Aging alters body composition to induce sarcopenia, particularly in women, but the mechanism remains unclear. We hypothesized that silk peptide(SP) intake could prevent an age-related decrease in muscle mass and strength in middle-aged female rats and explored the action mechanism. After the acute intake of SP and defatted soybean peptides, serum concentrations of amino acids were measured in ten middle-aged rats in each group. Forty 12-month-old female Sprague-Dawley rats were fed a high-fat and high-carbohydrate diet for 12 weeks including 0.5 g casein/kg body weight(BW)/day(Aged), 0.15 g SP plus 0.35 g casein/kg BW/day(Low-SP), 0.5 g SP/kg BW/day(High-SP), or 40 mg metformin plus 0.5 g casein/kg BW/day(Metformin). Ten rats aged 7-week old(Young) had the same treatment as the Aged-group. The body composition, grip strength, glucose metabolism, intestinal tissue morphology, and gut microbiota were also determined. After an acute consumption, total amino acids were more quickly absorbed and maintained at higher levels in SP than soybean peptides. Lean body mass(LBM) and grip strength were lower in the Aged-group than the Young and Low-SP groups, and the High-SP regimen increased these parameters as much as the Young-group. Serum concentrations and mRNA expression of TNF-α in the gastrocnemius and quadriceps muscles were higher in the Aged-group than the Young-group, whereas SP intake reduced their serum levels and skeletal muscles. Glucose and insulin tolerance indicated that insulin resistance was elevated in the Aged-group compared to the Young-group, while Low-SP and High-SP alleviated them as much as the Young-group. High-SP increased serum propionate and butyrate concentrations compared to the Aged-group. SP intake increased the relative abundance of Bacteroides and Prevotella and decreased Blautia and Clostridium in the feces. In conclusion, SP intake protects against a decrease in lean body mass and grip strength in middle-aged female rats. The protection was partly related to maintaining higher serum concentrations of total amino acids after SP consumption and decreasing inflammation and insulin resistance through gut microbiota modulation.

OBJECTIVE: The location of a myocardial lesion on a wall thickening polar map often does not coincide with the location of the lesion on a perfusion polar map, especially when the myocardial lesion is located at the mid cardiac region. The purpose of this study was to determine the frequency and extent of discrepancy in the location of the lesion between perfusion and wall thickening polar maps on gated single photon emission computed tomography (SPECT) using lesion axis angle (LAA). METHODS: One hundred and forty-seven consecutive patients (male : female = 80 : 67, age range: 41-96 years) who underwent myocardial gated (99m)Tc-tetrofosmin SPECT on the suspicion of myocardial ischemia or infarct between September 2003 and September 2008 and showed both reduced myocardial perfusion and wall thickening on gated SPECT at mid cardiac region were reviewed. LAA, which is the angle between the lesion axis on perfusion and wall thickening polar maps, was measured for the patients who showed a discrepancy in lesion axis between the two polar maps. LAA was said to have a positive value when the lesion axis of the wall thickening polar map showed a counterclockwise angular rotation as compared with that of a perfusion polar map. The patients with LAA of less than 10 degrees were considered as having no lesion axis discrepancy between perfusion and wall thickening polar maps. LAA was correlated with left ventricular ejection fraction (LVEF) on gated SPECT using Pearson's correlation. Furthermore, two groups, one with LAA of >or=10 degrees and the other with LAA less than 10 degrees were correlated with dichotomous groups with >or=50% and less than 50% LVEF, using the chi(2) test. Then, 35 patients with acute coronary syndrome (ACS group) were analyzed separately for correlation between LAA and LVEF. RESULTS: The mean +/- SD of LAA was 44.31+/-30.77 degrees (range: 0-145 degrees ). LAA was 0-10 degrees in 25 patients, 11-30 degrees in 24 patients, 31-60 degrees in 58 patients, 61-90 degrees in 30 patients, and >90 degrees in 10 patients. In addition, the lesion axis of the wall thickening polar map as compared with that of the perfusion polar map was rotated in the counterclockwise direction in 122 patients and not rotated in 25 patients. LVEF on gated SPECT showed positive correlation with LAA (P = 0.000147). In addition, there was statistically significant correlation (P = 0.001) when the two groups with LAA of >or=10 degrees and less than 10 degrees , respectively, were correlated with the groups of >or=50% and less than 50% LVEF. For the ACS group, the mean +/- SD of LAA was 45.88+/-30.30 degrees (range: 0-135 degrees ) and LVEF showed positive correlation with LAA (P = 0.0001). There was no significant statistical difference concerning LAA and LVEF between ACS group and non-ACS group (P = 0.725 and P = 0.473, respectively). CONCLUSION: In most of our patients with coronary artery disease, the lesion axis of reduced wall thickening was rotated in the counterclockwise direction as compared with that of reduced perfusion on SPECT polar maps, especially when the myocardial lesion was at mid cardiac region. The LAA decreased as the LVEF decreased. This might be related to spatiotemporal distortion of myocardial contraction mentioned in the helical heart concept.

