RESUMO

OBJECTIVE: To identify professionals' view of the problem-solving capacity of primary care in the health system. DESIGN: Interpretative study, based on the Grounded Theory method and the focus-group technique. SETTING: Eight primary care centres. PARTICIPANTS: In all, 198 professionals: 86 physicians, 69 nurse and 43 who were not health professionals. INTERVENTIONS: Data were obtained by: open questions in the focus groups, recording and transcription of the contributions; division of the text into autonomous sense units; sense units grouped by their common characteristics; inductive definition of each category; relationship between categories and triangulation of inter-team data. RESULTS: The problem-solving capacity of professionals should be characterized by: care for the bio-psycho-social demands and needs of the population; the integration of the following dimensions of professional competence: communication and teamwork; the promotion of prevention and public education. However, health-care daily practice prioritised quantitative parameters linked to reactive actions that were distant from the singularity of patients' pathologies. CONCLUSIONS: The desired problem-solving capacity of primary health care is threatened by excessive quantitative measurement, by shortcomings in human resources and facilities, and by the well-known lack of time in health-care delivery.

Objetivo. Identificar, desde la visión de los profesionales, el concepto de la capacidad resolutiva de la atención primaria en el sistema sanitario. Diseño. Estudio interpretativo basado en el método de la Grounded Theory, así como en la técnica del focus-group. Emplazamiento. Ocho centros de atención primaria. Participantes. Se incluyó a 198 profesionales, 86 médicos, 69 enfermeras y 43 no sanitarios. Mediciones. Obtención de datos mediante preguntas semiestructuradas en los focus-group, grabación y trascripción de las aportaciones; división del texto en unidades autónomas de sentido; agrupación de las unidades de sentido según características comunes; definición inductiva de cada categoría; relación entre categorías y triangulación de datos interequipos. Resultados. La capacidad resolutiva se debería caracterizar por: a) la atención de las demandas y las necesidades biopsicosociales de la población; b) la integración de las siguientes dimensiones de la competencia profesional: comunicación y trabajo en equipo, y c) la promoción de la prevención y la educación de la ciudadanía. Sin embargo, la cotidianeidad asistencial prioriza la realización de parámetros cuantitativos asociados con una actividad reactiva alejada de la singularidad de las enfermedades de los pacientes. Conclusiones. La capacidad resolutiva de la atención primaria de salud deseada se encuentra amenazada por su excesiva medición cuantitativa y por las deficiencias en cuanto a recursos humanos y de equipamientos, así como por la notoria falta de tiempo para la actividad asistencial

RESUMO

La función comunicativa se ha incorporado como un elemento de desarrollo de la gestión y dirección de las organizaciones sanitarias. Los objetivos, la misión, la visión y los valores deben estar adecuadamente definidos en cualquier organización. A pesar de que en el ámbito de la salud suelen estar definidos estos parámetros, se constata una desviación entre la formulación de la finalidad institucional y los valores intrínsecos de los profesionales que trabajan en la organización. Lo que se comunica institucionalmente no suele corresponder con el proceso de aculturación de la práctica asistencial de las organizaciones sanitarias. En este sentido, es necesario que las organizaciones de la salud empleen la auditoría de la comunicación como una herramienta que les permita establecer una estrategia organizativa coherente, adecuada y proclive a proceder a los cambios y a las transformaciones pertinentes para adaptarse continuamente al entorno interno y externo. La auditoría de la comunicación, a través de unas fases determinadas (preparación, encuesta preliminar, observación evaluativa e interpretación de resultados) y con la debida participación de los profesionales, contribuye a disminuir la incoherencia y la desviación existente entre lo que se comunica oficialmente y lo que se percibe en el desarrollo cotidiano de la asistencia


The communicative function has been incorporated as an element of the development of management of health organizations. Purposes, mission, views and values must be well-defined in all organizations. Despite this, there is a deviation between the formal expressions of main purpose of health organizations and the intrinsic values of health professionals. Official communication from institutions does not generally correlate with the socialization process in the everyday health care practice. In this sense, health organizations must make use of audit communication as a tool for a coherent, adequate and proclive organizational strategy. Communication audit contributes to the achievement of changes and appropriate transformations in order to continuously adapt to the internal and external environment. Audit communication, through different phases (preparation, preliminary survey, evaluative observation and results interpretation) and adequate participation of health professionals, contributes to decrease incoherence and deviation between official communication and everyday health care perception

RESUMO

The author reflects about the need to integrate health system professionals with the demands of society. The relationships among citizens and health professionals, inside the structure of the sanitary system, develop inside a global context constituted by society The tendency of the sanitary system to increase the power of a biomedical vision and the depersonalization of the relationships with patients should lead to a social debate about the desired, necessary and sustainable model of health care-nursing. A bio-psycho-social concept of health care, an integration of scientific knowledge and the incorporation of communicational competencies constitutes one of the challenges to be met by a sanitary system which should consider the individual person to be its prime reason to exist.

Reflexión sobre la necesidad de integrar los actores del sistema sanitario en el ámbito de las demandas de la sociedad. Las relaciones de los ciudadanos y los profesionales de la salud, enmarcadas en un sistema sanitario, se desarrollan en el contexto global constituido por la sociedad. La tendencia del sistema sanitario a potenciar la visión biomédica y la despersonalización de las relaciones con el usuario, debe dejar paso a un debate social sobre el modelo deseabele, necesario y sostenible de salud-enfermedad. La concepción bio-psico-social de la sanidad, la integración de conocimientos científicos y la incorporación de las competencias comunicacionales constituyen uno de los retos a alcanzar en un sistema sanitario que debe considerar a la persona como su máxima razón de ser
