RESUMO

RATIONALE AND OBJECTIVE: To determine the diagnostic performance of transvaginal ultrasound (TVUS) performed by an US specialist and MRI based on the O-RADS scoring system. MATERIALS AND METHODS: Between March 5th 2013 and December 31st 2021, 227 patients, referred to our center, underwent TVUS and pelvic MRI for characterization of an adnexal lesion proven by surgery or two years of negative follow-up. All lesions were classified according to O-RADS US and O-RADS MRI risk scoring systems. Imaging data were then correlated with histopathological diagnosis or negative follow-up for 2 years. RESULTS: The prevalence of malignancy was 11.1%. Sensitivity of O-RADS US / O-RADS MRI were respectively of 83.3%/83.3% and specificity was 73.2%/92.9% (p < 0.001). O-RADS MRI was more accurate than O-RADS US even when performed by an US specialist (p < 0.001). When MRI was used after US, 51 lesions were reclassified correctly by MRI and only 4 lesions incorrectly reclassified. Most of the lesions (49/51) rated O-RADS US 4 or 5 and reclassified correctly by MRI were benign, mainly including cystadenomas or cystadenofibromas. Only 4 lesions were misclassified by MRI but correctly classified by ultrasound. CONCLUSION: Our study suggests that MR imaging has equally high sensitivity but higher specificity than TVUS for the characterization of adnexal lesions based on O-RADS scoring system. MRI should be the recommended second-line technique when a mass is discovered during TVUS and is rated O-RADS 4 and 5 over than TVUS by an US specialist.

RESUMO

IMAGING OF OVARIAN CANCERS Imaging of ovarian lesions is based on the triptych: pelvic ultrasound with Doppler, magnetic resonance imaging (MRI) and computed tomography (CT). Endovaginal pelvic ultrasound is the first-line examination; tools are available to help with its interpretation (IOTA group rules, O-RADS US classification). When an ovarian mass is detected on ultrasound and if its characterization is «undetermined¼ or complex, MRI with perfusion sequences is used to clarify the criteria. Finally, a thoraco- abdominopelvic CT scan is necessary for extension assessment in the case of ovarian cancer, in particular to look for peritoneal carcinosis and to anticipate surgical difficulties.


IMAGERIE DANS LES CANCERS DE L'OVAIRE L'imagerie des lésions ovariennes repose sur le triptyque : échographie pelvienne avec Doppler, imagerie par résonance magnétique (IRM) et tomodensitométrie (TDM). L'échographie pelvienne par voie endovaginale est l'examen de première intention ; des outils aident à son interprétation (règles du groupe IOTA, classification O-RADS US). Lorsqu'une masse ovarienne est détectée à l'échographie et si sa caractérisation est « indéterminée ¼ ou complexe, la réalisation d'une IRM avec séquences de perfusion permet d'en préciser les critères. Enfin, la TDM thoraco-abdomino-pelvienne est nécessaire au bilan d'extension en cas de cancer de l'ovaire, notamment à la recherche de carcinose péritonéale et pour anticiper des difficultés chirurgicales.

This study aimed to use bioassays (single and multispecies) with organisms from different trophic levels to assess soil quality in reclaimed coal mining areas. Soil samples were collected from four sites: two sites with recent reclaim processes (one using topsoil and other using clayey soil), an natural attenuation site, and a control soil. The evaluated parameters were divided into (1) ecotoxicological tests (avoidance test with Eisenia andrei (earthworms) and Armadillidium vulgare (isopods); germination test with Sinapis alba seeds (mustard); reproduction tests with Folsomia candida (collembolans); bacterial toxicity test); (2) population and community assessments (a fungal count; microbial community analysis using Biolog EcoPlatesTM); (3) microcosms scale evaluation (the MS-3 multispecies system); and (4) chemical analysis (soil parameters, soil metal, and cations and anions in soil leachate). Results pointed to toxicity in the natural attenuation site that compromised of habitat function, probably due to low pH and low nutrient levels. The most recent reclaim process, using topsoil and clay soil, improved soil quality and fertility, with a further increase in habitat quality and heterogeneity. This study shows that some techniques used to reclaim degraded mining areas are effective in rebuilding habitats, sustaining soil biota, and reestablishing ecosystem services.

Discutir como são as relações de gênero entre pessoa hospitalizada e profissional de enfermagem em momentos de realização de procedimentos que necessitam expor o corpo e/ou intimidades e analisar até que ponto os direitos dos clientes hospitalizados têm sido garantidos nesses encontros. Consiste na reflexão acerca dos desafios do processo de cuidar em enfermagem, fundamentada nos direitos do cliente. Método: Trata-se de um estudo exploratório, com abordagem quanti-qualitativa, com participação de vinte clientes com faixa etária entre 25 e 70 anos, hospitalizados em uma instituição de saúde privada, situada no município de Niterói, baseado na Resolução n° 196/96 (CNS). O estudo foi encaminhado ao Comitê de ética em pesquisa da Escola de Enfermagem Anna Nery, sendo aprovado sob nº 104-08. Resultados e Conclusão: clientes do sexo feminino (55%) teriam uma preferência maior por profissionais do mesmo sexo, ao contrário dos clientes do sexo masculino (45%), que em sua maioria, demonstraram não possuir a preferência quanto ao sexo do profissional.


Objective: To discuss how the relationships of gender among hospitalized patients and nursing staff at times of exposure of intimacy. Consists in thinking about the challenges of caregiving in nursing, based on client rights. Method: This is an exploratory study with quanti-qualitative approach, with the participation of twenty customers aged between 25 and 70 years, hospitalized in a private health institution, located in Niterói, based on Resolution No. 196 / 96 (CNS). The study was referred to the Committee for Research Ethics College of Nursing Anna Nery, and approved under paragraph 104-08. Results and Conclusion: Female customers (55%) have a higher preference for professionals of the same sex, as opposed to male clients (45%), which mostly demonstrated not have a preference as to sex work.


Discutir cómo son las relaciones de género entre los pacientes hospitalizados y el personal de enfermería en los momentos de realización de procedimientos que necesitan exponer el cuerpo y / o intimidades y analizar hasta qué punto los derechos de los clientes hospitalizados están garantizados en estos encuentros. Consiste en la reflexión sobre los desafíos del proceso de la atención de enfermería, basada en los derechos del cliente. Método: Este es un estudio exploratorio con enfoque cuanti-cualitativo, con la participación de veinte clientes de edades comprendidas entre 25 y 70 años, hospitalizados en una institución de salud privada, situada en el municipio de Niterói, en base a la Resolución No. 196 / 96 (CNS). El estudio fue remitido al Comité de Ética en Investigación de la Escuela de Enfermería Anna Nery, y aprobado con el No. 104-08. Resultados y Conclusiones: Los clientes de sexo femenino (55%) tendrían una mayor preferencia por los profesionales del mismo sexo, en contraposición a los clientes masculinos (45%), que en su mayoría no han demostrado una preferencia en cuanto al sexo del profesional.
