A 32-year-old male was seen with a "large" lumbar disk herniation on a magnetic resonance image. His pain was described as mild and he was treated nonoperatively. Four years later the disk showed almost complete spontaneous regression. No other reports of a longer follow-up MR image interval have been reported in the literature.

A 43-year-old man presented with bubbly expansion and rarefaction of the body of the seventh cervical vertebra. The lesion involved the right side of the vertebral arch and had a soft-tissue component extending mainly anterior to the vertebra. Histopathologic examination of the specimen obtained by open biopsy revealed a giant cell tumour. Giant cell tumours of the spine, excluding the sacrum, are rare. Radiographically, they may be confused with metastatic carcinoma, plasmacytoma, lymphoma, chordoma and even benign lesions, particularly aneurysmal bone cyst and brown tumour of hyperparathyroidism. However, giant cell tumour occurs mainly in younger patients, involves the vertebral body selectively in most cases and has a bubbly appearance associated with rarefaction and expansion of the vertebral body, characteristics that may be helpful in the diagnosis.

Extradural tumors of the cervical spine may involve the vertebral artery on one or both sides, posing one of the limiting factors toward the radical resection of such neoplasms. A standard anterior approach may be inadequate for the management of such tumors. An anterolateral approach allows the dissection and mobilization of the vessel, which can then be preserved, resected, or reconstructed with a vein graft. An anterior approach can be supplemented with this for tumor resection and stabilization. This management strategy is described in 10 patients with a variety of tumors.

Wire breakage after a cervical occiput to C2 wire fixation and fusion resulted in the transdural and intramedullary migration of a wire fragment in a patient who remained neurologically intact. The risks of sublaminar wiring fixation are discussed, along with newer techniques for posterior cervical stabilization.

Central chondrosarcoma is an uncommon primary malignancy of the axial skeleton which usually affects the posterior elements or the posterior part of a vertebral body. We present an unusual case of chondrosarcoma involving the anterior part of a thoracic vertebra with massive extravertebral extension into the posterior mediastinum. The roles of computed tomography and magnetic resonance imaging in identifying this pathology are emphasized.

Osteopetrosis is due to a defect in osteoclastic cell function and results in osteosclerosis and progressive obliteration of the marrow spaces. Early onset osteoarthritis is associated with osteopetrosis. The authors describe a case of early onset osteoarthritis associated with osteopetrosis treated by total knee arthroplasty. The marblelike quality of osteopetrotic bone makes surgical treatment of these patients technically challenging and requires modification of standard surgical technique.
