Introdução: O questionário Internacional de Atividade Física (IPAQ) tem sido amplamente utilizado nas pesquisas. É considerado uma boa ferramenta de medição de Atividade Física (AF), no entanto, este questionário não é específico para idosos, geralmente, é aplicado à população em geral. Objetivo: Realizar uma Revisão Integrativa da Literatura apresentando os estudos que investigaram a validade do IPAQ na avaliação de AF em idosos nos últimos seis anos. Métodos: Foi realizada uma Revisão Integrativa da Literatura nas bases de dados PUBMED, BVS e SCOPUS, relativa aos anos de 2014 a 2020, utilizando-se os descritores: Elderly, Aged, IPAQ questionnaire validity, Physical Activity e Exercise. Resultados: Foram selecionados oito artigos cuja metodologia apresenta a validade do IPAQ, na avaliação em idosos com idade ≥65 anos. Seis dos artigos selecionados apresentaram em suas metodologias a avaliação realizada pelo IPAQ-L (versão longa), e apenas dois artigos apresentaram IPAQ-S (versão curta). Conclusão: O IPAQ- L tem sido amplamente utilizado para avaliar os níveis de AF e comportamento sedentário (CS) em idosos. As evidências demonstram que a aplicação do questionário combinado com os dados do acelerômetro, tem mostrado medições satisfatórias, no entanto, futuras versões devem ser estruturadas para atender a este público específico.

PURPOSE AND OBJECTIVES: Given their role in homing immune cells to the intestine, CC motif chemokine receptor 9 (CCR9) and its specific ligand CC motif chemokine ligand 25 (CCL25) are interesting candidate genes for celiac disease. These genes are located in regions previously shown to be associated with or linked to celiac disease, but no investigations on their association with various celiac disease phenotypes have so far been conducted. Here we studied such associations of both genotyped and imputed single nucleotide polymorphisms (SNPs) with either regulatory function or exonic location of the CCR9 and CCL25 loci. RESULTS: Exploiting a carefully phenotyped cohort of 625 celiac disease patients and 1817 non-celiac controls, we identified that multiple SNPs with predicted regulatory function (RegulomeDB score ≤3a and/or eQTL effect) located between 100 kB upstream and downstream of CCR9 and CCL25 are associated with celiac disease and/or selected phenotypes. Of the genotyped SNPs in the CCR9 loci, rs213360 with an eQTL effect on CCR9 expression in blood was associated with celiac disease and all investigated phenotypes except high HLA risk. Rs1545985 with an eQTL on CCR9 expression and rs7652331 and rs12493471, both with RegulomeDB score ≤3a, were all associated with gastrointestinal symptoms and malabsorption and the latter additionally with anemia. The genotyped CCL25 SNPs rs952444 and rs882951, with RegulomeDB scores 1d and 1f respectively and eQTL effect on CCL25 expression in small intestine, were associated with gastrointestinal symptoms and malabsorption. The CCL25 SNP rs2303165 identified in sequencing followed by imputation was associated with partial villous atrophy. However, the association did not pass the permutation based multiple testing correction (PEMP2 > 0.05). CONCLUSIONS: We conclude that SNPs in the region of CCR9 and CCL25 with predicted functional effect or exonic localization likely contribute only modestly to various celiac disease phenotypes.

BACKGROUND: Family screening has been advocated as a means to reduce the major underdiagnosis of coeliac disease. However, the precise risk of the disease in relatives and the impact of patient- and relative-related individual factors remain obscure. AIMS: To investigate the individual risk of coeliac disease among patients' relatives. METHODS: Altogether 2943 relatives of 624 index patients were assessed for the presence of previous coeliac disease diagnosis, or were screened for the disease. Coeliac disease-associated human leucocyte antigen (HLA) genotype was determined from all participants. The association between individual factors and new screening positivity was assessed by logistic regression. RESULTS: There were 229 previously diagnosed non-index relatives with coeliac disease and 2714 non-affected (2067 first-degree, 647 more distant) relatives. Of these 2714 relatives, 129 (4.8%) were screening-positive (first-degree 5.1%, second-degree 3.6%, more distant 3.5%). The combined prevalence of the previously diagnosed and now detected cases in relatives was 12.2% (6.3% clinically detected, 5.9% screen-detected). In univariate analysis, age <18 years at diagnosis (odds ratio 1.60, 95% CI 1.04-2.45) in index, and age 41-60 years (1.73, 1.10-2.73), being a sibling (1.65, 1.06-2.59) and having the high-risk genotype (3.22, 2.01-5.15 DQ2.5/2.5 or DQ2.5/2.2 vs other risk alleles) in relatives were associated with screening positivity. Only high-risk HLA remained significant (2.94, 1.80-4.78) in multivariable analysis. CONCLUSIONS: Unrecognised coeliac disease was common among at-risk relatives even in a country with an active case-finding policy, and also in relatives more distant than first-degree. The presence of a high-risk genotype was the most important predictor for screening positivity. ClinicalTrials.gov identifier NCT03136731.

