RESUMO

OBJECTIVE: To observe whether Jiedu granule (, JDG) modulates the composition of the gut microbiota during advanced hepatocellular carcinoma (HCC). METHODS: A randomized controlled trial was conducted. Sixty-two advanced HCC participants were randomly allocated to receive JDG or placebo. The median overall survival (OS) times of patients and the variation of relative abundance of bacteria over time were used as main outcome measures. RESULTS: Patients who received JDG demonstrated significantly longer median OS times compared with the placebo group. Pyrosequencing of the V3 regions of 16S rRNA genes revealed a dose dependent deviation of gut microbiota in response to JDG treatment. Redundancy analysis identified and Peptostre-ptococcaceae which related to the onset of liver cancer disappeared after 1-month and 2-month JDG treatment, while in control group, no significant changes of these two bacteria were found. The variation tendency of relative abundance of (essential in immunoblocking therapy of tumor) in JDG group was not obvious while in control group, it was decreased significantly with time. The relative abundance of (correlated with the occurrence of liver cancer) was increased in JDG group and was decreased in control group over time. CONCLUSION: Changes in the gut microbiota may be associated with the efficiency of JDG on survival period of advanced HCC patients. The purpose of the present study was to evaluate the pharmacological effects of Portulaca oleracea L. (Purslane) (PL) on N-nitrosodiethylamine- (NDEA-) induced hepatocellular carcinomas (HCC) and explore its potential mechanism. Mice were randomly assigned to four groups: control group, NDEA group, NDEA + Purslane (100 mg/kg) group, and NDEA + Purslane (200 mg/kg) group. The animal of each group was given NDEA (100 ppm) in drinking water. 1 h later, Purslane dissolved in PBS was intragastrically administered for continuous seven days. The results showed that Purslane reduced the activities of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) in liver and serum. Purslane also reduced the contents of interleukin-6 (IL-6), IL-1ß, tumor necrosis factor-α (TNF-α), and methane dicarboxylic aldehyde (MDA) and restored the activity of superoxygen dehydrogenises (SOD) in serum. Purslane could obviously attenuate the hepatic pathological alteration. Furthermore, treatment with Purslane effectively inhibited the phosphorylations of phosphatidylinositol 3 kinase (PI3K), protein kinase B (Akt), mammalian target of rapamycin (mTOR), nuclear factor-kappa B (NF-κB), and inhibitor of NF-κBα (IκBα) and upregulated the expressions of NF-E2-related factor 2 (Nrf2) and heme oxygenase- (HO-) 1. In conclusion, our research suggested that Purslane exhibited protective effects on NDEA-induced hepatocellular carcinomas by anti-inflammatory and antioxidative properties via the PI3K/Akt/mTOR and Nrf2/HO-1/NF-κB pathway.

To improve the preparation speed and quality of moxa cone, a mould for preparation of moxa cone was designed and applied in moxibustion. It consists three parts: a foundation (a flat-bottomed circular cylinder hollow in center), a moxa cone forming tube (a hollow column encircled by two semi-cylinders with symmetrical shape and size, and was inserted in the internal cavity of the foundation); and a compression rod (a solid cylinder used for compressing moxa placed in moxa cone forming tube). The mould is practical, simple, and convenient and can effectively speed up the preparation of moxa cone, and it is safe in application. Moreover, the obtained moxa cones are regular in shape, uniform in dosage and size, dense in texture, and convenient to pack, store and carry.

RESUMO

Traditional Chinese medicine (TCM), an important component of complementary and alternative medicine, has evolved over thousands of years with its own unique system of theories, diagnostics and therapies. TCM has been increasingly used in the last decades and become well known for its significant role in preventing and treating cancer. We believe that TCM possesses advantages over Western medicine in specific aspects at a certain stage of cancer treatment. Here we summarize the advantages of TCM from three aspects: preventing tumorigenesis; attenuating toxicity and enhancing the treatment effect; and reducing tumor recurrence and metastasis.

