RESUMO

In the present investigation, the conditions for in vitro submerged culture of a native strain of Ganoderma sp. were evaluated. Different culture medium ingredients, inoculum concentrations, inoculation methods, configuration, and airflows were evaluated to improve biomass production. The addition of thiamine and olive oil to the culture medium increased biomass production, as well as inoculating 6.6 g/L since there are no significant differences in biomass growth according to inoculum origin (pre-inoculum, discs or with spores). The best configuration of the 3 L stirred tank bioreactor was using three impellers and a porous air diffuser of 0.25 volume per volume per minute (vvm), the dry biomass concentration was 22.6 g/L after 12 days of cultivation at 30 °C, much higher than other investigations. This study provides relevant information for pilot-scale production of this fungus for future secondary metabolites. The culture medium was optimized, and it was defined that the concentration and origin of the inoculum did not influence the growth of Biomass, but the aeration and the configuration of the system allowed the establishment of protocols for the cultivation of Ganoderma sp.

The use of substances or conditions as elicitors can significantly increase the production of secondary metabolites. In this research, the effects of different elicitors on the production of antioxidant secondary metabolites were evaluated in a strain of Ganoderma sp. The elicitors tested were pH changes in different growth phases of the fungus (pH 3, 5.5 and 8), different concentrations of peptone as a nitrogen source (1 g/L and 10 g/L), and the addition of chemical agents to the culture medium (ethanol, growth regulators, and salts). The alkaline pH during the stationary phase and the high availability of nitrogen were effective elicitors, producing cultures with higher antioxidant activity (37.87 g/L and 43.13 g/L dry biomass) although there were no significant differences with other treatments.

RESUMO

Resumen El virus SARS-CoV-2 se ha extendido a nivel mundial, rápidamente ha sobrecargado los sistemas de salud. Esta emergencia ha implicado cambios en la atención usual del infarto agudo miocardio con elevación del ST (IAMCEST) puesto que la actividad habitual de las salas de hemodinamia y las vías de traslado de los pacientes se ha visto afectada. La afectación del personal de salud también es una preocupación relevante por lo que presentamos un documento de Consenso de la Asociación Costarricense de Cardiología que pretende generar una guía de trabajo al personal que atiende esta patología y garantizar la atención adecuada del IAMCEST durante la pandemia en Costa Rica.


Abstract The current COVID-19 has spread worldwide, the outbreak is altering the usual activity of the catheterization laboratorios and the usual treatment pathways of patients with chronic diseases or emergencies, such as Acute Coronary Syndrome could be disrupted. The involvement of health personnel is a relevant concern, so we created a consensus document of the Costa Rican Association of Cardiology that aims to generate a decision-making workflow to treat this pathology and guarantee adequate and continuous care for ST elevation myocardial infarction during the COVID-19 outbreak.

Objetivo: conocer la variabilidad de la técnica de extracción de hemocultivos en la práctica clínica de los enfermeros, así como valorar las diferencias en función de la existencia o no de un protocolo. Método: estudio descriptivo transversal realizado en 2018 mediante encuesta online de participación voluntaria y anónima, difundida a través del Consejo General de Enfermería, 200 direcciones-gerencia de centros hospitalarios nacionales, perfiles privados, institucionales y páginas de grupos. Se diseñó un cuestionario ad hoc de 23 preguntas para explorar variables sociodemográficas, laborales y relacionadas con la extracción de hemocultivos: existencia de protocolo en el servicio, indicaciones de solicitud correctas y cuestiones técnicas. Se realizó un análisis descriptivo de la muestra, así como un análisis bivariante mediante la prueba x2 de Pearson. Resultados: participaron 1.516 profesionales (84,4% mujeres, 64,4% con experiencia mayor a 10 años). El 53,2% refirió la existencia de un protocolo específico en su servicio. Quienes disponían de protocolo empleaban con mayor frecuencia el equipo estéril, clorhexidina alcohólica al 2% para la desinfección de la piel y algún tipo de antiséptico para la desinfección de los tapones (p< 0,001); en caso de muestras del catéter venoso central, fue más habitual que extrajeran otra muestra de un acceso periférico (p< 0,001), cambiaran la aguja para inocular la sangre en los frascos (p< 0,05) sin desechar ninguna cantidad (p< 0,001) y recogieran unos 10 cc de frente a otros volúmenes (p< 0,01). Conclusiones: hay una alta variabilidad en la realización de la técnica y diferencias en función de la existencia o no de un protocolo. La unificación de procedimientos consensuados se torna necesaria para optimizar y rentabilizar la extracción de hemocultivos


