RESUMO

Lectin-conjugated praecoxin A is a compound, which is combined Wheat Germ Agglutinin (WGA) Lectin with praecoxin A and also known to have an anti-tumor activity. In our lab, in order to investigate its immune reaction other than the anti-tumor activity ever known, we examined cytokines such as IL-6 and IL-12 through their mRNA expressions, which are generally secreted by macrophage both in vivo and in vitro. To analyze, we used RT-PCR for total RNAs of macrophages. As a result, we obtained that both in vitro and in vivo, lectin-conjugated praecoxin A showed an interesting increase on IL-6 and IL-12 even though it may be little hard to say the conjugated form is absolutely more effective than that of lectin or praecoxin A alone for immune response activities. Those results suggest that the conjugated form may give an additional opportunity in a future therapeutic use over its immuno activation properties.

RESUMO

Transient hyperkalemia has been reported to occur in eletric burn patients following intravenous administration of succinylcholine. We have studied the origin of the elevated serum K+ concentration after injection of succinylcholine from both the non-burned area and from the burned area in electirc burn patients. Blood samples for serum K+ measurement were drawn 1,2,3,4,5 and 10 minutes after administration of succinylcholine from both the burned are and the non-burned area in 6 electric burn patients. The following results were obtained: 1) Serum K+ levels increased in the burned group in 3 minutes after injection of succinylcholine. 2) The serum K+ concentration of venous samples from the electric burn area were more increased than from the non-burned area injection of succinylcholine.

RESUMO

After the clinical introduction of halothane, the problem of hepatotoxicity and mechanism has major implications for the practice of anesthesiology. This study was undertaken to investigate the liver function change after halothane anesthesia in 22 cases of electric burn patient. In this study, all cases had a great elevation of SGOT and SGPT levels, and previous experience of inhalation anesthesia, jaundice and who had not been transfused during this study. The following results were obtained: 1) Elevation of the SGOT and SGPT in electric burn patients may be influenced by damage of skeletal muscle and transaminase released into systemic circulation. 2) In the electric burn patients, elevation of the SGOT and SGPT does not enhance the susceptibility of the liver to injury by halothane.

RESUMO

Transient hyperkalemia is well known to occur in man following intravenous administration of succinylcholine chloride. We studied the change of the serum potassium concentration after injection of succinylcholine in nonburn patients and electricburn patients, physical status 1 or 2 adopted by the American Society of Anesthesiologists. We compared two groups: Non-burn patient group(Group 1) as control, Electric burn patient group (Group 2) as experimental. The following results were obtained: 1) In non-burn patient group(Group 1) serum potassium concentration was slightly decreased in the 1st, 2nd, 3rd, 4th, 5th, 6th and 10th minute after succinylcholine injection compared with control, but nostatistical significance was noticed in this group. 2) In electric burn patient group (Group 2) serum potassium concentration was maximally increased compared with the controls. Statistical significance was noticed at the 2nd and 3rd minute after succinylcholine injection in this group.

RESUMO

This preliminary study was intended to clarify the effect of intraoperative fluid therapy on Acid-Base balance of the use of Hartmann's solution and 5% Dextrose solution. The arterial carbon dioxide partial pressure and oxygen partial pressure and pH are basic indices of the adequacy of ventilation in anesthetized patients, respiratory care and intensive care. In order to achieve this goal, the data collections were done at the Department of Anesthesia in Hanil Hospital from Mar., 1980 to Aug., 1980. The samplings, 20 cases, were included A.S.A. class I and II. We used 1,500 to 1,800cc/2hrs. of Hartmann's solution and 1,000cc/2hrs, of 5% Dextrose solution each 10 patients through the operative period. Observations were made for the followings; pH, PaCO2HCO-3. From the results of this study, there were no significant relationship between the use of 5% Dextrose solution and Hartmann's solution on Acid-Base balance during the intraoperative period.