The phenotype of coeliac disease varies considerably for incompletely understood reasons. We investigated whether established coeliac disease susceptibility variants (SNPs) are individually or cumulatively associated with distinct phenotypes. We also tested whether a polygenic risk score (PRS) based on genome-wide associated (GWA) data could explain the phenotypic variation. The phenotypic association of 39 non-HLA coeliac disease SNPs was tested in 625 thoroughly phenotyped coeliac disease patients and 1817 controls. To assess their cumulative effects a weighted genetic risk score (wGRS39) was built, and stratified by tertiles. In our PRS model in cases, we took the summary statistics from the largest GWA study in coeliac disease and tested their association at eight P value thresholds (PT) with phenotypes. Altogether ten SNPs were associated with distinct phenotypes after correction for multiple testing (PEMP2 ≤ 0.05). The TLR7/TLR8 locus was associated with disease onset before and the SH2B3/ATXN2, ITGA4/UBE2E3 and IL2/IL21 loci after 7 years of age. The latter three loci were associated with a more severe small bowel mucosal damage and SH2B3/ATXN2 with type 1 diabetes. Patients at the highest wGRS39 tertiles had OR > 1.62 for having coeliac disease-related symptoms during childhood, a more severe small bowel mucosal damage, malabsorption and anaemia. PRS was associated only with dermatitis herpetiformis (PT = 0.2, PEMP2 = 0.02). Independent coeliac disease-susceptibility loci are associated with distinct phenotypes, suggesting that genetic factors play a role in determining the disease presentation. Moreover, the increased number of coeliac disease susceptibility SNPs might predispose to a more severe disease course.

This research focuses on the adjustments in systolic and diastolic functions that are not fully understood in dogs submitted to athletic training. Beagle dogs carried out an endurance training program (ETP) prescribed from the external training load, corresponding to 70-80% of the lactate threshold (VLT) velocity. Eighteen dogs were randomly assigned to two groups: control (C, n = 8), active dogs that did not perform any forced exercise, and trained (T, n = 10), submitted to the ETP during eight weeks. All dogs were evaluated before and after the ETP period using two-dimensional echocardiography, M-mode, Doppler, and two-dimensional speckle tracking. A principal component analysis (PCA) of the echocardiographic variables was performed. The ETP improved the left ventricular internal dimension at the end of diastole (LVDd), the left ventricular internal dimension at the end of diastole to aorta ratio (LVDd: Ao), and the strain rate indices. PCA was able to capture the dimensionality and qualitative echocardiography changes produced by the ETP. These findings indicated that the training prescribed based on the lactate threshold improved the diastolic and systolic functions. This response may be applied to improve myocardial function, promote health, and mitigate any injuries produced during heart failure.