The field of gene therapy has been increasingly studied in the last four decades, and its clinical application has become a reality in the last 15 years. Traditional Chinese medicine (TCM), an important component of complementary and alternative medicine, has evolved over thousands of years with its own unique system of theories, diagnostics and therapies. TCM is well-known for its various roles in preventing and treating infectious and chronic diseases, and its usage in other modern clinical practice. However, whether TCM can be applied alongside gene therapy is a topic that has not been systematically examined. Here we provide an overview of TCM theories in relation to gene therapy. We believe that TCM theories are congruent with some principles of gene therapy. TCM-derived drugs may also act as gene therapy vehicles, therapeutic genes, synergistic therapeutic treatments, and as co-administrated drugs to reduce side effects. We also discuss in this review some possible approaches to combine TCM and gene therapy.

Even in secular societies, a small portion of patients find their spirituality to help cope with illness. But for the majority of patients, psychosocial and spiritual needs are neither addressed nor even considered a relevant factor by health care professionals. To measure such specific needs, the Spiritual Needs Questionnaire (SpNQ) was developed. The aim of this study was to validate the Chinese version of the SpNQ (SpNQ-Ch) and thus to measure psychosocial and spiritual needs of Chinese patients.

In addition to its role in erythropoiesis, erythropoietin is also appreciated for its neuroprotective effects, and it has been suggested for treatment of some ischemic-hypoxic neurovascular diseases. The protective effects of endogenous erythropoietin in the brain give rise to the hypothesis that modulating erythropoietin expression might be a better way for treatment of ischemia-hypoxia neurovascular diseases. We have found that ethanol extract of Portulaca oleracea L. (EEPO) could increase erythropoietin expression in hypoxic mouse brain in our previous study. The present study is to investigate whether EEPO exerts its neuroprotective effects against hypoxia injury through regulating endogenous erythropoietin expression. The results demonstrated that EEPO decreased the serum neuron specific enolase level in hypoxia mice and the activity of caspase-3 in neuron, increased the neuron viability and attenuated the pathological damages caused by the hypoxia condition. Importantly, we also found that EEPO stimulated the endogenous erythropoietin expression at both mRNA and protein levels. Using the conditioned medium containing soluble erythropoietin receptor, we found that the neuroprotective effects of EEPO were dependent, at least partly, on erythropoietin expression. Although EEPO did not affect transcription of hypoxia inducible factor-1α (HIF-1α), it did stabilize expression of HIF-1α. It is concluded that EEPO has neuroprotective effects against hypoxia injury, which is at least partly through stimulating endogenous erythropoietin expression by stabilizing HIF-1α.

RESUMO

Primary liver cancer is one of the most commonly seen tumors in clinical practice. Due to the stealthiness and fast progress of liver cancer, only 20% of the patients may have chance to receive operation for radical therapy. Patients seldom get benefit from systematic chemotherapy and as a result, local chemotherapy methods such as transarterial chemoembolization (TACE) have become the mainstay in the treatment of liver cancer. Compared with systematic chemotherapy, TACE produces fewer side effects and most of such side effects are caused by postembolization syndrome manifested as nausea and vomiting, abdominal pain, fever, loss of appetite, etc.

Objective: To investigate the relationships between constitutional types of traditional Chinese medicine (TCM) and motion sickness. Methods: A survey of TCM constitutions in ocean sailors participating in a voyage was performed by using the TCM Constitution Questionnaire developed by Beijing University of Traditional Chinese Medicine, while the survey of motion sickness was operated by Graybiel's diagnostic criteria. The incidences of motion sickness among sailors with different types of constitutions were compared. Results: Prior to the voyage, 50.3% of sailors exhibited a gentleness constitution, 14.5% were of dampness-heat constitution, 10.3% were of qi-stagnation constitution, whereas the percentages of qi-deficiency, yang-deficiency, yin-deficiency, blood-stasis and special diathesis constitutions were 6.2%, 7.6%, 6.2%, 4.1% and 0.7%, respectively. None exhibited a phlegm-dampness constitution. By the end of the 176-day voyage, the percentages of gentleness, dampness-heat, qi-depression, qi-deficiency, yang-deficiency, yin-deficiency, blood-stasis, special diathesis and phlegm-dampness constitutions were 33.8%, 13.8%, 13.1%, 11.0%, 6.9%, 9.7%, 4.1%, 0.7% and 6.9%, respectively. The incidence of motion sickness was 69.7% (101 sailors) during this voyage. The incidences of motion sickness among sailors with different types of constitutions before the voyage showed significant difference (P<0.001). The incidence of motion sickness was higher in the sailors with dampness-heat constitution than in those with gentleness constitution. Conclusion: Types of Chinese medical constitution can be related to susceptibility to motion sickness. Furthermore, ocean voyage may have an effect or influence on the type of Chinese medical constitution of sailors involved.

