RESUMO

Mesenchymal stem/stromal cells (MSC) play a pivotal role in regenerative therapies. Recent studies show that factors secreted by MSC can replicate their biological activity, driving the emergence of cell-free therapy, likely to surpass stem cell therapy. Patents are an objective measure of R&D and innovation activities, and patent mapping allows us to verify the state of the art and technology, anticipate trends, and identify emerging lines of research. This review performed a search on Derwent World Patents Index™ and retrieved 269 patent families related to the MSC-derived cell-free products. Analysis reveals an exponential increase in patents from the mid-2010s, primarily focusing on exosomes. The patent's contents offer a great diversity of applications and associated technologies by using the products as medicinal agents or drug delivery systems. Nevertheless, numerous application branches remain unexplored, suggesting vast potential for cell-free technologies alone or combined with other approaches.

Cleomin, a 1,3-oxazolidine-2-thione, was recently isolated from Neocalyptrocalyx longifolium, a species traditionally used for treating painful conditions. Reports about the pharmacological activities of cleomin are lacking. Here, the antinociceptive effects of cleomin were investigated using mice models of pain, namely the formalin, the cold plate, and the tail flick tests. Motor integrity was assessed in the rota-rod test. Antagonism assays and in silico docking analyses were performed to investigate the putative mechanisms of action. Cleomin (12.5-25 mg/kg), at doses that did not induce motor impairment, induced dose-dependent antinociception in both early and late phases of the formalin test and reduced nociceptive behaviors in both the cold plate and tail flick tests. Pretreatments with phaclofen and atropine attenuated the antinociceptive effects of cleomin, implicating the involvement of GABAB and muscarinic receptors. In silico docking studies suggested satisfactory coupling between cleomin and GABAB and M2 receptors, hence corroborating their role in cleomin's activity. Pretreatments with naloxone, yohimbine, bicuculline, and methysergide did not affect the antinociception of cleomin. In silico pharmacokinetics prediction showed a good drug ability profile of cleomin. In conclusion, cleomin promoted antinociception mediated by GABAB and muscarinic receptors. These findings support further investigation of the analgesic potential of cleomin.

Carvacryl acetate (CA) is a monoterpene obtained from carvacrol, which exhibits anti-inflammatory activity. However, its low solubility in aqueous media limits its application and bioavailability. Herein, we aimed to develop a carvacryl acetate nanoemulsion (CANE) and assess its anti-inflammatory potential in preclinical trials. The optimized nanoemulsion was produced by ultrasound, and stability parameters were characterized for 90 days using dynamic light scattering after hydrophilic-lipophilic balance (HLB) assessment. To evaluate anti-inflammatory activity, a complete Freund's adjuvant-induced inflammation model was established. Paw edema was measured, and local interleukin (IL)-1ß levels were quantified using ELISA. Toxicity was assessed based on behavioral changes and biochemical assays. The optimized nanoemulsion contained 3% CA, 9% surfactants (HLB 9), and 88% water and exhibited good stability over 90 days, with no signs of toxicity. The release study revealed that CANE followed zero-order kinetics. Dose-response curves for CA were generated for intraperitoneal and oral administration, demonstrating anti-inflammatory effects by both routes; however, efficacy was lower when administered orally. Furthermore, CANE showed improved anti-inflammatory activity when compared with free oil, particularly when administered orally. Moreover, daily treatment with CANE did not induce behavioral or biochemical alterations. Overall, these findings indicate that nanoemulsification can enhance the anti-inflammatory properties of CA by oral administration.

RESUMO

Neuropathic pain (NP) is the most prevalent and debilitating form of chronic pain, caused by injuries or diseases of the somatosensory system. Since current first-line treatments only provide poor symptomatic relief, the search for new therapeutic strategies for managing NP is an active field of investigation. Multiple mechanisms contribute to the genesis and maintenance of NP, including damage caused by oxidative stress. The naturally occurring antioxidant alpha-lipoic acid (ALA) is a promising therapeutic agent for the management of NP. Several pre-clinical in vitro and in vivo studies as well as clinical trials demonstrate the analgesic potential of ALA in the management of NP. The beneficial biological activities of ALA are reflected in the various patents for the development of ALA-based innovative products. This review demonstrates the therapeutic potential of ALA in the management of NP by discussing its analgesic effects by multiple antioxidant mechanisms as well as the use of patented ALA-based products and how technological approaches have been applied to enhance ALA's pharmacological properties.

