INTRODUCTION AND DEVELOPMENT: Subthalamic stimulation is a therapeutic option that can be used to treat advanced cases of Parkinson's disease. However, psychiatric or cognitive disorders have been reported in some patients treated using this technique. Age and a long disease history are two important risk factors for the appearance of these problems. The complications that have been reported include cases of depression, apathy, manias and psychosis. Surgery can also exacerbate the syndrome of addiction to levodopa that is sometimes observed. In contrast, sleep disorders usually improve with this technique. As far as the cognitive sphere is concerned, verbal fluency has been seen to deteriorate and the executive functions become impaired in patients over 69 years of age. These disorders are usually due to a number of different causes and have been attributed to the action of stimulating areas close to the subthalamic nucleus, to the presence of previously existing cognitive or psychiatric problems, to unrealistic expectations about this technique or to the individual's inability to adapt to the functional situation after surgery. CONCLUSIONS: Although generally speaking these disorders are not usually serious, they must be borne in mind so that adequate treatment can be indicated.

Introducción y desarrollo. La estimulación subtalámica esuna opción terapéutica para los casos de enfermedad de Parkinsonavanzada. En algunos pacientes tratados con esta técnica se handescrito alteraciones psiquiátricas o cognitivas. La edad y la largaevolución de la enfermedad son dos factores de riesgo importantespara la aparición de estos problemas. Así, se han descrito casos dedepresión, apatía, manía y psicosis. La cirugía puede empeorar elsíndrome de adicción a la levodopa que se ve en ocasiones. Por elcontrario, esta técnica suele mejorar los trastornos del sueño. Sobrela esfera cognitiva, se ha constatado un empeoramiento de la fluenciaverbal, y en los pacientes mayores de 69 años, un empeoramientode las funciones ejecutivas. Las causas de estos trastornos suelenser variadas y se han atribuido a la acción de la estimulación sobreáreas próximas al núcleo subtalámico, a la existencia de problemascognitivos o psiquiátricos previos, a expectativas poco realistas sobreesta técnica o a la incapacidad de adaptarse a la situación funcionaltras cirugía. Conclusión. Aunque estas alteraciones en generalno suelen ser graves, se deben tener en cuenta para poder indicarun tratamiento adecuado

INTRODUCTION AND DEVELOPMENT: Psychiatric symptoms in Parkinson's disease are relatively frequent, with a variation between 10 and 50%, according to different publications. They generally occur after several years of treatment. They may be symptoms derived from the disease itself or linked to the treatment used. These complications vary and include anxiety, affective and psychotic (hallucinations, delirium) disorders, as well as other alterations such as sexual, sleep and cognitive problems. CONCLUSIONS: Treatment must always be tailor-made to suit each individual and in many cases we have to resort to lowering or withdrawing the dopaminergic medication while, in others, treatment with benzodiazepines, antidepressants and atypical neuroleptic drugs is indicated.

El adenoma hepático (AH) es una proliferación benigna de hepatocitos, que suele presentarse en mujeres con antecedentes de toma prolongada de anticonceptivos orales. Es muy infrecuente en varones. Actualmente las pruebas diagnósticas más útiles son la TC helicoidal y la resonancia magnética. El tratamiento es la resección hepática, tanto en los casos de AH sintomáticos como en los asintomáticos, por el riesgo de crecimiento, rotura y transformación maligna (AU)

La actitud tradicional ante un carcinoma no microcítico de pulmón con diseminación extratorácica era la abstención quirúrgica. Actualmente, en el caso de las metástasis suprarrenales, se ha visto que es posible obtener supervivencias prolongadas en algunos pacientes tras la resección del tumor primario y la metástasis. Presentamos el caso de un paciente de 62 años con una metástasis única suprarrenal derecha de un cáncer de pulmón resecado (neumonectomía izquierda) 5 meses antes. Se trató con quimioterapia neoadyuvante y suprarrenalectomía, y sigue libre de enfermedad 27 meses después. Se discuten las opciones de tratamiento y las indicaciones (AU)

Propósito: Describir una complicación infrecuente de los catéteres venosos centrales permanentes, así como su tratamiento. Material y métodos: Paciente de 78 años, en tratamiento por adenocarcinoma de pulmón, que requiere recolocación quirúrgica del catéter venoso central por migración del mismo a yugular interna. Se analizan las posibles causas y consecuencias de esta situación, así como las opciones terapéuticas. Conclusiones: La migración del catéter es una complicación infrecuente, de causa desconocida y tratamiento relativamente sencillo, que se diagnostica en la radiografía de tórax, y que puede originar otras complicaciones mayores, principalmente trombosis venos (AU)

La hipoglucemia causada por tumores de células no insulares es un fenómeno infrecuente, que puede presentarse en diversos tipos de tumores (epiteliales, mesenquimales, hematopoyéticos, neuroendocrinos) y que actualmente se atribuye a la producción de un péptido con acción insulínica (IGF-II) por parte de éstos. Presentamos un caso de hipoglucemia de ayuno debida a un tumor mesenquimal (hemangiopericitoma de localización retroperitoneal), y se discuten las características del tumor y el síndrome paraneoplásico, así como las posibilidades de tratamiento (AU)

This review article deals with the psychiatric complications of L-dopa in Parkinson's disease. There is a 20% incidence of these complications with estimations varying in the different publications between 10% and 50%. They usually occur after several years of treatment. The complications are varieds and include anxiety states, affective disorders, psychoses, disorders of recognition and other mental disorders. They may be explained in physio-pathological terms as an alteration of the dopaminergic system of the mesolimbic and mesocortical circuits. Treatment should be adjusted individually. In most cases the dosage of levodopa should be lowered or stopped altogether. In other patients treatment with benzodiazepines, neuroleptic antidepressants and even electric shock treatment may be necessary.

The present trial has been conducted in order to determine familia aggregation and the relationship between high blood pressure, inheritance and environmental factors. We studied 108 subjects (36 high blood pressure patients, 36 controls and 36 first degree relatives) ranging in age from 21 to 55 years. This case-control study was undertaken to determine risk factors. Multivariance analysis with logistic regression was carried out. Our results showed hereditary factors in 73.8% of the cases. Being the main gene autosomal recessive. Thus the main factor for high pressure is hereditary predisposition. Others factor are, body mass index and the coefficient waist-hip. We found no correlation with serum lipids.
