RESUMO

In this work a lipid extract from shrimp waste was obtained and characterized. The most abundant fatty acids found were C16:0, C18:2n6c, C18:1n9c, C22:6n3, and C20:5n3. The extract contained all-trans-astaxanthin, two cis-astaxanthin isomers, 5 astaxanthin monoesters, and 10 astaxanthin diesters (7±1mg astaxanthin/g). C22:6n3 and C20:5n3 were the most frequent fatty acids in the esterified forms. Appreciable amounts of α-tocopherol and cholesterol were also found (126±11mg/g and 65±1mg/g, respectively). Little lipid oxidation was observed after 120days of storage at room temperature, revealed by a slight reduction of ω-3 fatty acids, but neither accumulation of TBARS nor formation of oxidized cholesterol forms was found. This is attributed to the antioxidant effect of astaxanthin and α-tocopherol, as their concentrations decreased as storage continued. The lipid extract obtained has interesting applications as food ingredient, owing to the coloring capacity and the presence of healthy components.

OBJECTIVES: To estimate the impact of smart pump implementation in a pediatric intensive care unit in terms of number and type of administration errors intercepted. DESIGN: Observational, prospective study carried out from January 2010 to March 2015 with syringe and great volumen infusion pumps available in the hospital. SETTING: A tertiary level hospital pediatric intensive care unit. PARTICIPANTS: Infusions delivered with infusion pumps in all pediatric intensive care unit patients. INTERVENTIONS: Design of a drug library with safety limits for all intravenous drugs prescribed. MAIN VARIABLES: Users' compliance with drug library as well as number and type of errors prevented were analyzed. RESULTS: Two hundred and eighty-three errors were intercepted during 62 months of study. A high risk drug was involved in 58% of prevented errors, such as adrenergic agonists and antagonists, sedatives, analgesics, neuromuscular blockers, opioids, potassium and insulin. Users' average compliance with the safety software was 84%. CONCLUSIONS: Smart pumps implementation has proven effective in intercepting high risk drugs programming errors. These results might be exportable to other critical care units, involving pediatric or adult patients. Interdisciplinary colaboration is key to succeed in this process.

BACKGROUND: Strong acid inhibition using esomeprazole increases cure rates with triple therapy and 10-day treatments are more effective than 7-day ones. The combination of amoxicillin plus metronidazole at full doses, and using a physiologically-correct schedule three times a day, and has been shown to overcome metronidazole resistance and to achieve good eradication rates. AIMS: To assess the eradication rate of a new first-line treatment regimen associating strong acid inhibition, amoxicillin and metronidazole and to evaluate tolerance. METHODS: Patients from eight hospitals were included. Helicobacter pylori status was assessed by at least one of the following: histology, culture, rapid urease test or urea breath test (UBT). Ten-day treatment was prescribed comprising esomeprazole 40 mg twice a day plus amoxicillin 1 g and metronidazol 500 mg both three times a day. Helicobacter pylori cure was assessed by UBT. RESULTS: A hundred and thirty-six patients were enrolled. Mean age was 52.6 ± 16 years and 59.6% of patients were men. Main indications for treatment were: uninvestigated dyspepsia (13.6%); functional dyspepsia (18.2%); gastric ulcer (21.8%); and duodenal ulcer (39.8%). Helicobacter pylori eradication was achieved in 112 of the 127 patients who returned for follow-up. Eradication rates were 82.4% (95% CI: 74.7-88.1) by intention-to-treat analysis and 88.2% (95% CI: 81.2-92.8) by per protocol. Treatment was well tolerated and no major side effects were reported. Nine patients complained of mild side effects. CONCLUSIONS: Cure rates of the combination of esomeprazole, amoxicillin and metronidazole are high and the treatment was well tolerated. This pilot study warrants the comparison of this schedule with current standards.

Solid phase microextraction (SPME) coupled with either gas chromatography-ionization flame detector (CG-FID) or multidimensional gas chromatography-mass spectrometry (MDGC-MS) was evaluated for its ability to detect volatile hydrocarbons produced during the irradiation of cooked ham. The chromatogram of an irradiated sample obtained using GC-FID showed a complex pattern of peaks, with several co-eluting peaks superimposed, indicating that the method was unlikely to resolve adequately the volatile hydrocarbons formed during irradiation. Using SPME-MDGC-MS 1-tetradecene (C(1-14:1)), n-pentadecane (C(15:0)), 1-hexadecene (C(1-16:1)), n-heptadecane (C(17:0)) and 8-heptadecene (C(8-17:1)) were detected in cooked ham irradiated at 0.5, 2, 4 and 8kGy. This method allows the detection of most n-alkanes and n-alkenes produced during the irradiation of the majority of fatty acids in cooked ham, namely oleic acid, stearic acid and palmitic acid. SPME is rapid and inexpensive and does not require organic solvents. The proposed SPME-MDGC-MS method allows the determination of radiolytic markers in cooked ham in less than 115min.