The dogs' responses to training exercise are seldom monitored using physiological variables, and cardiac autonomic regulation (CAR) is a relevant determinant of endurance-training adaptation. There are studies in the literature establishing that regular exercise could interfere with CAR in dogs, measured by heart rate and vagal-derived indexes of heart-rate-variability (HRV). However, few studies were found using a prescribed training program based on the lactate threshold (LT) to determine HRV by a 24-h Holter analysis. The purpose of this study was to test whether an endurance-training program (ETP) guided individually by LT raises time-domain measures of HRV in healthy Beagle dogs. Twenty dogs were assigned to two groups: control (C) and trained (T). The dogs from group T underwent an incremental exercise test (IET) to determine their LT. Both LT and velocity corresponding to the LT (VLT) was determined by visual inspection. T group performed an eight-week endurance-training program consisting of treadmill runs set to 70-80% of the VLT. Next, dogs from the group T have submitted to IET again. The maximal velocities (Vmax) at which achieved by the trained dogs in both IETs were determined. The group S did not undergo IETs or ETP. HRV was determined by the 24-hour-Holter at rest, before and on the 2°, 4°, 6° and 8° training weeks. To examine the HR impact on HRV, standard HRV variables were normalized to prevailing HR. VLT and Vmax rose in group T, indicating an improvement of dogs' aerobic and anaerobic capacity. The normalized standard HRV indexes were relatively attenuated since these variables had a reduction in the degree of correlation concerning an average HR. The ETP resulted in decreased resting heart rate and increased time-domain indices, highlighting the log-transformed square root of the mean sum of the squared differences between R-R intervals (Ln rMSSD). The lactate-guided endurance-training program could lead to better parasympathetic cardiac modulation in Beagle dogs.

OBJECTIVE To determine whether the lactate threshold of dogs could be determined by a visual method and to assess the extent of agreement and bias among treadmill velocities for the lactate threshold as determined by visual (LTv) and polynomial (LTp) methods, glucose threshold as determined by visual (GTv) and polynomial (GTp) methods, and heart rate deflection point (HRdp) as a method for estimating the aerobic capacity of dogs. ANIMALS 18 healthy adult Beagles. PROCEDURES Each dog underwent a standardized incremental treadmill exercise test once. The test ended when the dog began to show signs of fatigue. Plasma lactate and glucose concentrations and heart rate (HR) were plotted against exercise intensity (treadmill velocity) for the duration of the test, and the LTv, GTv, and HRdp were determined visually. The LTp and GTp were determined by means of a second-order polynomial function. One-way ANOVA, Pearson correlation, Bland-Altman analyses, and ordinary least products regression were used to assess the extent of agreement and bias among the various threshold velocities. RESULTS Mean velocity did not differ significantly among the thresholds evaluated. There was a strong positive correlation between the LTv velocity and the velocity for GTv (r = 0.91), LTp (r = 0.96), GTp (r = 0.94), and HRdp (r = 0.95). CONCLUSIONS AND CLINICAL RELEVANCE Results indicated that LTv could be determined for dogs undergoing intense exercise on a treadmill, and the treadmill velocity corresponding to the LTv was associated with the velocity for the other hallmarks of endurance. Thus, that method may be useful for prescription and evaluation of conditioning programs for dogs.

Few data exist on the physiological consequences of an incremental exercise test (IET) and training in dogs. Here we evaluated the effect of an endurance-training program (ETP) on the kinetics of the serum biomarkers: creatine kinase (CK), aspartate aminotransferase (AST), myoglobin, cardiac troponin I (cTnI), and creatine kinase isoenzyme MB (CK-MB). Eighteen Beagle dogs were assigned to three groups: sedentary (S), untrained (U), and trained (T). The dogs from the groups T and U underwent IET, and their lactate threshold (LT) was determined. The LT and velocity corresponding to the LT (VLT) was determined by visual inspection. The VLT was utilized to determine the training intensity level. Thus, the dogs from group T underwent an eight-week aerobic conditioning program with the intensity training set to 70-80% of the VLT. Next, dogs from the groups T and U have submitted to IET again. The group S did not undergo IETs or training. The maximal velocity (Vmax) at which the dog achieves in the IET and heart rate were determined. For biomarkers, venous blood samples were collected before the experimental procedure (baseline) and before and 1, 6, 12, 24, 48, and 72 h after the IETs. VLT and Vmax increased in group T (P < .05), indicating an improvement in dogs' aerobic and anaerobic fitness. CK and AST rose (P < .05), peaking 6 h after both IETs and returning to baseline levels after 12-24 h. Levels of cTnI and myoglobin were unaltered. CK-MB peaked 1 h after the IETs and returned to baseline levels after 12 h (P < .05). We concluded that ETP improved the aerobic capacity of the dogs without any skeletal or cardiac muscle injury.

