RESUMO

When the new twin operating theatres at the Edinburgh Department of Surgical Neurology opened for the first time on 1 July 1960, they revealed a revolutionary space-pod design. The new department had been designed to firmly establish the specialty in Scotland and the UK, setting the stage for a period of real progress. The most distinctive feature of the two operating theatres was their egg shape, including domed ceilings pierced with operating lights, general lighting, ventilation grilles and viewing ports for visitors. Norman Dott (1897-1973) and his colleagues set the foundation for prosperity and success that lasted decades. However, 60 years after their opening, the DCN theatres at Western General Hospital shut forever, as the department moved to the new Department of Clinical Neurosciences, at the new Royal Infirmary Edinburgh. Echoes of the old theatres will live on in the new; the boldness of the design of the original theatres reflected the close cooperation between clinician-teachers, architects and administrators for the public good. This tradition of tangible confidence and optimism will hopefully carry into a new era, in the new hospital.

In late 18th century Britain, typhus fever plagued the mass mobilisation of soldiers and posed a significant challenge to physicians of the time. Epidemic typhus was spread through highly infectious faeces of infected lice and carried a high mortality in patients and healthcare staff alike. Physicians James Carmichael Smyth (1741-1821) and Archibald Menzies (1754-1842) theorized that typhus fever was caused by infection of human exhalation. They trialled the use of vapourised nitrous acid to fumigate patients, their clothes and their bedspace, with apparent success. Despite this, typhus fever continued to ravage deployments of soldiers into the early 19th century, stimulating the continuing evolution of the understanding of typhus and its treatment.

RESUMO

Alemtuzumab has been associated with the emergence of secondary autoimmune diseases. We report a case of a patient with relapsing-remitting multiple sclerosis who developed a refractory immune thrombocytopaenia associated with vasculitis, myelofibrosis and later Guillain-Barré syndrome following alemtuzumab. The medical community should be aware of unusual and unexpected adverse events that may be associated with alemtuzumab, especially when occurring simultaneously in the same patient.

It is difficult to recognize the identity of a face presented in negative contrast. This difficulty, however, is substantially reduced when only the eye region is contrast positive in an otherwise negative face image, and recognition of these so-called contrast chimeras approaches performance with full positive faces. This apparently similar accuracy has led researchers to suggest that familiar face representations are built around the eye region. The present study used the N250r, an event-related brain potential correlate of repetition priming, to examine whether chimera recognition is similarly efficient as positive face recognition. In a series of 3 experiments, we found a clear N250r for positive but reduced or even absent repetition effects for negative and chimera faces. This finding held true independent of whether the same basic pictures of familiar faces were used as prime and target stimuli (Experiment 1) or not (Experiments 2 and 3). Similar results were also obtained independent of whether positive, negative or chimera primes preceded full positive targets (Experiments 1 and 2) or targets in the same respective contrast format (Experiment 3). These results indicate that only positive faces contain all information necessary for optimal face recognition and that even though contrast chimeras are recognized highly accurately, the underlying processes work less efficiently as compared with normal face recognition. We conclude that familiar face representations are not built around the eyes but comprise detailed information from other regions of the face. Humans are remarkably accurate at recognizing familiar faces, whereas their ability to recognize, or even match, unfamiliar faces is much poorer. However, previous research has failed to identify neural correlates of this striking behavioral difference. Here, we found a clear difference in brain potentials elicited by highly familiar faces versus unfamiliar faces. This effect starts 200 ms after stimulus onset and reaches its maximum at 400 to 600 ms. This sustained-familiarity effect was substantially larger than previous candidates for a neural familiarity marker and was detected in almost all participants, representing a reliable index of high familiarity. Whereas its scalp distribution was consistent with a generator in the ventral visual pathway, its modulation by repetition and degree of familiarity suggests an integration of affective and visual information.