Objetiva-se identificar quais fatores da desigualdade social estão associados à gravidez na adolescência. Como método, utilizou-se a revisão integrativa da literatura e a busca dos artigos foi realizada entre junho/2021 e abril/2022 nos bancos de dados Pubmed via Medline, Scopus e Embase. A amostra final totalizou 28 artigos e verificou-se que os fatores de proteção associados à gravidez na adolescência foram a renda familiar mais elevada e a maior escolaridade do adolescente e dos pais. Como fatores de risco, destacaram-se o consumo de álcool, tabaco e outras drogas, a atividade sexual precoce e o perfil socioeconômico e demográfico. Investimentos em setores como o da educação contribuiriam para a redução das taxas de gravidez precoce porque a escola estimula a criação de projetos de vida e incentiva comportamentos mais adequados. O conhecimento acerca dos fatores da desigualdade social que aumentam o risco da gravidez precoce podem auxiliar a formulação de políticas públicas que diminuam as desigualdades em saúde.


The objective was to identify which factors of social inequality are associated with teenage pregnancy. As a method, an integrative literature review was used by searching for articles between June/2021 and April/2022 on the Pubmed databases via Medline, Scopus, and Embase. The final sample consisted of 28 articles, and we found that the protective factors associated with teenage pregnancy were: higher family income, higher schooling of the adolescent and parents; as risk factors, the following stood out: the consumption of alcohol, tobacco, and other drugs, early sexual activity, and socioeconomic and demographic profile. Investments in sectors such as education would contribute to reducing early pregnancy rates because school encourages the creation of life projects and more appropriate behaviors. Knowledge about the factors of social inequality that increase the risk of early pregnancy contributes to creating public policies that reduce health inequalities.

The proper pharmacological control of pain is a continuous challenge for patients and health care providers. Even the most widely used medications for pain treatment are still ineffective or unsafe for some patients, especially for those who suffer from chronic pain. Substances containing the chromone scaffold have shown a variety of biological activities, including analgesic effects. This work presents for the first time the centrally mediated antinociceptive activity of 5-O-methylcneorumchromone K (5-CK). Cold plate and tail flick tests in mice showed that the 5-CK-induced antinociception was dose-dependent, longer-lasting, and more efficacious than that induced by morphine. The 5-CK-induced antinociception was not reversed by the opioid antagonist naloxone. Topological descriptors (fingerprints) were employed to narrow the antagonist selection to further investigate 5-CK's mechanism of action. Next, based on the results of fingerprints analysis, functional antagonist assays were conducted on nociceptive tests. The effect of 5-CK was completely reversed in both cold plate and tail-flick tests by GABAA receptor antagonist bicuculline, but not by atropine or glibenclamide. Molecular docking studies suggest that 5-CK binds to the orthosteric binding site, with a similar binding profile to that observed for bicuculline and GABA. These results evidence that 5-CK has a centrally mediated antinociceptive effect, probably involving the activation of GABAergic pathways.

Physalins are seco-steroids with a variety of pharmacological activities already described. In this study the pharmacological properties of a standardized concentrated ethanolic extract from Physalis angulata (CEEPA), rich in physalins B, D, F and G, were studied in models of pain and inflammation in mice. Inflammatory mediators were measured by radioimmunoassay and Real-Time PCR in mice paws after the CFA stimuli. Systemic administration of CEEPA produced antinociceptive effect on the writhing test and formalin test. In the writhing test, physalins B, D, F and G showed that the antinociceptive effect of CEEPA is more potent than that of these purified compounds. In addition, CEEPA reduced the levels of TNF-α, IL-1ß, COX-2 and iNOS mRNA in the CFA-induced paw inflammation. Likewise, CEEPA decreased the TNF-α, IL-1ß and PGE2 paw levels. In conclusion, CEEPA induces antinociceptive and anti-inflammatory effects, with improved pharmacological potency relative to pure physalins, associated to modulation of cytokine and cyclooxygenase pathways.