This article proposes a women's health prevention indicator (WHPI) reflecting the combined status of mammography and Papanicolaou (Pap) smear according to the recommendations for age and considering the time elapsed since the last exam/test. The WHPI classifies prevention status into desirable, alert, or risk categories. The risk category includes women of all ages who never had a Pap smear, those aged >60 years who had a Pap smear more than 3 years ago but never had a mammography, and those aged ≥71 years who are up to date with the Pap smear but never had a mammography. The desirable category includes women with a Pap smear in the past 3 years, except women aged ≥41 who never had a mammography and those aged ≥51 years who had a mammography more than 2 years earlier. The alert category includes women whose Pap smear is more than three years old with the exception of those ≥61 years who never had a mammography and those aged ≥71 years whose mammography is more than 2 years old. For women who had experienced a Pap smear in the past 3 years, the alert category includes those aged 41-50 years who never had mammography, those aged 51-70 years with mammography older than 2 years or no mammography, and those aged ≥71 years with mammography older than 2 years. Applying the WHPI to data from the National Household Sample Survey of 2008 revealed that 24.8% of Brazilian women were at risk and 24.2% were in the alert category. The Northeast and the North had the highest risk rates (31.5% and 29.6% respectively). Of those >70 years old, 49.5% were in the risk category. The WHPI can be used to assess public initiatives and compare preventive status within and across regions.

En este artículo se propone un indicador preventivo de salud de la mujer (IPSM) que indique de manera combinada la situación respecto a la realización de una mamografía y de una prueba de Papanicolaou, según las recomendaciones para la edad y considerando el tiempo transcurrido desde el último examen. La situación de salud preventiva se clasificó en deseable, en alerta y en riesgo. Están en riesgo las mujeres de cualquier edad que nunca se han realizado la prueba de Papanicolaou, las mayores de 60 años que se la hicieron hace más de tres años, pero que nunca se han hecho una mamografía, y las de 71 años o más que están al día con el Papanicolaou, pero nunca se han hecho una mamografía. Están en una situación deseable las mujeres que se han hecho el Papanicolaou hace menos de 3 años, excepto las de 41 años o más que nunca se han hecho una mamografía y las de 51 años o más que hace más de dos años que se han hecho una mamografía. La situación de alerta se aplica a las mujeres que se han hecho el Papanicolaou hace más de 3 años, excepto las de 61 años o más que nunca se han sometido a una mamografía y las de 71 años o más que hace más de dos años que se hicieron una mamografía. Entre las que se han hecho el Papanicolaou hace menos de tres años, están en situación de alerta las mujeres de 41 a 50 años que nunca se han hecho una mamografía, las de 51 a 70 años que se han hecho una mamografía hace más de dos años y las de 71 años o más que hace más de dos años que se hicieron una mamografía. Al aplicar el IPSM a los datos de la encuesta nacional por muestra de hogares del 2008, se constató que un 24,8% de las mujeres brasileñas estaban en situación de riesgo y un 24,2% en situación de alerta. Las zonas nordeste y norte presentaban las mayores proporciones de riesgo (31,5% y 29,6%, respectivamente). En las mujeres mayores de 70 años, el 49,5% estaba en situación de riesgo. El IPSM puede utilizarse para evaluar acciones públicas y para comparar el patrón de prevención dentro de cada zona y entre las distintas zonas.

The risk category includes women of all ages who never had a Pap smear, those aged >60 years who had a Pap smear more than 3 years ago but never had a mammography, and those aged ≥71 years who are up to date with the Pap smear but never had a mammography. The desirable category includes women with a Pap smear in the past 3 years, except women aged ≥41 who never had a mammography and those aged ≥51 years who had a mammography more than 2 years earlier. The alert category includes women whose Pap smear is more than three years old with the exception of those ≥61 years who never had a mammography and those aged ≥71 years whose mammography is more than 2 years old. For women who had experienced a Pap smear in the past 3 years, the alert category includes those aged 41-50 years who never had mammography, those aged 51-70 years with mammography older than 2 years or no mammography, and those aged ≥71 years with mammography older than 2 years. Applying the WHPI to data from the National Household Sample Survey of 2008 revealed that 24.8% of Brazilian women were at risk and 24.2% were in the alert category. The Northeast and the North had the highest risk rates (31.5% and 29.6% respectively). Of those >70 years old, 49.5% were in the risk category. The WHPI can be used to assess public initiatives and compare preventive status within and across regions.(AU)


