[The factors influencing the incidence of heart rhythm disorders after the radical correction of the tetralogy of Fallot]. / Faktory, vliiaiushchie na chastotu narushenii ritma serdtsa posle radikal'noi korrektsii tetrady Fallo.

The frequency and factors favouring the development of cardiac rhythm disorders were studied in 306 patients operated on radically for Fallot's tetralogy (follow-up: 6 months--29 years). This complication was found in 99 patients (32.3%). Most patients showed ventricular arrhythmia--62 (62.6%). Paroxysmal tachycardia was noted in 11 patients, atrio-ventricular block in 15 patients, combination of different arrhythmias in 11 patients. Risk factors were: inadequate correction of the malformation, age over 15 at the time of operation, approach through the right ventricle with plastic repair of the drainage tract, five years after surgery.

A study is presented of the results of surgical treatment and the status of working capacity in 8,072 patients operated on for congenital heart disease. The results were good in 62%, fair in 30%, poor in 4% of these patients. There was a significant discrepancy between the state of invalidity as it was established by the medical expert commission and results of surgical treatment related to overestimation of the fact of surgical intervention and inadequate approach of evaluation of the health condition of the patients undergoing surgery based on the nosological form and initial severity of the disorders of cardiodynamics.

[The surgical treatment of an aortic aneurysm following a prior correction of its coarctation]. / Khirurgicheskoe lechenie anevrizmy aorty posle predshestvuiushchei korrektsii ee koarktatsii.

In a group of 381 patients who were operated on for coarctation of the aorta 34 (8.9%) were found to have an aneurysm of the aorta in the zone of the correction in the late-term postoperative period. The frequency of aneurysm formation was greater in patients with flap plastics. Fifteen patients underwent a second operation for aneurysm of the aorta, 2 (13.3%) of them died. In the group of patients who were not operated on 6 (31.6%) died.

[Functional state of the cardiovascular system in patients operated on for a secondary atrial septal defect]. / Functsional'noe sostoianie serdechno-sosudistoi sistemy u bl'nykh, operirovannykh po povodu vtorichnogo defekta mezhpredserdnoi peregorodki.

The functional state of the cardiovascular system in 64 adults with a secondary atrial septal defect and in 120 patients who were operated on for this disease was studied by means of the physical exercise test on a bicycle ergometer. In the late postoperative period, the physical working capacity of patients who underwent operation before the age of 20 years was 92% with respect to that of healthy individuals. The physical working capacity of patients in whom the atrial septal defect was closed after the age of 20 was only 76% as compared to the capacity of healthy individuals. The study showed surgical management of an atrial septal defect to be highly effective. The best functional results in the late postoperative period were produced in patients who were operated on before the age of 20.
