OBJECTIVE: Effect of photobiomodulation therapy (PBMT) in patients with CCLR after TPLO surgery by measuring C-reactive protein (CRP), percentage weight bearing, lameness using a short form of a composite measure pain scale, evaluated by the clinician and owners, and surgical site infection. SAMPLE: 54 client-owned dogs with CCLR undergoing unilateral TPLO surgery were enrolled in this study between April 5, 2021, through April 10, 2022. METHODS: The study population was randomly assigned to either a treatment group receiving PMBT (24 dogs) or a control group (30 dogs). PMBT was performed on the treatment group immediately after induction, and 6 hours, 24 hours, 48 hours, and 8 weeks postoperatively. The control group received sham PMBT (device turned off) at the same time. Evaluation of CRP, CMPS-SF, evidence of SSI, and %WB were evaluated for all dogs 24 hours preoperatively, and then 24 hours, 48 hours, and 8 weeks postoperatively. Owners completed CMPS-SF and subjective evaluations weekly for 8 weeks postoperatively. RESULTS: No statistically significant differences were found between treatment groups when evaluating CRP, %WB, and CMPS-SF by the clinician and weekly evaluation of the CMPS-SF by owners. Although no statistically significant differences were found in patients developing surgical site infections between treatment groups, SSI was only observed in patients in the control group (5/30, 16.6%). Most were minor/superficial infections (4/30 13.3%), and a single dog (1/30, 3.3%) had a major/deep surgical site infection. CLINICAL RELEVANCE: Although with promising but not statistically significant differences between groups, surgical site infections may be reduced after PBMT application.

This article summarizes the clinical practice guideline (CPG) for the management of acute pancreatitis in the Social Security of Peru (EsSalud), to provide evidence-based clinical recommendations for the management of acute pancreatitis in EsSalud. A guideline development group (GEG) was formed that included medical specialists and methodologists. The GEG formulated 7 clinical questions to be answered by this CPG. Systematic searches of systematic reviews and -when considered relevant- primary studies were carried out in PubMed during 2022. The evidence was selected to answer each of the clinical questions posed. The certainty of the evidence was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. In periodic work meetings, the GEG used the GRADE methodology to review the evidence and formulate the recommendations, the points of good clinical practice, and the corresponding flow charts. Finally, the CPG was approved with Resolution No. 105-IETSI-ESSALUD-2022. This CPG addressed 7 clinical questions on fluid therapy, start of enteral nutrition, analgesia, type of nutrition, antibiotic, and surgical treatment. Based on these questions, 8 recommendations (1 strong and 7 conditional), 13 BPCs, and 1 flowchart were formulated. This article summarizes the methodology and evidence-based conclusions of the CPG for the management of acute pancreatitis in EsSalud.

El presente artículo resume la guía de práctica clínica (GPC) para el manejo de la pancreatitis aguda en el Seguro Social del Perú (EsSalud). Su objetivo es proveer recomendaciones clínicas basadas en evidencia para el manejo de la pancreatitis aguda en EsSalud. Se conformó un grupo elaborador de la guía (GEG) que incluyó médicos especialistas y metodólogos. El GEG formuló 7 preguntas clínicas a ser respondidas por la presente GPC. Se realizó búsquedas sistemáticas de revisiones sistemáticas y -cuando fue considerado pertinente- estudios primarios en PubMed durante el 2022. Se seleccionó la evidencia para responder cada una de las preguntas clínicas planteadas. La certeza de la evidencia fue evaluada usando la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE). En reuniones de trabajo periódicas, el GEG usó la metodología GRADE para revisar la evidencia y formular las recomendaciones, los puntos de buenas prácticas clínicas y flujogramas correspondientes. Finalmente, la GPC fue aprobada con Resolución N° 105-IETSI-ESSALUD-2022. La presente GPC abordó 7 preguntas clínicas sobre fluidoterapia, momento de inicio de nutrición enteral, analgesia, tipo de nutrición, tratamiento antibiótico y quirúrgico. En base a dichas preguntas se formularon 8 recomendaciones (1 fuerte y 7 condicionales), 13 BPC, y 1 flujograma. El presente artículo resume la metodología y las conclusiones basadas en evidencia de la GPC para el manejo de la pancreatitis aguda en EsSalud.

