y cuantitativas (entrevistas clínicas con cuestionarios validados). Las entrevistas fueron presenciales y telemáticas por la pandemia (febrero-octubre 2021). Según los estándares del CMM, se generó la historia farmacoterapéutica, se registró la información biopsicosocial y se analizó la MedExp y la CVRS utilizando Medicines Optimisation Software®. Resultados: Se evaluaron los estándares antes y después de su implantación. La valoración inicial tuvo un análisis DAFO (Debilidades, Amenazas, Fortalezas, Oportunidades) consensuando estrategias para desarrollar CMM. El servicio de oncología derivó 17 personas, 10 mujeres aceptaron participar. Se completaron 30 entrevistas recogiendo narrativas sobre experiencias, observaciones y datos clínicos contextualizados a partir de las que se realizaron intervenciones biopsicosociales. Se resolvieron 2 condiciones clínicas, 2 mejoraron, 7 permanecieron estables, 9 mejoraron parcialmente y 4 no tuvieron mejoría. Tras recibir CMM, las participantes mejoraron su autopercepción en salud, y mejoraron principalmente las dimensiones de movilidad, realización de actividades cotidianas y ansiedad/depresión. Conclusiones: En la implantación del CMM se integró el modelo biopsicosocial considerando las experiencias reales de padecer cáncer alcanzando mejoría en condiciones clínicas y en CVRS de las pacientes atendidas. Introduction: People with cancer need to receive care focused on biopsychosocial aspects, consid-ering medication experience (MedExp), disease and illness progression, and health-related quality of life (HRQoL). The results of the Comprehensive Medication Management (CMM) implemented in a Colombian institution specialized in oncology are presented. Method: Mixed observational, descriptive, and pro-spective qualitative-quantitative design. Data were obtained by triangulating qualitative (in-depth inter-views and participant observation) and quantitative techniques (clinical interviews with validated ques-tionnaires). Data was collected through interviews carried out face-to-face and remotely due to the pandemic situation during February-October 2021. According to CMM standards, the pharmacothera-peutic history was obtained, recorded, and MedExp and HRQoL were analyzed using Medicines Optimi-zation Software®. Results: The standards of pharmaceutical care practice were established through the implemen-tation of a SWOT analysis to support care process. The oncology service referred 17 people; 10 women agreed to participate. Thirty interviews were completed collecting narratives about experiences, observations and contextualized clinical data from which biopsychosocial interventions were carried out. Clinical results obtained were: 2 clinical condi-tions resolved, 2 improved, 7 stable, 9 partially im-proved and 4 unimprovement. After receiving CMM, the participants improved their self-perception of health, and HRQoL dimensions of mobility, daily activities, and anxiety/depression improved. Conclusions: Through the implementing of CMM, clinical conditions related to the patients’ medica-tions were improved. Results guided the actions to be followed when implementing this biopsychoso-cial model in the institution. Providing benefits for patients and caregivers, in terms of avoiding the deterioration of quality of life despite suffering from of oncological diseases.

INTRODUCTION: There is a growing interest in vegetarian and vegan diets, but both can potentially affect tissue fatty acids (FA) composition. We aimed to evaluate the effect of vegetarian diets on plasma, erythrocytes, and sperm n-3 polyunsaturated fatty acids (n-3 PUFA) status in healthy young men. METHODS: Four groups were studied: i) men consuming a regular omnivore diet (OMV-1, n = 35); ii) men consuming an omnivore diet but excluding fish and seafood (OMV-2, n = 34); iii) men consuming a pescetarian diet (including dairy, eggs, fish, and seafood) (PESC, n = 36); and iv) men following a strict vegan diet (VEG, n = 35). Participants in each group should follow their diet for at least the previous 12 months. Diet evaluation used a structured validated food frequency questionnaire. FA composition was measured in plasma, erythrocyte phospho-lipids, and spermatozoa by gas-liquid chromatography, expressed as a mole percentage of the total FA content. RESULTS: Main findings showed higher alpha-linolenic fatty acid (ALA) and total n-3 PUFA dietary intake in the VEG group. In plasma, arachidonic and eicosapentaenoic acids were higher in OMV and PESC groups, whereas docosahexaenoic acid (DHA) level was lower in VEG. Higher ALA, but reduced DHA and total n-3 PUFA levels were found in erythrocytes and spermatozoa in the VEG group. CONCLUSION: Higher dietary ALA intake was found in pescetarians and vegan men. However, the higher ALA intake was not reflected in higher DHA content in the evaluated tissues. PUFA assessment, with particular emphasis in DHA, are necessary to improve PUFA status in vegan men.

