RESUMO
Six cases of lymphogranulomatosis of the thymus are described. There are few clinical sings as a rule or none at all. The main diagnostic method is X-ray (poly-position roentgenoscopy, computed tomography) which reveals a new growth in the thoracic cavity. Preoperative morphological verification of the character of the tumor is difficult. Only histological study shows the true character of the disease. Surgical intervention makes it possible to establish the diagnosis and is a component of the complex of the therapeutic measures (radiotherapy and chemotherapy) applied in the management of local forms of lymphogranulomatosis of the thymus.

Three clinical cases with the mediastinal form of Brill-Symmers disease are discussed. The variants of its symptomatology and course are described. The methods of diagnosis and surgical tactics are dealt with. Operative treatment of mediastinal giant follicular lymphoma in the stage of a benign course is the method of choice. The late-term results (of up to 20 years) were studied in 2 of the 3 patients.

During a period of 42 years the authors had 44 patients with various vascular neoplasms of the mediastinum--angiomas. Experience in the treatment of 6 of them is discussed in B. V. Petrovsky's monograph Surgery of the Mediastinum (1960). The other 38 cases were distributed as follows: hemangiomas 28 (14 mature and 14 immature) and lymphangiomas 10 (all mature). The patients' ages ranged from 11 to 63 years, the duration of the anamnesis from 4 weeks to 40 years. Mediastinal angiomas are mainly encountered in children and young people, with equal frequency among females and males. Their clinico-radiological picture is diverse and unspecific, the diagnosis is established with difficulty. Surgery is the principal method of treatment. Radical removal of the tumor must be attempted. Radiologically-guided endovascular occlusion of the vessel supplying the tumor is possible in some cases. The late results of surgical treatment are good in cases of mature angiomas and depend on the radicalism of the operation in immature forms.

The authors have analysed the data of clinical and pathoanatomical (biopsy) investigations in 41 patients, operated upon for the true tumor of the thymus (thymoma) without any signs of malignant miasthenia. Based on the literature materials and personal observations, it is considered necessary to provide for a rational clinico-anatomical classification of thymomas, which may allow certain prognostication of the results of surgical treatment. An attempt is made to work out a preliminary uniform working scheme of the histological nomenclature for thymomas.