In this paper, the 4E assessment (Energetic, Exergetic, Exergoeconomic and Exergoenvironmental) of a low-temperature ORC activated by two different alternatives is presented. The first alternative (S1) contemplates the activation of the ORC through the recovery of waste heat from a flash-binary geothermal power plant. The second alternative (S2) contemplates the activation of the ORC using direct heat from a geothermal well. For both alternatives, the energetic and exergetic models were established. At the same time, the economic and environmental impact models were developed. Finally, based on the combination of the exergy concepts and the economic and ecological indicators, the exergoeconomic and exergoenvironmental performances of the ORC were obtained. The results show higher economic, exergoeconomic and exergoenvironmental profitability for S1. Besides, for the alternative S1, the ORC cycle has an acceptable economic profitability for a net power of 358.4 kW at a temperature of 110 °C, while for S2, this profitability starts being attractive for a power 2.65 times greater than S1 and with a temperature higher than 135 °C. In conclusion, the above represents an area of opportunity and a considerable advantage for the implementation of the ORC in the recovery of waste heat from flash-binary geothermal power plants.

Mexicans and Mexican Americans share culture, genetic background, and predisposition for chronic complications associated with obesity and diabetes making imperative efficacious treatments and prevention. Obesity has been treated for centuries focused-on weight loss while other treatments on associated conditions like gout, diabetes (T2D), and hypertriglyceridemia. To date, there is no systematic review that synthesizes the origin of obesity clinics in Mexico and the efforts to investigate treatments for obesity tested by randomized clinical trials (RCT). We conducted systematic searches in Pubmed, Scopus, and Web of Science to retrieve anti-obesity RCT through 2019 and without an inferior temporal limit. The systematic review included RCT of anti-obesity treatments in the Mexican adult population, covering alternative medicine, pharmacological, nutritional, behavioral, and surgical interventions reporting metabolism-associated traits such as BMI, weight, waist circumference, triglycerides, glucose, among others. Only the studies with at least 3 months of treatment were included in the meta-analyses in order to reduce placebo effects. We found 634 entries, after removal of duplicates and screening the studies based on eligibility criteria, we analyzed 43 national, and 2 multinational-collaborative studies. Most of the national studies had small sample sizes, and the implemented strategies do not have replications in the population. The nutrition/behavioral interventions were difficult to blind, and most studies have medium-to-high risk of bias. Nutritional/behavioral interventions and medications showed effects on BMI, waist circumference, and blood pressure. Simple measures like pure water instead of sweet beverages decrease triglycerides and systolic blood pressure. Dark chocolate showed the highest effect for BMI and high blood pressure, and treatment with insulin increased weight in those with T2D. The study of obesity in Mexico has been on-going for more than four decades, the interest on RCT just increased until this millennium, but with small sample sizes and lack of replication. The interventions affect different cardiometabolic associated traits, which should be analyzed in detail in the population living near the Mexico-U.S. border; therefore, bi-national collaboration is desirable to disentangle the cultural effects on this population's treatment response. Understanding the assembly processes of symbiont communities, including viromes and microbiomes, is important for improving predictions on symbionts' biogeography and disease ecology. Here, we use phylogenetic, functional, and geographic filters to predict the similarity between symbiont communities, using as a test case the assembly process in viral communities of Mexican bats. We construct generalized linear models to predict viral community similarity, as measured by the Jaccard index, as a function of differences in host phylogeny, host functionality, and spatial co-occurrence, evaluating the models using the Akaike information criterion. Two model classes are constructed: a "known" model, where virus-host relationships are based only on data reported in Mexico, and a "potential" model, where viral reports of all the Americas are used, but then applied only to bat species that are distributed in Mexico. Although the "known" model shows only weak dependence on any of the filters, the "potential" model highlights the importance of all three filter types-phylogeny, functional traits, and co-occurrence-in the assemblage of viral communities. The differences between the "known" and "potential" models highlight the utility of modeling at different "scales" so as to compare and contrast known information at one scale to another one, where, for example, virus information associated with bats is much scarcer.