BACKGROUND: Loxoscelism is an important public health problem in Chile and South America, due to the higher rate of cutaneous-visceral involvement. The diagnosis of loxoscelism is mostly clinical without established diagnostic criteria. There is little evidence to support any treatment used in this condition. AIM: To characterize the clinical features and epidemiology of loxoscelism among patients consulting at the Emergency and Dermatology Services of a clinical hospital between 2013 and 2017. MATERIAL AND METHODS: Review of medical records of patients registered in the electronic clinical record system with a confirmed diagnosis of loxoscelism. Epidemiological, clinical, laboratory tests and treatment variables were analyzed. RESULTS: We reviewed data from 200 patients. Ninety-four percent presented cutaneous loxoscelism and 5.5% cutaneous-visceral loxoscelism. Systemic symptoms were present in 73% of patients with cutaneous-visceral loxoscelism. Patients who developed systemic symptoms had an 18 times higher risk of developing cutaneous-visceral loxoscelism. Laboratory abnormalities were more common in patients with cutaneous-visceral loxoscelism. Not all patients with hematuria had cutaneous-visceral loxoscelism. Most patients required analgesia. Anti-loxosceles serum was not used in any patient. CONCLUSIONS: Many questions remain to be answered regarding the diagnosis and treatment of the disease. Studies are required to validate diagnostic criteria for loxoscelism, predictors for visceral involvement and response to treatment.

Continuous medical education focused on health problems emerging in low- and middle-income countries (LMICs) is scarce. Although tobacco consumption is increasing in LMICs, there is a lack of tobacco cessation training programs in these countries. To promote smoking cessation interventions in Bolivia, Guatemala and Paraguay, we adapted an e-learning program developed in Catalonia (Spain). This process evaluation study reports on reach, dose and satisfaction of participants with the course, as well as the contextual factors of its application. We conducted a multiple method evaluation, which included a survey and several focus groups, each one specific to the same type of healthcare professional (nurses, doctors, other professionals). Two hundred and ninety-two participants registered into the online course. The motivation for undertaking the course was different between doctors and nurses. The main sources of difficulty in enrolling and finishing the course were the technical problems experienced when accessing the platform, and lack of acquaintance with computers and the Internet in general. Our results show that implementing e-learning education in hospitals from LMICs is feasible, especially when there are similarities between participating countries and the country in which the original program was developed. However, several elements such as strong organizational commitment, technical support and resources and adequate communication channels should be provided to facilitate enrollment and training completion. Efforts to improve Internet access should be made to avoid jeopardizing students' motivation to enroll and complete online training.

This paper describes the global distribution and temporal evolution of bovine brucellosis due to Brucella abortus during a 19-year period (1996-2014) using the information officially reported to the World Organisation for Animal Health (OIE) by veterinary services of 156 countries. Variables that can influence the health status of bovine brucellosis (i.e., year, per capita Gross Domestic Product (GDP), continent and bovine population) were also analysed. Countries were classified into three categories of health situations: ENZOOTIC: countries infected, which may have been free of brucellosis but for periods of fewer than 3 years; NON-ENZOOTIC: countries where the disease was present but that had at least a 3-year period without the disease; and FREE: countries where the disease remained absent during the whole period. The countries free from bovine brucellosis, or in the process of eradication, were located in Oceania and Europe, while the more affected regions were Central and South America, Africa and parts of Asia. Among the Non-Enzootic countries, the results showed that a very high proportion managed to control the disease during the period of study, with a sharp decline in the percentage of infected countries from 71% in 1996 to 10% in 2014. Among the Enzootic countries, a much smaller proportion managed to control the disease, with a slight drop in the percentage of infected countries from 92% in 1996 to 80% in 2014. A relationship was found between the status of the disease and the availability of economic resources; thus, countries with a high GDP per capita tended to be free from bovine brucellosis. On the other hand, countries with a larger bovine population showed a greater probability to have the disease present. An increase in surveillance programmes and implementation of control policies were observed during the period of study.