RESUMEN En este artículo se propone un indicador preventivo de salud de la mujer (IPSM) que indique de manera combinada la situación respecto a la realización de una mamografía y de una prueba de Papanicolaou, según las recomendaciones para la edad y considerando el tiempo transcurrido desde el último examen. La situación de salud preventiva se clasificó en deseable, en alerta y en riesgo. Están en riesgo las mujeres de cualquier edad que nunca se han realizado la prueba de Papanicolaou, las mayores de 60 años que se la hicieron hace más de tres años, pero que nunca se han hecho una mamografía, y las de 71 años o más que están al día con el Papanicolaou, pero nunca se han hecho una mamografía. Están en una situación deseable las mujeres que se han hecho el Papanicolaou hace menos de 3 años, excepto las de 41 años o más que nunca se han hecho una mamografía y las de 51 años o más que hace más de dos años que se han hecho una mamografía. La situación de alerta se aplica a las mujeres que se han hecho el Papanicolaou hace más de 3 años, excepto las de 61 años o más que nunca se han sometido a una mamografía y las de 71 años o más que hace más de dos años que se hicieron una mamografía. Entre las que se han hecho el Papanicolaou hace menos de tres años, están en situación de alerta las mujeres de 41 a 50 años que nunca se han hecho una mamografía, las de 51 a 70 años que se han hecho una mamografía hace más de dos años y las de 71 años o más que hace más de dos años que se hicieron una mamografía. Al aplicar el IPSM a los datos de la encuesta nacional por muestra de hogares del 2008, se constató que un 24,8% de las mujeres brasileñas estaban en situación de riesgo y un 24,2% en situación de alerta. Las zonas nordeste y norte presentaban las mayores proporciones de riesgo (31,5% y 29,6%, respectivamente). En las mujeres mayores de 70 años, el 49,5% estaba en situación de riesgo. El IPSM puede utilizarse para evaluar acciones públicas y para comparar el patrón de prevención dentro de cada zona y entre las distintas zonas.(AU)

RESUMO

Conhecer a produção bibliográfica acerca do assunto e analisar a prática da enfermagem na promoção desses direitos a partir da bibliografia levantada. Métodos: Trata-se de um estudo qualitativo de natureza exploratória do tipo pesquisa bibliográfica. Resultados: O levantamento ocorreu nas bases dedados da BIREME. Foram encontrados 114 artigos de 2004 a 2008. Após o refinamento que consistiu na retirada de artigos repetidos em base de dados diferentes e afinidade com o tema proposto e conseqüentemente sua relevância para o estudo, o número total de artigos selecionados para entrarem no processo de análise consistiu em 46. Conclusão: Os resultados mostram uma incipiente produção nesta área pela enfermagem. Refletindo assim, não só a necessidade de aprofundamento do tema pelos profissionais de saúde como também sua posterior publicação como fruto científico.


Objectives: To know literature about sexual and reproductive rights, and to discuss the practice of nursing in the promotion of these rights. Methods: This is an exploratory study of qualitative nature based on bibliographic research. Results: The research was done in BIREME database. 114 articles were found between 2004 and 2008. After refinement, which consisted in taking off repeated articles found on differentdatabases affinity with the proposed topic and consequently their relevance to the study, the total number of articles selected to enter the review process consisted of 46. Conclusion: The results show an incipient production in this area, reflecting not only the need for deepening knowledge about this the theme by health professionals as well as its subsequent publication as a scientific production.


Conocer la producción bibliográfica sobre el tema y analizar la práctica de la enfermería en la promoción de estos derechos a partir de la bibliografía revisada. Métodos: Se realizó un estudio cualitativo de naturaleza exploratora de tipo investigación bibliográfica. Resultados: La búsqueda se realizó en las bases de datos de BIREME. Fueron encontrados 114 artículos de 2004 a 2008. Después del refinamiento queconsistió en la retirada de artículos repetidos en diferentes bases de datos y afinidad con el tema propuesto y por lo tanto su importancia para el estudio, el número total de artículos seleccionados para entrar en el proceso de revisión consistió en 46. Conclusión: Los resultados muestran una incipiente producción en esta área por la enfermería. Lo que refleja la necesidad de profundizar el tema por los profesionales de la salud, así como también su posterior publicación como resultado científico.