The epidemiological behavior of six Leptospira serovarieties was analyzed by spatial autocorrelation and co-occurrence of leptospirosis, diagnosed in goat herds located in the State of Guanajuato, Mexico. A total of 1650 goat serum samples were analyzed by microscopic agglutination (MAT). True prevalence (Pv) and 95% confidence interval (CI95) were determined. Spatial autocorrelation was calculated using the spdep package, applying the global Moran's I and local Moran's I of Leptospira in Guanajuato. The probabilistic model of co-occurrence was applied using the co-occur package. Seroprevalence in the State was found to be 45.5% (CI95 42.96; 48.06%). The highest registered frequency was for the Icterohemorrhagiae serovar (Pv 34.16%; CI95 31.74, 36.65%), followed by the serovar Hardjo-prajitno (Pv: 6.77%; CI95 5.33, 8.40%). Other serovarieties showed a Pv < 5%. Global spatial autocorrelation, only for the Icterohemorrhagiae serovar, was I > 1, while local Moran's I revealed that five of the six Leptospira serovarieties were spatially correlated. The probabilistic model of co-occurrence detected negative associations between Icterohemorrhagiae and the other serovarieties. The current study demonstrates the presence of Leptospira in goat herds of the State of Guanajuato. The diagnosed serovarieties show an aggregation pattern associated to risk zones and disease-transmitting vectors. Antibody co-occurrence analysis revealed dominance of the Icterohemorrhagiae serovar. A multidisciplinary approach including spatial epidemiology, ecological analyses, and serological vigilance will generate useful information for the prevention and control of leptospirosis in caprine production units.

La epidemia de COVID-19 ha modificado la cultura de la comunicación. La solución para los problemas de salud puede ser asertiva cuando es consensuada. El método Delphi es una herramienta de consenso que emplea rondas de listas de preguntas para recopilar información del conocimiento de un panel de expertos que analizan planteamientos y posibles soluciones a problemas. Se basa en la premisa de que, con la libertad del anonimato, la inteligencia combinada mejora el juicio individual y captura la opinión colectiva experta. El proceso del método es muy flexible, pues las rondas de preguntas pueden realizarse de manera presencial o remota. En este artículo se describe cómo implementar el método Delphi convencional en tiempos de confinamiento, y se analizan la utilidad y las limitaciones del método para su uso por expertos en salud para la resolución de problemas de tratamiento, diagnóstico o administrativos. Las tecnologías actuales para recolectar los datos permiten gran flexibilidad en el formato de los cuestionarios y facilitan la recopilación de la opinión experta. Gracias a su adaptabilidad, el método Delphi se está convirtiendo en una estrategia popular que involucra los ámbitos cualitativo y cuantitativo.

El deficiente hábito de lectura de los estudiantes que llegan a la educación superior no solo afecta su cultura general sino repercute significativamente en sus aptitudes investigativas,; demostrando desinterés por las investigaciones, en ocasiones carentes de innovaciones carentes de pensamiento crítico, e incluso reflexivo, lo que limita su papel proactivo., por ellos aplicamos como método una revisión del estado del arte en la temática. De este modo, el presente artículo tuvo como objetivo reflexionar acerca de la implementación de estrategias conscientes para incentivar la comprensión lectora en los estudiantes que ingresan a la educación superior. Se utilizaron métodos teóricos y empíricos como el análisis y la síntesis, el inductivo-deductivo, la revisión bibliográfica y la observación. Se abordaron temáticas relacionadas con la comprensión lectora, las aptitudes investigativas, y se estableció su relación con la docencia, mediante técnicas didácticas que incentivaran al estudiante a la lectura reflexiva y a la investigación acerca de temáticas de gran pertinencia social. Este estudio sustenta que el ámbito académico de la educación superior presenta un papel protagónico para inducir los hábitos y las aptitudes lectoras en los estudiantes. A través de situaciones polémicas, se incentiva la investigación como manera de desarrollar las capacidades necesarias para, una vez graduados, los estudiantes estén dotados de los conocimientos y las herramientas necesarias para transformar la sociedad., haciendo altamente pertinente su perfil profesional para el cual fue preparado(AU)