The use of on-line coupling of reversed-phase liquid chromatography and gas chromatography (RPLC-GC) with the through oven transfer adsorption desorption (TOTAD) interface and mass spectrometry (MS) was proposed for testing different types of commercial Spanish dry-cured ham for irradiation treatment at various doses (0, 1.5, 2, and 4 kGy). The qualitative analysis of radiation-specific compounds (e.g., n-pentadecane, 1-hexadecene, 1,7-hexadecadiene, n-heptadecane, 8-heptadecene, and 2-dodecylcyclobutanone) can be simultaneously established in a single run with samples that have or have not been irradiated. The overall analysis, which takes less than 100 min, includes a rapid extraction step using a small amount of dichloromethane-methanol (1:1, vol/vol) and anhydrous sodium sulfate, the subsequent fractionation of the sample in the first dimension of the system (RPLC), the transfer of the target fraction to the second dimension, the GC separation, and the MS detection. The calculated limits of detection in ham were lower than 22 ng/g. Repeatability studies provided relative standard deviation values of 0.8 to 13.5%.

Tomato industries yield a high amount of by-products mainly tomato peel and seeds. Since tomato peel is rich in lycopene, the direct addition of peel to food products could be a way to use this by-product to obtain a new products enriched in lycopene. This work describes experiments performed to develop dry fermented sausages (salchichón) containing this carotene. 0%, 0.6%, 0.9% and 1.2% (w/w) of dry tomato peel was added to the meat mixture used in sausage manufacture. A slight losts of lycopene was detected after 21 days ripening, however, levels remained between 0.26 and 0.58mg of lycopene/100g of sausage. The sensory and textural properties and overall acceptability of all sausages were good, indicating that tomato peel could be added to dry fermented sausages to produce a meat product enriched in lycopene.

BACKGROUND: The rendezvous technique combines endoscopy with percutaneous transhepatic cholangiography to facilitate cannulation of the bile duct when previous attempts have failed. METHODS: Over a 7-year period, a total of 1753 ERCPs were performed. Twelve of these patients with a diagnosis of choledocholithiasis were poor candidates for surgery. Percutaneous transhepatic cholangiography as well as ERCP with precut papillotomy failed to resolve biliary obstruction. In a further 2 cases the percutaneous approach was used by means of a T-tube positioned at a prior cholecystectomy. OBSERVATIONS: The combined procedure was successful in 13 patients (93%). It was unsuccessful in 1 patient because of a stone lodged distally near the papilla. There was only 1 complication (7%), a retroperitoneal perforation that occurred during papillotomy; no mortality was directly attributable to the technique. CONCLUSIONS: The rendezvous technique is recommended for patients who are not eligible for surgery when ERCP is unsuccessful and when it is impossible to resolve biliary obstruction by percutaneous transhepatic cholangiography.

The influence of the species, fat, curd incubation temperature (25, 30, and 35 degrees C), heat treatment of milk (70 degrees C for 5 or 30 min), and milk pH on the initial volume drained, syneresis rate, and the loss of proteins in drainage was studied. The volume drained as a function of the curd incubation time (up to 60 min) was adjusted to a first-order kinetic reaction. The k values (drained rate) and the initial volume obtained applying the equation were compared to establish the possible influence of the studied factors. In general, for all the factors studied, the syneresis rate of curd from caprine and ovine milk did not differ from those described previously in the literature for the curds from bovine milk. However, for each studied factor the pattern of syneresis rate was significantly different among the species in most of the experiments.

The influences of ultrafiltration (UF), the intensity of heat treatment (70 degrees C for 5 or 30 min) applied to milk prior to UF processing, and species on the syneresis rate of curd from cow's, ewe's, and goat's milk were analyzed. The influence of the species was significant (P < 0.05) in the syneresis rate. The syneresis rate depended significantly on the intensity of the heat treatment applied before UF and the degree of concentration; these two parameters showed significant (P < 0.05) differences in the syneresis rate depending on the species studied. These results could indicate a possible difference in the structure of the curd formed. The differences in the curd structure could be due to the different relative composition of the retentates of the different species as well as the possible effect of the UF process on the physicochemical composition of milk.

BACKGROUND: To assess efficacy and tolerability of pefloxacin in association with other antibiotics in the treatment of acute and chronic bone and joint infections. METHODS: From January to December 1997, all the outpatients with diagnosis of acute or chronic bone and joint infections have been enrolled in a perspective study. If possible a cultural or histopathological study was performed. Treatment response was evaluated with radiological and clinical chemistry parameters. RESULTS: Fifteen patients [10 males, 5 females; mean age 40.7 +/- 15 years (range 15-71)] have been studied. They had 5 knee septic arthritis, 1 sacroileitis, 1 hip septic arthritis, 4 long bone osteomyelitis, 1 sterum osteomyelitis, 3 spondilitis. Three patients were HIV infected. Twelve were acute infections, 3 chronic ones. Overall, 7 were hematogenous infections, 6 subsequent to elective surgery, 1 post-traumatic thighbone osteomyelitis, 1 osteomyelitis by external fixation device. Isolates were S. aureus in 5 cases, P. mirabilis in 1 case, S. aureus+ Serratia marcescens in 1. In the remaining part cultural tests were negative. Pefloxacin was administered i.v. or orally at the dose of 400 mg/bid for a mean time of 114 +/- 74.5 days (range 30-270) in association with other chemotherapic agents. Ten good recoveries, 3 partial and 2 no responses were observed. CONCLUSIONS: Pefloxacin resulted to be useful in the treatment of acute and chronic bone and joint infections. No severe side effect was observed during the treatment.

