Innovations in prevention of infections in oncological megaprostheses: a narrative review.

Infection is a significant complication in oncological megaprostheses. The purpose of our study is to indagate the innovations and new trend about the prevention of infection in this kind of surgery. The research focused on the use of antimicrobic prophylaxis, the defensive antibacterial coating and the use of silver coated.

Ankle fracture and necrotizing fasciitis: a common fracture and a dreadful complication.

Necrotizing fasciitis is a dreadful complication of the soft tissue. This pathology could be triggered by many factors, such as a fracture. We present a case of case of a necrotizing fasciitis in ankle fracture.

A rare case of post-traumatic infected pilomatricoma of the finger of the hand diagnosed after performing radioiodine therapy.

We present the clinical case of a young woman with pilomatricoma of the finger, a very rare location. The patient got infected after receiving radioiodine therapy to treat a thyroid carcinoma. Given the patient's high functional requirements we choose a minimal treatment which allowed her to maintain a sufficient functionality.

Infections in primary intramedullary nailing of open tibial fractures: a review article.

OBJECTIVE: Open fractures are insidious and life-threatening injuries. They frequently involve the leg. Our aim is to evaluate the efficacy of primary intramedullary nailing as a definitive treatment of open tibial fractures, compared to other fixation tools in terms of deep infections, healing fracture time and bony repair. PATIENTS AND METHODS: We reviewed the available literature concerning treatment and management of open tibial fractures. RESULTS: Primary intramedullary nailing is similar to delayed intramedullary nailing after external fixation in terms of union, malunion and nonunion rate. Furthermore, primary intramedullary nailing is superior compared to all other fixation tools, in terms of development of deep infections, provided that a good debridement of soft tissue, lavage of the fracture site and adequate antibiotic prophylaxis should be performed before nailing. CONCLUSIONS: Primary intramedullary nailing should be considered for the definitive treatment of open tibial fractures, providing many advantages in terms of rehabilitation, time of hospitalization and costs.

[Organizational and managerial model for the patient with chronic heart failure]. / Modello organizzativo e gestionale del paziente con scompenso cardiaco cronico.

Patients with chronic heart failure show a range of care and global therapeutic management problems. The main elements for an efficient management are: hospital-territory integration based on a structured mode of operating and on the healthcare professional/patient relationship; rational continuity in the treatment; multi-disciplinarity of the interventions; the patient's active participation. In the light of these considerations the authors report their own direct experience describing the organizational and management model for patients with chronic heart failure during the various phases of treatment, from the global functional evaluation and treatment program to homecare management integrated with the general practitioner.
