INTRODUCTION: Objective: to present the results of the Spanish home enteral nutrition (HEN) registry of the NADYA-SENPE group for the year 2016 and 2017. Material and methods: from January 1st 2016 to December 31st 2017, the HEN registry was recorded and afterwards a further descriptive and analytical analysis was done. Results: in 2016, 4,578 active patients were recorded and prevalence was 98.33 patients per one million inhabitants; in 2017, 4,777 patients were recorded, with a prevalence of 102.57 per one million inhabitants; 50.8% were males in 2016 and 50.5% in 2017. During the period 2016-17, median age was 71.5 years (IIQ 57-83), 1,558 HEN episodes were finished and the main cause was death (793 patients, 50.89%). Adult males were younger than females (65.3 vs. 73.3 years, p-value < 0.001). The most frequent diagnosis was the neurological disorder that presents with aphagia or severe dysphagia (59%). Nasogastric tube was the most frequent administration route (48.3%) and it is the most widely used in elderly patients (p < 0.001). One hundred and twenty-six pediatric patients were registered (57.1% females). Median age at the beginning of HEN in children was four months. "Other disorders" was the most recorded diagnostic group (41.3%), followed by the group of neurological disorder that presents with aphagia or severe dysphagia. Regarding children, 57.6% were fed through gastrostomy and the younger ones were fed through nasogastric tube (p-value 0.001). Conclusions: the number of patients in the registry, as well as the number of participating centers, is progressively increasing. The main characteristics of the patients have not changed. Despite the increase in diagnostic possibilities in the pediatric population, the classification within the group of "Other pathologies" is quite significant.

Los varones adultos fueron más jóvenes que las mujeres (65,3 vs. 73,3 años, p-valor < 0,001) y el diagnóstico más frecuente fue la enfermedad neurológica que cursa con afagia o disfagia severa (59%). La sonda nasogástrica (SNG) fue la vía de acceso más utilizada (48,3%) y se observa, además, que esta es la vía que se utiliza en los pacientes más ancianos (p < 0,001). Se registraron 126 pacientes pediátricos (57,1% niñas). La edad mediana de inicio de la NED fue de cuatro meses. Otras patologías fue el grupo diagnóstico más registrado (41,3%), seguido por la enfermedad neurológica que cursa con afagia o disfagia severa. Se alimentaban a través de gastrostomía en el 57,6% de los casos. Se observó que los niños más pequeños eran los que se alimentaban preferentemente por SNG (p-valor 0,001). Conclusiones: el número de pacientes del registro, así como el número de centros participantes, se va incrementando progresivamente. Las principales características de los pacientes no han variado. A pesar del aumento de posibilidades diagnósticas en la población pediátrica, llama la atención la clasificación dentro del grupo de Otras patologías.

Objective: To present the results of the Spanish home enteral nutrition (HEN) registry of the NADYA-SENPE group for the years 2014 and 2015. Results: In 2014, 3749 patients were recorded, and 4202 in 2015; prevalence was 80.58 patients/one million inhabitants in Spain in 2014 and 90.51 in 2015. There were 49.9% females in 2014 and 50.3% in 2015. Median age was 73 years (IQI 59-83) in 2014 as well as in 2015. 684 episodes finished in 2014 and 631 in 2015, with death as the main cause, in 54.9% and 50.4%, respectively. The ones who were fed through nasogastric tube had a mean age higher than the ones fed by any other route (p-value < 0.001). Sisty-seven paediatric patients were recorded in 2014 (56.7% females) and 77 in 2015 (55.8% females). Median age at the beginning of HEN among children was 5 months in 2014 and 5 months in 2015. The main route of administration was gastrostomy, in 52.5% in 2014 and nasogastric tube in 50.8% in 2015. 7 episodes finished in 2014 and 13 in 2015, having death as the main cause (57.1% in 2014 and 38.5% in 2015). It was found that were younger children the ones who were mainly fed by nasogastric tubes (p-value 0.004 vs. 0.002). Among paediatric patients as well as adults, the main diagnosis leading to HEN was neurological disease which gives aphagia or severe dysphagia. Conclusions: There has been an increase in the number of patients in the registry as well as the participating centers and the number of patients per center, without any significant change in the characteristics of the patients other than longer duration of the episodes.

Conclusiones: se ha incrementado el número de pacientes del registro, así como el número de centros participantes y el número medio de pacientes comunicados por cada centro respecto a años anteriores, sin que se hayan modificado sustancialmente las características de los pacientes, salvo mayor duración de los episodios.

