RESUMO

A case of male breast carcinoma in a very young subject (seventeen years old) is described. Breast carcinoma in male is infrequent, and at a so young age it is really rare. The patient was subjected to "Patey" mastectomy with removing the lymph nodes of the breast with the tumor, and subcutaneous mastectomy at the opposite side breast. As agreed with the oncologist other therapy was carried out.

RESUMO

The authors explore the correlation between the neuroradiologic and clinical patterns of "subcortical arteriosclerotic encephalopathy" (SAE) or Binswanger's disease. The typical neuroradiologic pattern has been found in 62 of the 1804 CT scans of the department's archive. Available patients were enrolled for neuropsychologic evaluation. The authors point out that there is no correlation whatever between severity of the CT pattern and findings of psychological decline. Therefore SAE may be considered merely a neuroradiologic pattern. The discrepancy between neuroradiologic and clinical pattern depends on the individual history of every single patient as well as on the quality of the neuronal cells involved in vascular pathology. It may in fact be thought that the injury to command neurones plays a fundamental role in bringing about the psychopathologic picture.

RESUMO

Results of personal experience of the application of enteral nutrition in a group of patients (21) submitted to surgery for gastric cancer are reported. Enteral nutrition proved easy to perform, provided a significant improvement in some of the parameters considered in the study, was without significant complications and presented acceptable management costs.

In order to characterize neurological, intellectual and behavioral sequelae better after surgical treatment of brain abscess in childhood, 32 children were studied over a 10-year time span after treatment of a brain abscess. Depending on the clinical condition, surgical treatment consisted of simple aspiration through a burr hole or of total excision after craniotomy. There were only 3 postoperative deaths (9%). The scheduled time for long-term follow-up was 3 years after surgery. The patient assessment included information concerning the presence of seizures, accurate neurological examination, an EEG, and a battery of intellectual tests to evaluate the presence of behavioral-mental deficits. Late seizures developed in 7 of the 29 survivors. Five patients presented permanent hemiparesis after surgical treatment. It is noteworthy that the intellectual impairment observed was more severe in younger patients, while the behavioral disturbances were more marked in school-age children.

In order to investigate the role of EEG in minor head traumata in the pediatric age, EEG and CT scan findings were compared in a series of 103 consecutive cases of children hospitalized within 24 h after head trauma. The EEGs were classified as normal in 50 patients, borderline in 10 patients, and abnormal in 43 patients. CT scan showed contusion in 6 patients and extracerebral hematoma in 4. All cases of abnormal CT scans were reported for patients with frankly abnormal EEG findings. In contrast, no pathological findings were found in CT scans for patients with normal EEG. The data suggest that EEG findings can play a major role in the diagnostic workup of patients with minor head traumata. Specifically, in the case of asymptomatic patients with normal EEG findings, it is likely that the CT scan will also be normal.

RESUMO

Rehabilitation by means of periodic irrigation is an easy-to-use method that is generally well accepted by the patient. By "giving up" an hour less a day, or better still every 48 hours, the patient can avoid the inconvenience of numerous, continual and uncontrollable evacuations. In confirmation of previous experiments, no significant differences were observed concerning early rehabilitation (up to 10 years after the operation). Irrigation has also been shown to have few negative aspects.
