RESUMO

There have been increasing concerns regarding adverse local tissue reactions (ALTR) following metal-on-metal (MOM) hip arthroplasties. This study examined wear rates in retrievals of one design of MOM resurfacing arthroplasty, and assessed the differences in wear between those with and without ALTR. Wear measurements were made on 39 MOM resurfacing components (30 femoral, 9 acetabular) which were at least 2years in vivo. Seven hips (6 patients; 4 acetabular components, 7 femoral components) were identified to have ALTR. Acetabular component abduction and anteversion angles were determined using EBRA, and the contact-patch-to-rim (CPR) distance was calculated. The ALTR group had higher linear femoral and acetabular wear rates, acetabular anteversion and abduction angles, lower CPR, and longer time to revision. Given the increased risk for ALTR associated with acetabular component malpositioning, patients with malpositioned acetabular components may require closer clinical follow-up and monitoring.

RESUMO

Infection, loosening, osteolysis, or other causes can lead to the development of pain about a previously well-functioning total hip arthroplasty. An inflammatory reaction unique to metal on metal arthroplasty can lead to a painful total hip. A synovial biopsy is needed to make this specific diagnosis, and included in the differential diagnosis is infection. The workup of infection includes obtaining a C-reactive protein and erythrocyte sedimentation rate. Elevations of both the C-reactive protein and erythrocyte sedimentation rate are felt to indicate possible infection. This case report describes both of these findings and the treatment rendered in a painful subluxing metal-on-metal total hip arthroplasty presenting with ongoing pain and a large effusion.

In 12 patients undergoing a revision hip arthroplasty after a failed metal-on-metal primary hip arthroplasty, the effectiveness of intraoperative cell salvage (ICS) in removing metal ions was investigated. Samples of blood collected during surgery were filtered using 2 ICS devices. The samples had the concentrations of cobalt (Co) and chromium (Cr) measured before and after filtration. There was an average reduction of 76.3% for Cr concentration and 78.6% for Co concentration after ICS filtering. The Co-to-Cr ratio before and after filtration was similar. At the present time, these salvage systems should be used with caution in the patient undergoing revision of metal-on-metal bearing surfaces.

In an effort to augment the available grafting material as well as to increase spinal fusion rates, the utilization of a demineralized bone matrix (DBM) as a graft extender or replacement is common. There are several commercially available DBM substances available for use in spinal surgery, each with different amounts of DBM containing osteoinductive proteins. Each product may have different osteoinductivity potential due to different methods of preparation, storage, and donor specifications. The purpose of this study is to prospectively compare the osteoinductive potential of three different commercially available DBM substances in an athymic rodent spinal fusion model and to discuss the reasons of the variability in osteoinductivity. A posterolateral fusion was performed in 72 mature athymic nude female rats. Three groups of 18 rats were implanted with 1 of 3 DBMs (Osteofil, Grafton, and Dynagraft). A fourth group was implanted with rodent autogenous iliac crest bone graft. The rats were sacrificed at 2, 4, 6, and 8 weeks. A dose of 0.3 cm(3) per side (0.6 cm(3)per animal) was used for each substance. Radiographs were taken at 2 weeks intervals until sacrifice. Fusion was determined by radiographs, manual palpation, and histological analysis. The Osteofil substance had the highest overall fusion rate (14/18), and the highest early 4 weeks fusion rate of (4/5). Grafton produced slightly lower fusion rates of (11/17) overall, and lower early 4 weeks fusion rate of (2/5). There was no statistically significant difference between the rate of fusion after implantation of Osteofil and Grafton. None of the sites implanted with Dynagraft fused at any time point (0/17), and there was a significantly lower fusion rate between the Dynagraft and the other two substances at the six-week-time point and for final fusion rate (P = 0.0001, Fischer's exact test). None of the autogenous iliac crest animals fused at any time point. Non-decalcified histology confirmed the presence of a pseudarthrosis or the presence of a solid fusion, and the results were highly correlated with the manual testing. Although all products claim to have significant osteoinductive capabilities, this study demonstrates that there are significant differences between some of the tested products.

BACKGROUND: Following the reintroduction of metal-on-metal articulating surfaces for total hip arthroplasty in Europe in 1988, we developed a surface arthroplasty prosthetic system using a metal-on-metal articulation. The present study describes the clinical and radiographic results of the first 400 hips treated with metal-on-metal hybrid surface arthroplasties at an average follow-up of three and a half years. METHODS: Between November 1996 and November 2000, 400 metal-on-metal hybrid surface arthroplasties were performed in 355 patients. All femoral head components were cemented, but only fifty-nine of the short metaphyseal stems were cemented. The patients had an average age of forty-eight years, 73% were men, and 66% had a diagnosis of osteoarthritis. Clinical and radiographic follow-up was performed at three months postoperatively and yearly thereafter. RESULTS: The majority of the patients returned to a high level of activity, including sports, and 54% had activity scores of >7 on the University of California at Los Angeles activity assessment system. Kaplan-Meier survivorship curves demonstrated that the rate of survival of the components at four years was 94.4%. For patients with a surface arthroplasty risk index score of >3, the rate of survival of the components at four years was 89% compared with a rate of 97% for those with a score of