Poor reading habits of students who reach higher education not only affects their general culture but also has a significant impact on their research skills; for example, they show lack of interest in research, sometimes with little innovation or critical thinking capacities, and even at the reflective level, which limits their proactive role. Thus, the present article aimed to reflect on the implementation of conscious strategies to encourage reading comprehension in students entering higher education. Topics related to reading comprehension and research skills were addressed, while their relationship with teaching was established, through didactic techniques that would encourage students to use reflective reading and research on topics of great social relevance. This study sustains that the academic field of higher education plays a leading role in inducing reading habits and skills in students. Through controversial situations, research is encouraged as a way to develop the necessary skills so that, once they graduate, students are endowed with the knowledge and tools necessary to transform society, making their professional profile highly relevant, for which they were prepared(AU)

Parasites are strictly associated with their hosts and present a great diversity of life histories, often resulting in different diversity patterns than those observed in free-living species. However, ecological approaches have detected that, in some cases, mammal-associated helminths respond similarly to non-parasitic species in terms of diversity patterns. Using 2200 recorded interactions, we analysed the diversity patterns of helminths (Acanthocephala, Nematoda and Platyhelminthes) harbored by humans, wild and domestic mammals of Mexico, depending on latitude, host body mass and trophic guild (carnivore, herbivore, insectivore, omnivore), considering helminth richness and average taxonomic distinctness, and host phylogenetic diversity and phylogenetic clustering. Latitude was positively correlated with the average taxonomic distinctness encompassing the three parasite phyla and nematodes. Northern latitudes had less taxonomically related parasite assemblages. Host body mass had a significant negative relationship with the average taxonomic distinctness of acanthocephalans and the richness of helminths associated to wild hosts. The omnivore hosts had greater parasite richness, while insectivores had a less taxonomically related parasite assemblage and herbivores had a more heterogeneous parasite assemblage. Our results highlight the importance of incorporating different dimensions of diversity, such as average taxonomic distinctness and to consider the composition of parasite assemblages to better understand their diversity patterns.

The apicomplexan parasite Toxoplasma gondii (T. gondii) has been found in more than 350 species of homoeothermic vertebrates in diverse climates and geographic areas. In most animals, T. gondii produces mild or asymptomatic infection. However, acute and hyperacute toxoplasmosis is associated with high mortality rates observed in Neotropical primates (NP) in captivity. These primates are distributed in 20 countries across the Americas, and although infection has been reported in certain countries and species, toxoplasmosis in the wild and its impact on NP population survival is unknown. Differences among species in exposure rates and disease susceptibility may be due in part to differences in host behavior and ecology. Four species of NP are found in Costa Rica, i.e., howler (Alouatta palliata), spider (Ateles geoffroyi), capuchin (Cebus imitator), and squirrel monkeys (Saimiri oerstedii). This study reports NP exposure to T. gondii using the modified agglutination test in 245 serum samples of NP (198 wild and 47 from captivity) from Costa Rica. Associations of serostatus with environmental (forest cover, annual mean temperature), anthropogenic (human population density), and biological (sex) variables in howler and capuchin monkeys were evaluated. The seroprevalence among wild NP was 11.6% (95% CI = 7.7-17.34), compared with 60% in captive monkeys (95% CI = 44.27-73.63), with significant differences between species (X 2 = 20.072; df = 3, p = 0.000164), suggesting an effect of behavior and ecology. In general, antibody titers were low for wild NP (<1:128) and high for captive NP (>1:8192), suggesting higher exposure due to management factors and increased life span in captivity. Seropositivity in howler monkeys was positively related to forest cover and inversely related to annual rainfall. For capuchins, annual rainfall was inversely related to seropositivity. Surveillance of T. gondii exposure in NP in captivity and in the wild is required to understand drivers of the infection and develop novel strategies to protect them.