RESUMO

We have analysed 72 cases of geriatric patients over 70 year of age who underwent anesthesia in our Aospital from Jan. 1970 to May 1979 and the results were as follows; 1) Male patients were more than female(1.05: 1). (Male, 37 cases(51.4%) and female, 35cases (48. 6%). 2) By ASA classification, physical status was class II in 32 cases, E II in 24 cases, III in 12 cases, E III in 2 cases and IV in 2 eases. 3) The number of the patients of general surgery department(46.63%) was highest. 4) Preoperative problems were cardiovascular diseases including hypertension (11cases) and LVH(25 cases), arrhythmia(6cases), tachycardia(4cases), RBBB(2cases) and bradycardia (2 cases) in EKG. 5) Preoperative pulmonary disease including pulmonary tbc(14 cases), chronic bronchitis (6 cases), and emphysema(4 cases) was revealed in the chest PA. 6) There was no mortality during the operative or postoperative period.

RESUMO

Little has been reported concerning post-operative hypoglycemia in previously asymptomatic individuals. Post-operative hypoglycemic coma is one of the most perplexing problems and must be differentiated from other conditions accompaning coma. We experienced a case of hypoglycemia following emergency cesarean section in primigravida with cephalopelvic disproportion on the fourth post operative day. The comatous state appeared abruptly and developed repeatedly. The causes of post-hypoglycemic coma are discussed and the prolonged starvation before and after operation was suspected to be the cause of coma.

RESUMO

Maternal and fetal effects of anesthesis for 781 cases ceasarean sections, performed during the past 10 years (1965~1974) in Hanil Hospital have been evaluated clinically. It is emphasized that the selection of anesthetic agents and methods, should depend upon the physical status of the patients and the ability of both obstetricians and anesthesiologists. Optimal results require effective teamwork amongst obstetricians, anesthesiologists and nursing personnel. The anesthesiologist should be fully infermed of obstetric aspects. A variety of anesthetic techniques can be used for elective and emergency Caesarean sections. Each technique has its own advantages, disadvantages and limitations which must be thoroughly understood for proper application. The choice of anesthesia should be determined primarily by the experience of the operator, the degree of ugency, condition of the monther and the fetus, the physiopathology, the skill of the obstetrician and the desires and the emotional state of the monther.

RESUMO

Cardiac arrhythmias were produced in the dog by injections of epinephrine or isoproterenol during intravenous thiopental or propanidid anesthesia. Propanidid did not increase the pressor response to epinephrine, reduce the depressor response to isoproterenol, or diminish the tachycardia following epinephrine or isoproterenol. Propanidid, however, consistently increased the arrhythmia threshold doses of epinephrine and isoproterenol four times those during thiopental anesthesia. It is concluded, therefore, that the antiarrhythmic effect of propanidid is not related to beta adremergic blockade but to its local anesthetic or quinidine-like action and that propanidid is an excellent intravenous anesthetic in view of its antiarrhythmic action on the heart.

RESUMO

Our own preliminary experiences with the use of lactated Ringer solution during major surgery have been reported. We used 10cc/kg/hr in 40 patients throughout the operative period, and compared them with a control group of 30. Observations were made for the following; RBC, Hct, Hgb, electrolytes, and urine output. There were no significant differences in blood constituents and electrolytes between the two groups, but considerable increase in urinary outupt was noticed in the experimental group during 24 hour post-operative period. Blood transfusion had been done to replace blood loss and the experimental group had considerable reduction of transfused blood volume during surgery. We accept and produce evidence for the concept, pronounced by Epstein, that decrease in functional rextra-cellular fluid is a major determinant to sodium excretion. A major limitation of post-operative sodium excretion seemed to be the acute contraction of the functional extracelluar fluid incurred by surgery. If the losses of extracellular fluid were replaced during the operation, the phenomenon of sodium retention, was not seen. Neither normal saline nor 5% dextrose adequately replace the extracellular loss and kidneys do not eliminate excess water in this way. Lactated Ringer solution prevents acidosis and concomittant retention of water and sodium, and at the same time, sodium is adequately eliminated. It appears that this is an advance in managing fluid and electrolyte balance during surgery.