This work aimed to use sodium trimetaphosphate/sodium tripolyphosphate cross-linked potato, banana, corn, cassava, and breadfruit starches as wall materials for C-phycocyanin encapsulation, characterize them and evaluate their in vivo pharmacological effects in an inflammation model. The cross-linked starches were successfully obtained, characterized, and submitted to C-phycocyanin encapsulation by freeze-drying. The characterization of cross-linked starches-C-phycocyanin composites by scanning electron microscopy, Fourier transform infrared spectroscopy, X-ray diffraction, thermogravimetry, and differential scanning calorimetry demonstrated that the C-phycocyanin was encapsulated between amorphous chains of cross-linked starches. Among the five preparations, the cross-linked potato starch presented the highest phosphorous content (0.084%), substitution degree (0.004), water uptake capacity (0.88 g g-1), and C-phycocyanin encapsulation efficiency (67.58%), thus was tested in vivo. The cross-linked potato starch-C-phycocyanin prolonged the antihyperalgesic effects attributed to C-phycocyanin, evaluated by complete Freund's adjuvant (CFA) model. Starch cross-linking promoted the formation of a hydrogel network in swollen state entrapping C-phycocyanin, thus, acting as a barrier to its release to the medium and promoting long-lasting in vivo effects. The combination of chemical modification of starches followed by physical treatment presented itself as a useful tool for the development of pharmaceutical formulations.

Chromones are a group of natural substances with a diversity of biological activities. Herein we assessed the pharmacological potential of three chromones (1, 2 and 3) isolated from Dictyoloma vandellianum as anti-inflammatory agents using in vitro and in vivo approaches. During in vitro screening, the production of NO and cytokines by macrophages stimulated with LPS and IFN-γ was inhibited by all chromones at concentrations (5-20 µM) that did not induce cytotoxicity. Analysis of pharmacokinetic parameters (in vitro half-life and intrinsic clearance) using human liver microsomes revealed that 3 has a superior pharmacokinetic profile, compared to 1 and 2. Treatment with 3 (100 mg/kg, ip) did not affect the mice motor performance, while 1 and 2 induced motor deficit. Taking into account the pharmacokinetic profile and absence of motor impairment, 3 was selected for further pharmacological characterization. Corroborating the data from in vitro screening, treatment of cell cultures with 3 (5-20 µM) reduced TNF-α, IL-6 and IL-1ß production by stimulated macrophages. In the complete Freund's adjuvant-induced paw inflammation model in mice, 3 (25 and 50 mg/kg, ip) inhibited mechanical hyperalgesia, edema and cytokine production/release (IL-1ß, IL-6 and TNF-α). 3 (5-20 µM) also reduced the transcriptional activity of NF-κB in stimulated macrophages. Furthermore, treatment with RU486, a glucocorticoid receptor (GR) antagonist, partially prevented the inhibitory effect of 3 on macrophages, indicating that this chromone exerts its anti-inflammatory effects in part through the activation of GR. The results presented herein demonstrate the pharmacological potential of natural chromones, highlighting 3 as a possible candidate for the drug discovery process targeting new anti-inflammatory drugs.

A dermatomioste é uma doença que caracteriza-se principalmente pela presença de fraqueza muscular, causando disfagia em aproximadamente 15% a 50% dos casos. Devido à escassez de dados na literatura e por se tratar de uma doença rara com alta mortalidade, sendo a disfagia um de seus agravantes, torna-se importante relatar os achados relacionados à deglutição. Portanto, o objetivo deste estudo foi descrever as alterações de deglutição apresentadas por uma paciente com diagnóstico de dermatomiosite juvenil. Riparins, natural alkaloids of the alkamide group, can be synthesized by simple methods, enhancing their potential application in pharmaceutical development. Here, the pharmacological properties of riparins were investigated in in vitro and in vivo assays of pain and inflammation in Swiss mice. Inflammatory mediators were measured by radioimmunoassay and Real-Time PCR. Riparins I, II, III and IV (1.56-100 mg/kg; ip) produced dose-related antinociceptive effects in the formalin test, exhibiting ED50 values of 22.93, 114.2, 31.05 and 6.63 mg/kg, respectively. Taking the greater potency as steering parameter, riparin IV was further investigated. Riparin IV did not produce antinociceptive effect on the tail flick, suggesting that its antinociception is not a centrally-mediated action. In fact, riparin IV (1.56-25 mg/kg) produced dose-related antinociceptive and antiedematogenic effects on the complete Freund's adjuvant (CFA)-induced paw inflammation in mice. During CFA-induced inflammation, riparin IV did not modulate either the production of cytokines, TNF-α and IL-10, or COX-2 mRNA expression. On the other hand, riparin IV decreased the PGE2 levels in the inflamed paw. In in vitro assays, riparin IV did not exhibit suppressive activities in activated macrophages. These results indicate, for the first time, that riparin IV induces antinociceptive and anti-inflammatory effects, possibly through the inhibition of prostanoid production.

