Balanopostitis asociada al uso de cetuximab en adenocarcinoma colorrectal avanzado / Balanopostitis related to cetuximab treatment in advanced colorectal cancer patient

CASO CLÍNICO: paciente varón de 80 años, diagnosticado de adenocarcinoma de colon sigmoide + M1 hepáticas, que presenta lesiones en pene durante el tratamiento con Cetuximab, junto a otras reacciones adversas cutáneas. DISCUSIÓN: cetuximab es un anticuerpo monoclonal, cuya diana es el receptor del factor de crecimiento epidérmico. Se emplea en el tratamiento de tumores, principalmente cáncer de colon metastásico, cáncer de células escamosas de cabeza y cuello y otros. Los efectos secundarios se toleran mejor que la quimioterapia clásica, aunque presenta toxicidad en diversos tejidos epiteliales. No está descrita la lesión en el pene, y el mecanismo de afectación lo deducimos por analogía. CONCLUSIÓN: las medidas de soporte preventivas, el seguimiento y el tratamiento clásico de las balanopostitis son el pilar básico en su curación (AU)

CASE REPORT: A 80-year-old man with sigmoid colon adenocarcinoma + hepatic M1, afected by penis damage at the time of being treated with Cetuximab, beside other skin toxicities. DISCUSION: Cetuximab is a monoclonal antibody that specifically blocks epidermal growth factor receptor activity. It´s been used to treat tumours, such metastasic colorectal cancer, head and neck scamous cells cancer and others. Tolerance to it seems to be better than that to the classic chemotherapeutic agents, but has skin toxicities as side-effect. The injury over the penis skin is not yet been noticed, is for that reason that we explain the mechanism by analogy. CONCLUSION: Supportive and preventive measures, follow-up and classic treatment of balanopostitis are the key of success in the healing process of this injuries (AU)

[Inverted bladder papilloma. Our contribution (7 cases)]. / Papiloma invertido vesical. Nuestra aportación (7 casos).

Seven cases of urothelium inverted papilloma in vesical location. Frequency in our centre is 1.4% of all urothelium tumours and mean age of patients is 63 years, 57.1% men (4 cases) and 42.9% women (3 cases). Typical clinical signs and symptoms at presentation are hacmaturia and, less frequently, a mictional irritative picture or recurrent urinary infection. It can also be accidentally found during vesico-prostate surgery performed for other cases. Evolution is benign, similar to that of transitional papilloma in our experience, and only one case of asynchronic association with a low grade vesical papillar tumour has been observed. Treatment has been endoscopic in all cases and we do not believe that chemoprophylaxis and/or immunotherapy are useful.

[Leiomyosarcoma of the spermatic cord]. / Leiomiosarcoma de cordón espermático.

Presentation of an uncommon paratesticular tumour, located in the spermatic cord and originated in the smooth muscle of the cremaster, deferent, or the cord's vascular structures. In 1966, KYLE in Canada reviewed 22 cases of paratesticular leiomyosarcoma from the English literature, and in 1984 PUTS in the Netherlands extended this catalogue to 39. TAKAHA in 1991 presented 18 cases found in the Japanese literature. The theory of malignant degeneration from a previously existing leiomyomatous tumour is generally accepted. This entity is clinically associated with a clear tendency to local relapse, which does not imply a worse prognosis, and with a high percentage of blood metastasis three years after exeresis. The choice therapy is oncologic inguinal orchiectomy. Radiotherapy and chemotherapy, as coadjuvant therapies, appeared to be more effective in children's than in adult's sarcoma. Prognosis of these neoplasias in the reviewed literature is highly variable, ranging between 10% and 62% survival at 5 years, possibly due to the diversity of therapies and the patient's stage at the time of diagnosis.

Herein we describe two cases of prostatic carcinoma whose presenting feature was a left supraclavicular lymphadenopathy. The patients had presented no urinary symptomatology and the rectal examination of the prostate had disclosed minimal changes. A review of the literature has shown that the survival and treatment should be approached with the same protocols used in patient that initially present with bony or visceral metastasis. The finding of a supraclavicular lymphadenopathy should always prompt us to suspect the possibility of prostatic cancer.

[Bladder schistosomiasis: presentation of a new case]. / Esquistosomiasis vesical: presentación de un nuevo caso.

We report a case of urinary schistosomiasis involving only the bladder and mimicking a transitional cell pseudotumor. We describe the clinical and pathological features and discuss the diagnosis, medical and surgical treatment of this disease entity whose incidence is slowly increasing in black immigrants of the Maresme area of Barcelona.