Background: The side effects of glucocorticoid in treatment of systemic lupus erythematosus (SLE) have been the focus of debate, and our preliminary study indicates that ginsenosides can enhance the efficacy of dexamethasone. Objective: To observe the effects of ginsenosides combined with prednisone in SLE patients. Design, setting, participants and interventions: A total of 60 SLE patients from Department of Rheumatology and Immunology, Changhai Hospital, Second Military Medical University, were randomly divided into treatment group and control group, with 30 patients in each group. Patients in the treatment group were given routine treatment with prednisone plus ginsenosides, while those in the control group were given routine treatment with prednisone plus placebo. They were all treated for 3 months. Main outcome measures: After three-month treatment, syndrome score in traditional Chinese medicine (TCM), total response rate and symptom improvement rate were measured and evaluated. Results: Twenty-eight cases in treatment group and twenty-seven cases in control group were included in analysis. The total response rates in the treatment group and control group were 89.28% and 66.67% respectively, and there was a significant difference between the two groups (P<0.05). After treatment, the TCM syndrome scores in the two groups were lower than those before treatment (P<0.01), and prednisone plus ginsenosides was better in decreasing the TCM syndrome score than prednisone plus placebo (P<0.05). The symptoms were improved in the treatment group as compared with the control group (P<0.05). Conclusion: Prednisone combined with ginsenosides can increase the clinical effective rate and improve the clinical symptoms of SLE patients.

The past decade has witnessed the remarkable progress on nanotechnology and nanoherb. With the globally rapid development of nanotechnology, we are considering to construct new nanoformula systems of traditional Chinese medicine (TCM) by using porous materials, multilayered core-shell particles or nanoparticles containing various multifunctional parts. With the compatibility of sovereign, minister, assistant and courier in a formula, new nanoformula systems of TCM will have various advantages, such as containing multiple active species, controlled release, targeting function, and labeling and tracing capabilities. Using the latest breakthroughs of nanotechnology for the modern research of TCM will greatly help enhance the ability to investigate the principles of TCM, and to design, screen and utilize new nanoformula systems of TCM.

BACKGROUND: Currently, most therapies of postembolization syndrome following transcatheter arterial chemoembolization (TACE) aim directly at a single symptom, thus leading to limitations. OBJECTIVES: To seek for a systematic approach to prevent and treat the syndrome, we carried out this study to observe the effect of ginsenosides (GS) and dexamethasone (Dex) in alleviating the postembolization syndrome following TACE. METHODS: In the randomized, double-blinded and controlled trial, 120 patients with primary liver cancer were divided into 4 groups, with 30 patients in each group. The changes of clinical symptoms and laboratory tests before TACE and on 3 and 7 days after TACE were observed. RESULTS: The results indicated that Dex combined with GS not only markedly decreased the occurrence ratio and duration of such symptoms as nausea, vomiting, and fever, but also significantly reduced levels of total bilirubin, glutamic oxaloacetic transaminase, and glutamic-pyruvic transaminase (AST) and improved the Child-Pugh stage of liver function as compared with single use of GS or Dex. CONCLUSIONS: In conclusion, although single use of Dex or GS may improve some indices of adverse effects after TACE, the combination of Dex and GS can systematically prevent and treat the postembolization syndrome following TACE.