Objetivo: O presente estudo teve por objetivo avaliar o Tratamento Restaurador Atraumático em molares decíduos e comparar seu aspecto clínico com restaurações convencionais que utilizaram amálgama e resina composta como material restaurador. Material e método: O estudo foi realizado em 10 Unidades de Saúde, na Cidade de Olinda, Pernambuco. A amostra foi composta por 136 crianças entre 6 e 7 anos de idade que apresentavam, pelo menos, um molar decíduo com cárie em média profundidade. Resultados: Foram avaliadas 231 restaurações realizadas por dentistas do serviço público após o período de 4 meses, sendo 159 restaurações atraumáticas com ionômero de vidro (ART), 43 restaurações convencionais com amálgama e 29 com resina. Foi utilizada a estatística descritiva para análise dos dados. Considerando o índice de sucesso, as restaurações com amálgama apresentaram o maior percentual (76,7%), seguido das restaurações com resina (51,7%) e ART (45,9%). As restaurações atraumáticas (ART) apresentaram índice mais elevado de menor defeito (43,4%). As restaurações em resina apresentaram maior índice de maior defeito ou perda do material (13,8%). Conclusão: o Tratamento Restaurador Atraumático apresentou resultados semelhantes às restaurações convencionais realizadas com resina e pior desempenho que as restaurações realizadas com amálgama.


Aim: The present study aimed to evaluate the Atraumatic Restorative Treatment in primary molars and compare their clinical aspect with conventional restorations, that used amalgam and composite resin as a restorative material. Methods: The study was conducted in 10 health units in the city of Olinda, Pernambuco. The sample consisted of 136 children between 6 and 7 years of age who had at least one deciduous molar with caries in average depth. Results: We evaluated 231 restorations for public service dentists, after a period of 4 months, 159 atraumatic restorations with glass ionomer (ART), 43 conventional amalgam fillings and 29 with resin. Descriptive statistics for data analysis was used. Considering the success rate, the amalgam fillings had the highest percentage (76.7%), followed by restorations with resin (51.7%) and ART (45.9%). The atraumatic restorations (ART) showed higher rate of minor defect (43.4%). The resin restorations showed higher default rate or loss of the material (13.8%). Conclusion: Atraumatic Restorative Treatment showed similar results to conventional restorations with resin and worse performance than with amalgam restorations.

BACKGROUND: This in situ study evaluated the roughness and microhardness of enamel bleached with 10% carbamide peroxide (PC10) and brushed with different toothpastes. MATERIALS AND METHODS: Two groups of volunteers received PC10 and placebo agents for 21 days in two phases in a crossover 2 × 3 study. Fragments of human enamel were distributed among intraoral removable appliances (IRA). Nine fragments, divided into three triplets, were used in each IRA, and these were brushed with toothpastes R (Colgate), W (Colgate Total 12 Whiteness Gel) or BS (Colgate Whitening Oxygen Bubbles Fluoride). Treatments agents were applied for 8 h overnight. After brushing, the volunteers used the IRA for about 16 h/day. After a washout period, new IRAs were distributed and the volunteers were crossed over to the alternate agent for 21 days. Roughness and microhardness were measured before and after each phase. RESULTS: According to the paired Student's t-test, roughness of enamel increased and microhardness decreased (P < 0.05). According to analysis of variance generalized linear models, only the toothpaste factor was significant (P = 0.037) for roughness. CONCLUSION: Enamel microhardness and surface roughness are altered when PC10 bleaching is associated with tooth brushing using toothpastes BS, R, and W.