Mosquito-borne flaviviruses (MBFVs) are of public and animal health concern because they cause millions of human deaths annually and impact domestic animals and wildlife globally. MBFVs are phylogenetically divided into two clades, one is transmitted by Aedes mosquitoes (Ae-MBFVs) associated with mammals and the other by Culex mosquitoes (Cx-MBFVs) associated with birds. However, this assumption has not been evaluated. Here, we synthesized 79 published reports of MBFVs from wild mammals, estimating their host. Then, we tested whether the host specificity was biased to sampling and investigation efforts or to phylogenetic relationships using a viral phylogenetic tree drawn from analyzing whole flavivirus genomes obtained in GenBank. We found in total 18 flaviviruses, nine related to Aedes spp. and nine to Culex spp. infecting 129 mammal species. Thus, this supports that vectors are transmitting MBFV across available host clades and that ornithophilic mosquitoes are readily infecting mammals. Although most of the mosquito species are generalists in their host-feeding preferences, we also found a certain degree of MBFV's specificity, as most of them infect closely related mammal species. The present study integrates knowledge regarding MBFVs, and it may help to understand their transmission dynamics between viruses, vectors, and mammal hosts.

Hantaviruses are transmitted by rodents producing the hantavirus pulmonary syndrome (HPS) in the Americas. Today, no human cases of HPS have been reported in Mexico in spite of similar environmental conditions with Central America and the USA where several cases have occurred. To understand the current situation of hantaviruses in Mexico and the public health risk, a systematic review of studies was conducted reporting hantaviruses in rodents to known state seroprevalence and hantavirus genotypes. Simultaneously, this study identified the potential hantaviruses based on the phylogenetic diversity (PD) of hantaviruses reported in the Americas in hosts with the distribution in Mexico. A total 3862 rodents belonging to 82 species have been tested since 1999 to 2017. Overall, 392 individuals representing 43 rodent species were seropositive, and the seroprevalence ranged from 0 to 69.22%. Seven hantaviruses genotypes have been described in Mexico and three are zoonotic. Four host species of rodents are widely distributed in Mexico harboring the highest PD of viruses. According to the hosts distribution, 16 genotypes could be circulating in Mexico and some of these represent a potential risk for public health. This study proposed multidisciplinary and interinstitutional collaborations to implement systematic surveillance in rodents.

Bats and rodents are recognized to host a great diversity of viruses and several important viral zoonoses, but how this viral diversity is structured and how viruses are connected, shared and distributed among host networks is not well understood. To address this gap in knowledge, we compared the associative capacity of the host-virus networks in rodents and bats with the identification of those viruses with zoonotic potential. A virus database, detected by molecular methods, was constructed in the two taxonomic groups. We compiled 5,484 records: 825 in rodents and 4,659 in bats. We identified a total of 173 and 166 viruses, of which 53 and 40 are zoonotic viruses, in rodents and bats, respectively. Based on a network theory, a non-directed bipartite host-virus network was built for each group. Subsequently, the networks were collapsed to represent the connections among hosts and viruses. We identified both discrete and connected communities. We observed a greater degree of connectivity in bat viruses and more discrete communities in rodents. The Coronaviridae recorded in bats have the highest values of degree, betweenness and closeness centralities. In rodents, higher degree positions were distributed homogeneously between viruses and hosts. At least in our database, a higher proportion of rodent viruses were zoonotic. Rodents should thus not be underestimated as important reservoirs of zoonotic disease. We found that viruses were more frequently shared among bats than in rodents. Network theory can reveal some macroecological patterns and identify risks that were previously unrecognized. For example, we found that parvovirus in megabats and Gbagroube virus in rodents may represent a zoonotic risk due to the proximity to humans and other zoonotic viruses. We propose that epidemiological surveillance programmes should consider the connectivity of network actors as a measure of the risks of dispersion and transmission.