Highly pathogenic avian influenza (HPAI) is of major importance for human and animal health because of high morbidity and mortality in poultry and the potential for transmission of this zoonotic pathogen to humans. Knowledge of HPAI epidemiology in avian populations and practical information on the temporal and spatial spread of the disease after introduction into a country is important in order to enhance the capacity of predicting and managing epidemics to minimize the negative impacts on human and animal health. Using data reported to the World Organisation for Animal Health between 2005 and 2017 by 199 countries for 14,129 outbreaks in poultry, we used a spatial and time-series analysis to determine that: (a) During the last 12 years, there were two major global peaks in the number of countries affected by HPAI with 23% and 26% of countries affected in 2006 and 2016. (b) Based on the seasonality analysis, spread is the lowest in September, begins to rise in October, and peaks in February. (c) The median distance HPAI outbreaks spread from the index outbreak was 111 km, while the median apparent rate of spread of outbreaks was 1.9 km/day. (d) In 39% of HPAI events, the disease did not spread beyond the index outbreak and the median maximum spread from the index outbreak per event was 45 km. (e) The distance HPAI outbreaks spread from the index outbreak was significantly negatively correlated with the number of outbreaks during the same time period, indicating that the spread of HPAI was lower during global panzootics than during periods of low transmission. These findings are of major importance for veterinary services to design and implement surveillance measures for improving preparedness to minimize the impacts of this disease.

Objective: To examine changes in hospital workers' interventions before and after online training. Method: Pre-post evaluation of the self-reported performance of the 5A's by hospital workers from the three organizations involved. We assessed individual, behavioural, and organisational-level factors through a questionnaire that included 43 items (0 = none to 10 = most possible) completed before and 6 months after the training. Medians and interquartile ranges were calculated. To examine changes, the non-parametric test for paired data (Wilcoxon) was used. Results: 202 professionals (76 in Bolivia, 79 in Guatemala, and 47 in Paraguay) finished the course, of these 99 (28, 42, and 29 respectively) completed both questionnaires before and after the training. Overall, there was an increase in the performance of each of the 5A components [Ask (7 to 9): Advise (7 to 9); Assess (6 to 8); Assist (2 to 7); and Arrange a follow up (0.52 to 5); all p <0.001]. Doctors, former smokers, and those from Paraguay obtained higher scores. The level of perception of the participants degree of preparedness, level of competence and familiarity with resources increased (p <0.001). Conclusion: The online training had a positive impact on the implementation of the brief intervention. Online education on smoking cessation is feasible and effective in improving smoking cessation interventions in these countries


Objetivo: Examinar los cambios en las intervenciones de los trabajadores antes y después de la formación. Método: Evaluación pre-post de la intervención breve autorreportada para dejar de fumar (5A). Se evaluaron factores individuales, de comportamiento y de nivel organizativo mediante un cuestionario de 43 ítems (de 0 = nada a 10 = completamente). Los cuestionarios se completaron antes y 6 meses después de la formación. Se calcularon medianas y rangos intercuartílicos. Para examinar los cambios se utilizó la prueba no paramétrica para datos apareados (Wilcoxon). Resultados: 202 profesionales (76 en Bolivia, 79 en Guatemala y 47 en Paraguay) finalizaron el curso; de ellos, 99 (28, 42 y 29, respectivamente) completaron ambos cuestionarios. Hubo un aumento en el desempeño de cada uno de los componentes de las 5A [Averiguar (7,5 a 9); Asesorar (7 a 9); Analizar (6 a 8); Ayudar (2 a 7); y Acordar un seguimiento (0,5 a 5); p <0,001]. Los médicos, los exfumadores y los profesionales de Paraguay obtuvieron puntuaciones más altas. El grado de preparación, el nivel de competencia y la familiaridad con los recursos aumentó entre los participantes (p <0,001). Conclusión: El curso tuvo un impacto positivo en la implementación de la intervención breve. La formación online en cesación tabáquica es factible y eficaz en estos países

