[Prevention of infectious complications of major upper respiratory and digestive tract surgery by the combination of pefloxacin-amoxicillin-clavulanic acid]. / Prévention des complications infectieuses de la chirurgie majeure des cancers des voies aéro-digestives supérieures par l'association: pefloxacine-amoxicilline acide clavulanique.

The authors report the results of a therapeutic trial aimed at assessing the efficacy and costs of a twin-antibiotic therapy associating Pefloxacin and Amoxicillin-clavulanic acid for the prevention of infectious complications in major surgery for cancer of the upper respiratory and digestive tracts. The results obtained show 10% of complications, including 6.5% of local complications, with 4.5 to 9.5 day treatments. In our opinion, the poor results noted in the literature in studies on a simple intraoperative antibiotic prophylaxis confirm the need to use a real, so-called "curative" antibiotic therapy over a sufficient period for this type of surgery. This antibiotic therapy must be well tolerated, active for the contaminating organisms, produce no selection of resistant bacteria, and raise hopes to reduce postoperative infectious morbidity, therefore the costs of treatment.

The diagnosis of mucoceles of the facial sinuses is essentially based on CT scans. The authors define here the main features of these benign tumours, the evolution of which is sometimes formidable.

A case of cholesteatoma of the ethmoid extending to the orbit is reported. These cholesteatomas of facial sinuses are rarely reported, those documented involving localization in maxillary or frontal, or ethmoido-frontal or ethmoido-maxillary sinuses. Primary perisinusal cholesteatomas of embryonic origin must be distinguished from the rarer secondary iatrogenic or post-traumatic lesions. Histopathology allows distinction between epidermoid cysts and cholesterol type granulomas. Treatment is by surgery using a wide approach.

The authors relate the case of two patients of 61 and 64 years old, each one having a high latero-cervical tumor, arching the soft palate, developed from the right vagus nerve and which was a chromaffin paraganglioma. After a recall about the APUD system tumors and the "paraganglioma" concept, they relate their observations at state of the vagal paraganglioma histology, pathology and treatment.

[Cutaneous parotid fistula of calculous origin]. / Fistule cutanée parotidienne d'origine lithiasique.

A case of cutaneous salivary fistula is presented. A 42-year-old man had a recurrent inflammatory parotid disease which disappeared with the expulsion of the calculus. The authors note the rarity of this evolution in the literature and suggest a protocol of treatment.