BACKGROUND: Appropriate maternal intake of iodine during pregnancy is essential for maternal thyroxine production and thyroid status of the fetus. It should be possible to enhance iodine intake during pregnancy by using iodine fortified salt or taking iodine supplements. In the present report we determined the status of iodine nutrition in pregnant women who were stratified on the basis of their history of taking or not taking iodized salt or iodine supplements. The study was performed in Toledo (Spain), a region in which prior studies have noted borderline iodine sufficiency. Iodine nutrition was assessed by measuring urinary iodine concentration (UIC) and neonatal thyrotropin (TSH). METHODS: UIC was measured in 525 pregnant women. They were grouped according to their history of iodine intake. Diet Group 1 patients (n = 69) did not take iodized salt or iodine supplements during pregnancy. Diet Group 2 patients (n = 75) took iodized salt but not iodine supplements during pregnancy. Diet Group 3 patients (n = 381) took iodine supplements during pregnancy. Plasma determinations included TSH, free thyroxine, thyroid peroxidase antibody, and thyroglobulin antibody. UIC was measured in a single urine sample from all the pregnant women. Neonatal TSH was measured in capillary spot blood from all the neonates as part of a screening for congenital metabolic abnormalities. RESULTS: The median UIC in all subjects was 164 µg/L (interquartile range [IR]: 116-245). The median UICs in Diet Groups 1, 2, and 3 were 134.5 (IR: 90-196), 146 (IR: 103-205), and 183 (IR: 124-261) µg/L, respectively (p = not significant [NS] for Diet Group 1 vs. 2; p < 0.01 for Diet Group 2 vs. 3; all other comparisons NS). The median (IR) TSH of the neonates in all Diet Groups was 1.0 (IR: 0.7-1.6) µU/mL. Only 2 neonates had blood TSH concentrations >5 mU/L. Neonatal blood TSH concentrations were similar in all Diet Groups. CONCLUSIONS: In a region with a history of borderline iodine deficiency the UICs were below 150 µg/L in a substantial percentage of pregnant women who did not take iodine supplements, regardless of whether or not they took iodized salt. Our results support the use of iodine supplements from the start of the pregnancy, or even before pregnancy in women who live in regions with a history of even small degrees of iodine deficiency. In addition, neonate TSH screening is not the best tool to assess whether the iodine status in populations is ideal.

Introducción: Actualmente, el hiperparatiroidismo primario (HPP) se considera una enfermedad escasamente sintomática y poco progresiva. El HPP normocalcémico (HPPN) es una entidad reconocida e identificada esencialmente por complicaciones asociadas al HPP (osteoporosis y urolitiasis). Pacientes y método: Estudio de 56 pacientes con HPP no operados o con seguimiento mínimo de 1 año previo a la paratiroidectomía. Se subdividió a los pacientes en HPP hipercalcémico (HPPH) (27 pacientes) y HPPN (29 pacientes). Se pretendió analizar las comorbilidades (hipertensión arterial [HTA] y nefrolitiasis) en el grupo total y en cada subgrupo, valorar la evolución anual de calcemia, fosfatemia, paratirina (PTH) y fosfatasa alcalina séricas, calciuria, filtrado glomerular y densidad mineral ósea lumbar durante un período de 1 a 5 años y hallar posibles diferencias entre HPPH y HPPN para estas variables. Resultados: El 55,4% de los pacientes presentaban HTA y se registraron episodios de nefrolitiasis en el 37,5% de los casos. No se apreciaron cambios a lo largo del tiempo en los parámetros bioquímicos y densitométricos ni en el grupo total ni por subgrupos. Excepto en los valores de calcio, fósforo y PTH séricos no se hallaron diferencias entre HPPH y HPPN. Conclusiones: El HPP parece una enfermedad poco progresiva si bien no se incluyó en este estudio a los pacientes remitidos precozmente a cirugía con seguimiento previo escaso que podrían corresponder a casos más agresivos. El HPPN es una entidad asociada a complicaciones similares a las del HPPH por lo que pensamos debería someterse a los mismos criterios de derivación quirúrgica (AU)