PURPOSE: F-18 fluorodeoxyglucose (FDG) positron emission tomography (PET) is known to be a useful diagnostic tool for staging, restaging, and monitoring therapy for lymphoma. The purpose of this retrospective study is to present a spectrum of FDG PET findings at initial presentation of Burkitt lymphoma and subsequent findings after therapy. METHOD AND MATERIALS: We retrospectively reviewed 48 patients with Burkitt lymphoma referred for a total of 160 FDG PET/computed tomography (CT) scans at our institution. We characterized the disease distribution of Burkitt lymphoma in all patients and measured representative FDG activity from initial staging scans. Therapeutic response and disease remission were assessed in patients with PET/CT and clinical follow-up studies. RESULTS: Of the 48 patients diagnosed with Burkitt lymphoma, 25 patients had FDG PET/CT scans for initial staging. All untreated lesions of Burkitt lymphoma were highly FDG avid. The mean maximum standardized uptake value of 54 representative lesions is 16.5 (range: 6-54). Twelve patients were immune compromised. The majority of patients had disease localized to the abdomen and the pelvis. Extranodal involvement was identified in more than half of the patients studied. CONCLUSION: The American (or sporadic) form of Burkitt lymphoma presented with intense hypermetabolic lesions when untreated. The information is useful in evaluating post-treatment studies in the absence of a pretreatment scan.

The preoperative diagnosis of intraabdominal panniculitis is difficult due to its rarity. However, the increased use of abdominal computed tomography (CT) for a variety of indications has increased the diagnosis of intraabdominal panniculitis, including omental panniculitis. The characteristic CT features of intraabdominal panniculitis are increased attenuation of the adipose tissue, the fat-ring sign, a tumoral pseudocapsule, soft-tissue nodules, and a left-sided orientation of mass maximum transverse diameter. Recognition of these features is valuable in the diagnosis of panniculitis, and hence percutaneous CT-guided biopsy to determine their presence may prevent unwarranted surgery. We report the case of a 61-year-old man found to have an idiopathic isolated omental panniculitis that was diagnosed by abdominal CT and percutaneous CT-guided biopsy.

The aim of this study was to assess the correlation between 18F-fluorodeoxyglucose positron emission tomography (FDG PET) positivity of tumor recurrence and vascularity, Ki-67, p53, and histologic grade in patients with ovarian cancer. Nineteen patients with recurrent ovarian cancer underwent FDG PET before second-look surgery. Archival paraffin-embedded tissue materials were used to assess histologic grade including architectural pattern, mitotic activity, and nuclear pleomorphism; intratumor microvessel density (MVD); Ki-67; and p53. Univariate analysis was used to evaluate the correlation between FDG PET positivity and each biomarker. Stepwise logistic regression analysis was used to determine the best parameter to explain FDG PET positivity. MVD revealed significant positive correlation with FDG PET positivity (p=0.0341). There was no significant correlation between FDG PET positivity and Ki-67 or p53 (p=0.4040, p=0.6027). Mitotic activity yielded statistically significant positive correlations with FDG PET positivity (p=0.0448) although histologic grade revealed no positive correlation (p=1). Stepwise logistic regression analysis revealed MVD to be the strongest parameter for FDG PET positivity (OR=0.696, 95% CI 0.487-0.993, p=0.0458). In conclusion, FDG PET positivity revealed positive correlation with MVD and mitotic activity. MVD was the strongest parameter in predicting positive tumor recurrence on FDG PET.

RESUMO

Abdominal and pelvic recurrence of pseudomyxoma peritonei after the surgery is occasionally seen but extraperitoneal spread and hematogeneous metastases are rare. This case of pseudomyxoma peritonei provides interesting radiologic findings of extraperitoneal spread, which occurred after an extremely long interval from initial diagnosis.

The aim of this study was to assess imaging findings on CT or MR images of histologically proven ovarian cystadenofibromas. In the period 1995-2001, 32 histologically proven ovarian cystadenofibromas were identified in 28 women. Of the 32 ovarian cystadenofibromas, 16 tumors were purely cystic and the remaining 16 were complex cystic on CT or MR images. Solid components of 16 complex cystic tumors were seen as nodular ( n=8) or trabecular ( n=9) solid areas. One tumor had both nodular and trabecular solid components. Among 16 complex cystic tumors, 14 had thick or irregular septa; thus, half of ovarian cystadenofibromas had morphological imaging features of malignancy on CT or MR images. On histology, solid components in the cystic tumors were correlated with fibrous stromas that occasionally made a false-positive result for malignancy on imaging.

OBJECTIVE: The purpose of our study was to evaluate the diagnostic accuracy of FDG positron emission tomography (PET) in comparison with CT in detecting recurrent ovarian carcinoma and its ability to reveal small tumor recurrence. MATERIALS AND METHODS: We reviewed the records of 31 consecutive patients with pathologically proven epithelial carcinoma who underwent FDG PET 1 month before second-look surgery to assess recurrent tumor. Of these 31 patients, 21 patients also underwent CT 1 month before second-look surgery. The diagnostic accuracies of FDG PET (n = 31), CT (n = 21), and combined FDG PET and CT (n = 21) in detecting recurrent tumor were calculated and compared with each other using the Bennett's test in 21 patients who underwent both imaging studies. Detection rates of individual tumors relative to their sizes were compared between FDG PET and CT using the McNemar test. RESULTS: The sensitivity, specificity, and accuracy of FDG PET, CT, and combined FDG PET and CT for revealing recurrent ovarian cancer were 45.3%, 99.7%, 91.0%; 54.5%, 99.6%, 91.7%; 58.2%, 99.6%, 92.4%, respectively. We found no statistically significant difference in the diagnostic accuracy of FDG PET, CT, and combined FDG PET and CT (chi(2) < 5.991). Detection rates of tumor nodules found on CT were significantly greater than those on FDG PET when nodule size was 0.3-0.7 cm (p < 0.05). CONCLUSION: FDG PET did not improve the overall diagnostic accuracy in detecting recurrent ovarian carcinoma compared with CT. Rather, FDG PET was inferior to CT in its ability to reveal small-tumor recurrence.