The expanding international wildlife trade, combined with a lack of surveillance for key animal diseases in most countries, represents a potential pathway for transboundary disease movement. While the international wildlife trade represents over US $300 billion per year industry involving exchange of billions of individual animals, animal products, and plants as traditional medicines, meat from wild animals, trophies, live exotic pets, commercial products and food, surveillance and reporting of OIE-Listed diseases in wildlife are often opportunistic. We reviewed peer-reviewed literature for reports of 73 OIE-Listed terrestrial animal diseases in wild animals and found 528 possible wild animal hosts using our methodology. Not all host-pathogen relationships indicate that a particular species serves an epidemiologically significant role in the transmission of disease, but improved reporting of infections in wild animals along with clinical and pathological findings would contribute to improved One Health risk assessments.

BACKGROUND: For several decades now an antagonism between Trypanosoma cruzi infection and tumor development has been detected. The molecular basis of this phenomenon remained basically unknown until our proposal that T. cruzi Calreticulin (TcCRT), an endoplasmic reticulum-resident chaperone, translocated-externalized by the parasite, may mediate at least an important part of this effect. Thus, recombinant TcCRT (rTcCRT) has important in vivo antiangiogenic and antitumor activities. However, the relevant question whether the in vivo antitumor effect of T. cruzi infection is indeed mediated by the native chaperone (nTcCRT), remains open. Herein, by using specific modified anti-rTcCRT antibodies (Abs), we have neutralized the antitumor activity of T. cruzi infection and extracts thereof, thus identifying nTcCRT as a valid mediator of this effect. METHODS: Polyclonal anti-rTcCRT F(ab')2 Ab fragments were used to reverse the capacity of rTcCRT to inhibit EAhy926 endothelial cell (EC) proliferation, as detected by BrdU uptake. Using these F(ab')2 fragments, we also challenged the capacity of nTcCRT, during T. cruzi infection, to inhibit the growth of an aggressive mammary adenocarcinoma cell line (TA3-MTXR) in mice. Moreover, we determined the capacity of anti-rTcCRT Abs to reverse the antitumor effect of an epimastigote extract (EE). Finally, the effects of these treatments on tumor histology were evaluated. RESULTS: The rTcCRT capacity to inhibit ECs proliferation was reversed by anti-rTcCRT F(ab')2 Ab fragments, thus defining them as valid probes to interfere in vivo with this important TcCRT function. Consequently, during infection, these Ab fragments also reversed the in vivo experimental mammary tumor growth. Moreover, anti-rTcCRT Abs also neutralized the antitumor effect of an EE, again identifying the chaperone protein as an important mediator of this anti mammary tumor effect. Finally, as determined by conventional histological parameters, in infected animals and in those treated with EE, less invasive tumors were observed while, as expected, treatment with F(ab')2 Ab fragments increased malignancy. CONCLUSION: We have identified translocated/externalized nTcCRT as responsible for at least an important part of the anti mammary tumor effect of the chaperone observed during experimental infections with T. cruzi.

RESUMO

This article gives an overview of the World Organisation for Animal Health (OIE) Worldwide Animal Disease Notification System and highlights the major achievements during the past decade. It describes the different types of disease notification reports received and processed by the OIE. It also evaluates the three strategies implemented by the OIE in the recent years aimed at improving disease notification: introduction and use of a secure online notification system World Animal Health Information System (WAHIS) and its database interface World Animal Health Information Database (WAHID); implementation of active search and verification procedures for non-official information; and enhanced building of capacity for animal disease notification to the OIE by Members Countries. The improvements are evidenced by the increasing number of reports submitted on an annual basis and the reduction in submission time together with an improvement in the quality and quantity of the immediate notifications and follow-up reports, six-monthly and annual reports submitted by Veterinary Authorities. In the recent years, the OIE's notification system provides an early warning system more sensitive and global. Consequently, there is a greater knowledge of animal diseases' distribution worldwide. As a result, it is possible to ensure better prevention, more accurate risk assessment and evaluation by diminishing the spread of known or newly emerging pathogens.