Introduction: Primary hyperparathyroidism (PHP) is considered a relatively stable, asymptomatic disease. Normocalcemic PHP (NPHP) is now a recognized entity that is usually diagnosed through its associated complications (such as osteoporosis and nephrolithiasis). Patients and method: We studied 56 patients with PHP who did not undergo surgery or who completed a minimum follow-up of 1 year before parathyroid surgery. The group was subdivided into patients with hypercalcemic PHP (HPHP) (27 patients) and those with NPHP (29 patients). The presence of comorbidities (hypertension, nephrolithiasis) was analyzed in the whole group and in each subset of patients, as was yearly progression of calcemia, phosphatemia, serum parathyroid hormone (PTH) and alkaline phosphatase, urinary calcium, glomerular filtration rate and bone mineral density in the lumbar spine during a follow-up period ranging from 1 to 5 years. Possible differences in these variables between HPHP and NPHP were also studied. Results: A total of 55.4% of the patients were hypertensive and nephrolithiasis events were reported in 37.5%. No changes were observed in biochemical or densitometric variables over time or in the whole group or in the subgroups. Except for serum calcium, phosphate and PTH values, no differences were found between HPHP and NPHP. Conclusions: PHP seems to be a relatively stable disease, although patients referred early to surgery with a short follow-up, who could have had more aggressive kinds of PHP, were not included in this study. The complications of NPHP are similar to those of HPHP and therefore we believe the same recommendations for surgery should be followed in both forms (AU)

Introducción: La utilización de bexaroteno, agonista selectivo del receptor de retinoide X (RXR), en pacientes con linfoma cutáneo de células T (CTLC), ha puesto de manifiesto la aparición de dos complicaciones frecuentes, como son el hipotiroidismo central y la hipertrigliceridemia. Nos proponemos comprobar las repercusiones en las hormonas tiroideas y los lípidos plasmáticos en un pequeño grupo de pacientes con CTLC tratados con bexaroteno. Pacientes y método: Presentamos a 6 pacientes (2 mujeres, 4 varones) con CTLC, en distintos estadios de la enfermedad y resistentes por lo menos a un tratamiento sistémico previo, que recibieron tratamiento con bexaroteno a dosis de 300 mg/m2. Se evaluó la TSH basal y la T4 libre y los lípidos plasmáticos (colesterol y triglicéridos) al inicio del tratamiento, a las 4 y las 8 semanas y, en los pacientes en que se suspendió el tratamiento, 4 semanas después de finalizado. Resultados: En 4 de 6 pacientes, la función tiroidea antes del tratamiento con bexaroteno era normal, en un paciente se desconocía y otro había sido diagnosticado previamente de hipotiroidismo primario autoinmunitario subclínico. A las 4 semanas de iniciado el tratamiento, todos mostraron una disminución de las concentraciones de tirotropina (TSH) y tiroxina (LT4), y 4 precisaron tratamiento con dosis sustitutivas de levotiroxina. Todos los pacientes presentaron elevaciones en las concentraciones plasmáticas de colesterol y/o triglicéridos que requirieron tratamiento con estatinas o fenofibrato. En aquellos en que se suspendió el bexaroteno, las hormonas tiroideas y los lípidos retornaron a cifras similares a las basales. Conclusiones: El tratamiento con bexaroteno en pacientes con CTLC induce la aparición de alteraciones endocrino-metabólicas, como hipotiroidismo central y dislipemia mixta, que requieren tratamiento con dosis sustitutivas de levotiroxina y fármacos hipolipemiantes en la mayoría de las ocasiones. Dichas alteraciones son reversibles tras la suspensión del fármaco


Introduction: Bexarotene, a synthetic retinoid receptor X (RXR)-selective ligand recently approved for the treatment of cutaneous T-cell lymphoma (CTCL), often produces two complications: central hypothyroidism and hypertriglyceridemia. The aim of this study was to analyze the effects of bexarotene on thyroid hormones and plasma lipids in a small group of patients with CTLC treated with this drug. Patients and method: We assessed a cohort of 6 patients (2 women and 4 men) who were in different stages of the disease and resistant to at least one previous systemic treatment. The patients received bexarotene at a dose of 300 mg/m2. Levels of thyroid-stimulating hormone (TSH), free thyroxine (LT4), cholesterol, and triglycerides were evaluated at baseline and at weeks 4 and 8. In patients who suspended treatment, these levels were evaluated 4 weeks after treatment was stopped. Results: Pretreatment thyroid function was normal in four out of the 6 patients and was unknown in one; subclinical autoimmune hypothyroidism had previously been diagnosed in the remaining patient. After 4 weeks of treatment, TSH and LT4 were reduced in all patients, and thyroid hormone replacement therapy was required in 4. Plasma cholesterol and/or triglyceride levels were increased in all patients, and additional treatment with statins or fenofibrate was required. In patients who discontinued bexarotene treatment, thyroid hormone and lipid levels returned to baseline values. Conclusions: Bexarotene treatment in patients with CTCL produces endocrine-metabolic alterations such as central hypothyroidism and mixed dyslipidemia, which usually require treatment with additional doses of levothyroxine and lipid-lowering agents. These changes are reversible after discontinuing the drug