PURPOSE: To verify the corrected gestational age when oral feeding (breast half full) and exclusive breastfeeding were initiated in preterm newborns submitted to the transition technique from enteral feeding directly to breastfeeding, and time spent (in days) under speech-language pathology intervention until discharge. METHODS: Study participants were 35 preterm infants (PTI) divided into two groups. Group 1 (G1) was composed by 22 PTI who did not present any important medical complications during hospitalization, and Group 2 (G2) comprised 13 PTI with a history of major medical complications. The following data were recorded: gestational age at birth, birth weight, weight and corrected gestational age during speech-language pathology evaluation, corrected gestational age when started oral feeding, number of days stimulated through the non-nutritive sucking (NNS) technique, corrected gestational age when oral feeding started and when they were discharged from speech-language pathology intervention, and total number of days under speech-language pathology intervention. RESULTS: No differences were found between the groups (G1 and G2) for any of the analyzed parameters. In general, oral feeding started with an average of 36 weeks corrected gestational age. Exclusive oral feeding occurred with an average of 36.6 weeks corrected gestational age. In average, speech-language pathology intervention lasted 12.3 days, and the NNS technique in "gloved finger" or "empty breast" lasted an average of 4.54 days. CONCLUSION: The technique allowed the PTI to be discharged in exclusive breastfeeding at a corrected gestational age corresponding to that of healthy term newborns.

OBJETIVO: Verificar a idade gestacional corrigida do início da dieta por via oral (mama parcialmente cheia) e da alimentação exclusiva em seio materno e o tempo dispendido (em dias) de atendimento fonoaudiológico para alta de recém-nascidos prematuros submetidos à técnica da transição da alimentação enteral direta para o seio materno. MÉTODOS: Fizeram parte do estudo 35 recém-nascidos prematuros (RNPT) divididos em dois grupos. O Grupo 1 (G1) foi composto por 22 RNPT que não apresentaram intercorrências médicas importantes durante a internação e o Grupo 2 (G2) por 13 RNPT com histórico de intercorrências médicas importantes. Foram registrados os seguintes dados: idade gestacional ao nascimento, peso ao nascimento, peso do recém-nascido e idade gestacional corrigida durante avaliação fonoaudiológica, idade gestacional corrigida no momento do início da alimentação por via oral, número de dias em que o recém-nascido realizou a técnica de sucção não nutritiva, idade gestacional corrigida no momento do início da alimentação por via oral exclusiva e da alta fonoaudiológica, e o número total de dias de atendimento fonoaudiológico. RESULTADOS: Não houve diferença entre os grupos (G1 e G2) para nenhum dos parâmetros analisados. De maneira geral, o início da oferta por via oral ocorreu com média de idade gestacional corrigida de 36 semanas. A alimentação por via oral exclusiva ocorreu com média de 36,6 semanas de idade gestacional corrigida. A média de dias de atendimento fonoaudiológico total foi de 12,3, sendo que o treino de sucção não nutritiva (SNN) em "dedo enluvado" ou "mama vazia" durou em média 4,54 dias. CONCLUSÃO: A técnica possibilitou a alta hospitalar do recém-nascido prematuro com alimentação exclusiva em seio materno em idade gestacional corrigida correspondente a do recém-nascido termo e saudável.


PURPOSE: To verify the corrected gestational age when oral feeding (breast half full) and exclusive breastfeeding were initiated in preterm newborns submitted to the transition technique from enteral feeding directly to breastfeeding, and time spent (in days) under speech-language pathology intervention until discharge. METHODS: Study participants were 35 preterm infants (PTI) divided into two groups. Group 1 (G1) was composed by 22 PTI who did not present any important medical complications during hospitalization, and Group 2 (G2) comprised 13 PTI with a history of major medical complications. The following data were recorded: gestational age at birth, birth weight, weight and corrected gestational age during speech-language pathology evaluation, corrected gestational age when started oral feeding, number of days stimulated through the non-nutritive sucking (NNS) technique, corrected gestational age when oral feeding started and when they were discharged from speech-language pathology intervention, and total number of days under speech-language pathology intervention. RESULTS: No differences were found between the groups (G1 and G2) for any of the analyzed parameters. In general, oral feeding started with an average of 36 weeks corrected gestational age. Exclusive oral feeding occurred with an average of 36.6 weeks corrected gestational age. In average, speech-language pathology intervention lasted 12.3 days, and the NNS technique in "gloved finger" or "empty breast" lasted an average of 4.54 days. CONCLUSION: The technique allowed the PTI to be discharged in exclusive breastfeeding at a corrected gestational age corresponding to that of healthy term newborns.
