RESUMO

Clinical neurophysiology is the medical specialty in the UK responsible for training doctors in the interpretation of electroencephalograms (EEGs). At present it is difficult to recruit doctors to train in clinical neurophysiology which causes difficulty in delivering services. We established a tele-EEG service to record and report on EEGs at a UK hospital which was unable to attract a resident clinical neurophysiologist. The EEGs were recorded in digital format and placed on a web server to be read by a clinical neurophysiologist in another part of the UK. Where possible this was done using NHS web servers and email, but a back-up system was established using other systems with suitable encryption. A total of 1007 EEGs were reported in 13 months from patients of all ages, epilepsy being the commonest reason for referral. All cases were reported within 24 h of recording. The back-up system was used in 60 cases. Tele-EEG is a feasible, secure, timely and effective method of providing an EEG service to hospitals which cannot recruit a resident clinical neurophysiologist.

Potatoes produce a number of toxic secondary metabolites, which are divided into two groups: the sesquiterpenes and the glycoalkaloids (PGAs): whereas PGAs are largely preformed and present in toxic quantities in both the foliage and "green" potatoes, it is well documented that the levels of PGAs and sesquiterpenes are effected by many biotic an abiotic stresses. The development of genetically modified potato varieties has made it prudent to ascertain whether there may be changes in the amounts or types of these secondary metabolites either as a direct effect of the transgene or due to its interactions with environmental variables. Transgenic potato lines were exposed, along with nontransgenic lines, to a range of biotic and abiotic stresses and a range of environmental conditions in the field and store. Following stressing, a comparison was made of levels of potato glycoalkaloid and sesquiterpene levels between the two groups. Significant differences were observed in the levels of both glycoalkaloid and sesquiterpene levels between transgenic and control material and between infected and noninfected material.

We initially evaluated in mice the ability of naked DNA encoding intracellular forms of the E1E2 envelope proteins from HCV to induce antibody responses and compared the responses induced with the same plasmid adsorbed onto cationic poly (lactide co-glycolide) (PLG) microparticles. Although naked DNA was only able to induce detectable responses at the 100 microg dose level, making this approach impractical for evaluation in larger animals, PLG/DNA induced detectable responses at 10 microg. In addition, the PLG/DNA microparticles induced significantly enhanced responses to naked DNA when compared at the same dose level. Remarkably, PLG/DNA induced comparable responses to recombinant E1E2 protein adjuvanted with the emulsion MF59. Furthermore, PLG/DNA effectively primed for a booster response with protein immunization, while naked DNA did not. Therefore, PLG/DNA was selected for further evaluation in a non-human primate model. In a study in rhesus macaques, PLG/DNA induced seroconversion in 3/3 animals following three immunizations. Although the antibody responses appeared lower than those induced with recombinant protein adjuvanted with MF59, following a fourth dose, PLG/DNA and protein induced comparable responses. However, a single booster dose of recombinant protein administered to the animals previously immunized with PLG/DNA induced much higher responses. In addition, one of three animals immunized with PLG/DNA showed a cytotoxic T lymphocyte response in peripheral blood lymphocytes. In conclusion, cationic PLG microparticles with adsorbed HCV DNA generates potent immune responses.