Objective: to understand the variability in the technique for blood culture extraction in the clinical practice of nurses, as well as to assess any differences based on whether there is a protocol or not. Method: a descriptive cross-sectional study conducted in 2018 through an on-line survey with voluntary and anonymous participation; it was distributed through the General Council of Nurses, 200 managements of national hospitals, private and institutional profiles, and group webpages. A 23-question ad-hoc questionnaire was designed, in order to explore sociodemographical, and occupational variables, as well as those associated with blood culture extraction: the existence of a protocol in the unit, correct indications of application, and technical matters. A descriptive analysis of the sample was conducted, as well as bivariate analysis through Pearson's x2 test. Results: the study included 1,516 professionals (84.4% were women, 64.4 % with >10-year experience); 53.2% of them reported there was a specific protocol in their unit. Those who had a protocol used more frequently the sterile equipment, chlorhexidine 2% for skin disinfection, and some type of antiseptic for disinfecting the stoppers (p< 0.001). In the case of central venous catheter samples, it was more frequent that they extracted another sample from a peripheral (p< 0.001), changed the needle in order to inject the blood into the vials (p< 0.05) without discarding any (p< 0.001), and collected 10cc vs. other volumes (p< 0.01). Conclusions: there is high variability in terms of conducting the technique, as well as differences based on whether there is a protocol or not. The standardization of procedures by consensus becomes necessary in order to optimize and make the most of blood culture extraction

Objetivo: Determinar el grado de concordancia y validez, a la hora de predecir la hospitalización y el consumo de recursos en urgencias, del Sistema de Ayuda al Triaje 3M TAS y su comparación con el Sistema Español de Triaje - Model Andorrà de Triatge (SET-MAT). Método: Estudio observacional de cohortes prospectivo que incluyó de forma consecutiva los episodios filiados de 9 a 22 horas en un servicio de urgencias de un hospital secundario entre el 24 de marzo y el 30 de abril de 2014. Los pacientes se clasificaron de forma enmascarada y simultánea por una enfermera asistencial mediante el programa de ayuda al triaje SET-MAT y por una enfermera de investigación mediante el 3M TAS. Se recogieron variables demográficas, de gravedad, las pruebas de laboratorio, radiológicas e interconsulta a especialistas realizadas en urgencias, el tiempo de estancia y el destino final. Las variables de resultado fueron la hospitalización y el consumo de al menos un recurso en urgencias. Resultados: Se incluyeron 3.379 episodios. El índice de concordancia mediante el kappa ponderado cuadrático entre los sistemas de triaje fue de 0,26 y el kappa triaje de 0,17. El sistema 3M TAS presentó una mayor capacidad predictiva de hospitalización en comparación con el SET-MAT (p < 0,001), no siendo así en lo que respecta al consumo de recursos en urgencias (p = 0,111). Conclusiones: La concordancia entre los sistemas de triaje 3M TAS y SET-MAT fue baja, sin diferencias para predecir el consumo de recursos en urgencias, aunque el 3M TAS predijo mejor ingreso hospitalario que el SET-MAT (AU)

The influence of life support withholding and withdrawal on the deaths which occurred in an Intensive Care Unit (ICU) over a period of 32 months was analysed. Of 2640 patients admitted in ICU, one of the following five mutually exclusive categories was registered on the 548 patients who died: (i) complete treatment; (ii) complete treatment with non-resuscitation order (NRO); (iii) withholding of life-sustaining treatment; (iv) withdrawal of life-sustaining treatment; and (v) brain death. There was therapeutic limitation of life support in 45.6% of cases (n = 250), with an important majority of withholding (NRO and withholding) in 32.6% of cases, in comparison to withdrawal of life support (8.2% of cases). The comparative analysis with other statistic information suggests the existence of a similar global therapeutic limitation mean in communities with similar cultural background, even if there is a lower influence of life support withdrawal (8.2%) when compared to other countries regardless of their attitude towards the need to establish different degrees of control over technological resources applied to the critically ill. Further research should analyze the influence that moral perception of withdrawal as inconvenient in our society, has over our findings.