BACKGROUND: There are two main modes of failure of the femur following surface arthroplasty of the hip: femoral neck fracture and aseptic loosening. The purpose of the present study was to present our experience with femoral neck fractures that occurred after metal-on-metal hybrid surface arthroplasty and to assess their cause. METHODS: A series of 600 metal-on-metal surface arthroplasties was performed between late 1996 and early 2003. Failures that occurred during this period were assessed radiographically and with implant retrieval analysis to determine their cause. RESULTS: Five femoral neck fractures occurred in this series (prevalence, 0.83%). Four of the five fractures occurred at the component-neck junction within the first five months (average, three months) after surgery. All five fractures were associated with a traumatic episode, but all five also were associated with structural and/or technical risk factors, which we believe weakened the femoral neck. The most important technical deficiency that contributed to three of the five fractures was the failure to cover all of the reamed bone with the component. CONCLUSIONS: It is important to avoid or at least minimize notching of the femoral neck by performing the cylindrical reaming at the recommended angle of 140 degrees and to stop reaming before the reamer touches the lateral cortex. Osteophytes should be removed judiciously only if there is notable impingement when the hip is flexed to 90 degrees and internally rotated. We believe that understanding the factors that contribute to femoral neck fracture after surface arthroplasty may reduce the prevalence of this mode of failure.

The purpose of the present study was to compare wear particles isolated from metal-metal (MM) hip implants worn in an orbital bearing simulator with particles from similar MM total hip replacement (THR) implants worn in vivo. Comparison of these particles is important because it will help to assess the overall suitability of this type of hip simulator for reproducing in vivo wear and for producing physiological wear particles suitable for biological studies of in vitro cellular response. Commercial grade components made of ASTM F75 (cast) alloy were evaluated. Simulator tests were performed in 95% bovine calf serum with a 28-mm-diameter implant. Wear particles were collected from 0 to 0.25 million cycles (run-in wear period) and 1.75 to 2 million cycles (steady-state wear period). Tissues from seven patients with MM implants (surface replacement or stem type) were harvested at revision surgeries (after 1-43 months). Metal wear particles were isolated from serum lubricant or tissues using an enzymatic protocol that was previously optimized to minimize particle changes due to reagents. After isolation, particles were centrifuged, embedded in epoxy resin, and characterized by transmission electron microscopy (TEM) and energy dispersive X-ray analysis (EDXA). Results of EDXA on particles from the hip simulator primarily indicated a predominance of particles containing Cr and O but no Co (most likely chromium oxide particles), and fewer CoCrMo particles presenting varying ratios of Co and Cr. Image analysis of TEM micrographs demonstrated that the majority of the particles from the simulator were round to oval, but a substantial number of needle-shaped particles were also found, especially from 0 to 0.25 Mc. The particles generated from 0 to 0.25 Mc had an average length of 53 nm, whereas those generated from 1.75 to 2 Mc had an average length of 43 nm. In vivo, EDXA and TEM analysis of particles that were retrieved from two patients at 23 and 43 months respectively, revealed that they were the most comparable in composition, average length (57 nm), and shape to particles generated in the hip simulator during the run-in wear period. Because a large clinical retrieval study in the literature suggested that a run-in wear regime might occur in vivo for some 6-36 months, the fidelity of the simulator of the present study was strongly supported. However, some uncertainties existed, including the finding that the particles isolated from the other five patients generated from 1 month up to 15 months (shorter implantation times than the other two patients) were smaller and mostly contained only Cr and O (no Co). In the opinion of the authors, this particular very short term patient group was somewhat atypical. Therefore, despite these uncertainties, the present study was deemed to support the ability of the orbital bearing hip simulator to produce physiological wear particles.

Cytokines associated with osteolysis have been demonstrated in tissues surrounding failed metal-metal (MM) total hip replacements (THRs). The objective of the present study was to semi-quantify the amounts of inflammatory cytokines in tissues from 28 failed MM THRs, and determine their relationship with the quantity of metal particles. Paraffin sections were immunohistochemically stained with monoclonal antibodies: anti-IL-1-beta, anti-IL-6 and anti-TNF-alpha. Cytokines and metal particles were rated in 10 fields per tissue using standard light microscopy. Because of the use of light microscopy, only relatively large particles or agglomerations of particles were visible. Therefore, a polarized light and a semi-quantitative scheme based on the discoloration of cell cytoplasms induced by the presence of particles were used to evaluate the quantity of metal particles. Results showed an overall higher amount of IL-6 than IL-1beta while TNF-alpha remained at very low levels. For each patient, the average IL-1beta and IL-6 ratings decreased when the average particle rating increased, following a linear regression, with relatively high correlation factors (r=-0.69 for IL-1beta and r=-0.57 for IL-6). IL-1beta decreased about twice as fast as IL-6. TNF-alpha, remaining at very low levels, did not demonstrate any correlation with particle rating. When multiple tissues were available for the same patient, the correlation factors between the average cytokine and particle ratings were highly variable between samples, demonstrating the heterogeneity between the tissues from the same patient. At the cellular level, there was an even higher correlation between the quantity of metal particles and the production of IL-1beta and IL-6 (r=-0.99), while TNF-alpha did not demonstrate any correlation, remaining at very low levels. In conclusion, this study showed that tissues surrounding failed MM THRs with low to moderate quantities of metal particles can induce the production of potentially osteolytic cytokines. However, the overall number of cells producing these cytokines tended to be lower than that typically seen in tissues surrounding metal-polyethylene THRs.