AIM: to prospectively determine the diagnostic accuracy of magnetic resonance cholangiopancreatography (MRCP) in the evaluation of the normal and diseased pancreatic duct. METHODS: patients seen during a 6-month period with a diagnosis of biliary tract or pancreatic disease underwent endoscopic retrograde cholangiopancreatography (ERCP) after a previous MRCP. The pancreatic duct was evaluated with both techniques in 37 patients. RESULTS: the pancreatic duct appeared normal in ERCP in 27 patients, and also appeared normal in MRCP in 25 of these patients (specificity 93%). ERCP showed moderate-severe pancreatic duct dilation in 8 patients, in whom the same diagnosis was reached with MRCP (sensitivity 100%). The causes of dilation were chronic pancreatitis (2 patients), pancreatic cancer (3 patients) and ampullary tumor (2 patients); in 1 patient the findings with both techniques were suggestive of neoplasm of the head of the pancreas or focal chronic pancreatitis. Pancreas divisum was diagnosed in 2 patients by both methods, and the predominant dorsal duct as well as the ventral duct were visualized by MRCP. CONCLUSIONS: MRCP is an accurate technique for evaluating the normal or diseased pancreatic duct, and for determining the underlying disease.

RESUMO

MR cholangiopancreatography (MRCP) may replace direct pancreatography in the evaluation of the pancreatic duct. The aim of this pictorial review is to demonstrate the usefulness of MRCP in the evaluation of pancreatic duct pathology. The examination technique included coronal, sagittal and axial breath-hold HASTE 2D imaging using a body phase array coil. We present the diagnostic features on MRCP of a variety of benign and malignant disorders of the pancreatic duct, and conclude that MRCP is a suitable method for imaging the pancreatic duct system.

OBJECTIVE: To assess whether the clinical course of HIV infection has changed from 1985 to 1995. DESIGN: Cohort Study. SETTING: Infectious disease clinic. SUBJECTS: 285 patients recruited from September 1985 to January 1995 with < or = 12 months between the dates of their last seronegative and first seropositive test result and with first follow up visit in the six months after seroconversion and at least 12 months' follow up. Patients were grouped according to the date of seroconversion. MAIN OUTCOME MEASURES: Time to CD4 cell count of < 500, 400, and 200 x 10(6) cells/l, and clinical outcome defining AIDS; variation in cell count per day between consecutive visits, and ratio between this variation and time from estimated date of seroconversion at each visit. RESULTS: The groups were similar in age, number with acute primary HIV infection, CD4 cell count at intake, and cell count at the beginning of antiretroviral treatment; they differed in sex ratio, risk factors for HIV, probability of CD4 cell decline to < 500, 400, and 200 x 10(6) cells/l. and risk of developing AIDS. Acute infection, seroconversion after December 1989, and serum beta 2 microglobulin > 296 nmol/l were independent predictors of poor clinical course. The speed of CD4 cell decline, expressed as cell variation divided by the number of days between consecutive visits, increased with more recent seroconversion (P = 0.02). Ratio between the speed of CD4 cell decline and time from estimated date of seroconversion at each visit was also higher in the patients who seroconverted after December 1989. CONCLUSIONS: The faster disease progression and the higher speed of CD4 cell decline at early stages in the patients with recently acquired HIV infection suggest changes in the clinical course of HIV infection.

Our paper describes an unusual case of herpetic whitlow due to HSV-2 in an HIV-1 and HIV-2 infected patient. This patient was a 33-year-old cook, HIV-1Ab and HIV-2Ab positive for 4 years. The CD4+ cell count was below 50 cells/microL and no previous AIDS-defining illness happened. After having had a jagged tearing wound by a carving-knife on index finger of his right hand, he showed a rapid advancing erosion, which completely encircled his forefinger, due to HSV-2. Twenty days later he also showed two small adjacent lesions on penile shaft which rapidly extended with multiple subpreputial lesions. These lesions were caused by HSV-2 infection too. Both, finger and penile lesions, completely healed after a 3-week treatment with intravenous and oral acyclovir.

The effect of CO2 on the growth of psychrotrophic milk spoilage organisms was studied, both in raw fresh milk and in pure cultures of three species of Pseudomonas growing in sterilised milk. Changes of sensory properties of CO2-treated samples after heat treatment were also analysed. Inhibition of psychrotrophic growth at 7 degrees C in milk treated with CO2 to a pH 6.2 or 6.0 was impaired by a gradual reduction of the CO2 content during storage. Growth inhibition was considerably improved by pH adjustment at 24-h intervals. Sensory analysis showed significant differences between non-acidified and acidified samples after heat treatment at 75 degrees C for 20 s or 110 degrees C for 5 min. No sensory differences were found between non-acidified and acidified milks degassed before heat treatment.