Influenza constitutes a challenge to animal and human health. It is a highly contagious disease with wildlife reservoirs and considered as endemic among swine populations. Pigs are crucial in the disease dynamics due to their capacity to generate new reassortant viruses. The risk of informal animal trade in the spread of zoonotic diseases is well recognized worldwide. Nevertheless, the contribution of the backyard pig trade network in the transmission of influenza in a wildlife/livestock interface area is unknown. This study provides the first simulation of influenza transmission based on backyard farm connections in Mexico. A susceptible-infectious-recovered (SIR) model was implemented using the Epimodel software package in R, and 260 backyard farms were considered as nodes. Three different scenarios of connectivity (low, medium, and high) mediated by trade were generated and compared. Our results suggest that half of the pig population were infected within 5 days in the high connectivity scenario and the number of infected farms was approximately 65-fold higher compared to the low connected one. The consequence of connectivity variations directly influenced both time and duration of influenza virus transmission. Therefore, high connectivity driven by informal trade constitutes a significant risk to animal health. Trade patterns of animal movements are complex. This approach emphasizes the importance of pig movements and spatial dynamics among backyard production, live animal markets, and wildlife.

BACKGROUND: In Mexico, estimates of Chagas disease prevalence and burden vary widely. Updating surveillance data is therefore an important priority to ensure that Chagas disease does not remain a barrier to the development of Mexico's most vulnerable populations. METHODOLOGY/PRINCIPAL FINDINGS: The aim of this systematic review and meta-analysis was to analyze the literature on epidemiological surveys to estimate Chagas disease prevalence and burden in Mexico, during the period 2006 to 2017. A total of 2,764 articles were screened and 36 were retained for the final analysis. Epidemiological surveys have been performed in most of Mexico, but with variable study scale and geographic coverage. Based on studies reporting confirmed cases (i.e. using at least 2 serological tests), and taking into account the differences in sample sizes, the national estimated seroprevalence of Trypanosoma cruzi infection was 3.38% [95%CI 2.59-4.16], suggesting that there are 4.06 million cases in Mexico. Studies focused on pregnant women, which may transmit the parasite to their newborn during pregnancy, reported an estimated seroprevalence of 2.21% [95%CI 1.46-2.96], suggesting that there are 50,675 births from T. cruzi infected pregnant women per year, and 3,193 cases of congenitally infected newborns per year. Children under 18 years had an estimated seropositivity rate of 1.51% [95%CI 0.77-2.25], which indicate ongoing transmission. Cases of T. cruzi infection in blood donors have also been reported in most states, with a national estimated seroprevalence of 0.55% [95%CI 0.43-0.66]. CONCLUSIONS/SIGNIFICANCE: Our analysis suggests a disease burden for T. cruzi infection higher than previously recognized, highlighting the urgency of establishing Chagas disease surveillance and control as a key national public health priority in Mexico, to ensure that it does not remain a major barrier to the economic and social development of the country's most vulnerable populations.

Neotropical wild rodents from Costa Rica were analyzed for the presence of herpesviruses (order Herpesvirales, family Herpesviridae). Using a broadly generic PCR, herpesvirus sequences were detected in 5% (8/160) of liver and heart samples: seven putative gammaherpesviruses in samples from Talamancan oryzomys (Nephelomys devius), sprightly colilargo (Oligoryzomys vegetus), Mexican deer mouse (Peromyscus nudipes), and Chiriqui harvest mouse (Reithrodontomys creper) and one putative betaherpesvirus in long-tailed singing mouse (Scotinomys xerampelinus). Results from this study could guide ecological investigations targeting the prevalence and host associations of herpesviruses in wild rodents from Costa Rica.

This study evaluated the impact of soft tissue grafts to reduce marginal peri-implant recession (MPR) after 1 year of follow-up. A total of 24 patients with one single failing maxillary incisor presenting facial bone dehiscence and receiving an immediate implant, bone graft, and provisional were randomly divided into three groups (n = 8 in each group): control (CTL), collagen matrix (CM), and connective tissue graft (CTG). Clinical, photographic, and tomographic analyses were performed to evaluate tissue alterations. The use of a CTG avoided MPR (P < .05) and provided better contour of the alveolar ridge (P < .01) and greater thickness (P < .05) of the soft tissue at the implant facial aspect.
