La instrumentación de Kaneda en la estabilización vertebral anterior / Kaneda instrumentation in anterior vertebral stabilization

Objetivo: Analizar las deformidades raquídeas neuromusculares y los resultados de su tratamiento quirúrgico con instrumentación tipo Galveston-Luque. Diseño experimental: Estudio retrospectivo de los enfermos intervenidos siguiendo dicha técnica. Pacientes: Se revisan 34 enfermos con deformidades raquídeas intervenidos entre 1989-1999 mediante abordaje posterior e instrumentación tipo Galveston-Luque, con una edad media de 14,7 años. Resultados: Se analizan los valores pre y postoperatorios de los siguientes parámetros radiológicos: en el plano frontal: Escoliosis media de 73° corregida a 35°; traslación corporal de 5,5 a 0,8 cm; traslación apical de 5,9 a 2 cm; rotación vertebral de 32° a 22°; oblicuidad pélvica de 24° a 7°. En el plano sagital: se encontraron 4 tipos de alteraciones: Cifosis colapsante de 89° corregida a cifosis 42° y lordosis 42°; lordosis única: 59° corregida a cifosis 17° y lordosis 40°; inversión de curvas con lordosis torácica de 24° y cifosis lumbar de 42°, llevadas a curvas fisiológicas de 26° y 49°; hipercifosis e hiperlordosis de 71° y 58°, corregidos a 46° y 43°, respectivamente. Las complicaciones médicas encontradas fueron: 1 muerte tardía, 2 lesiones medulares parciales, 4 neumonías, 4 dehiscencias de heridas, una infección profunda y 2 infecciones tardías. Tras un seguimiento medio de 62 meses, se observó un deterioro del 8 por ciento en la escoliosis y del 16 por ciento en la oblicuidad pélvica. La cifosis progresó en 6 casos, en 4 de ellos por instrumentación corta y en 2 por arrancamiento de la fijación. Conclusión: La instrumentación Galveston-Luque permite una buena corrección tridimensional de las deformidades raquídeas neuromusculares así como la rápida recuperación funcional con unas complicaciones asumibles (AU)

[The immunogenicity of the vaccine against the HBV in the mentally retarded]. / Inmunogenicidad de la vacuna contra el VHB en deficientes mentales.

Our group carried out during 1989 a serological cross sectional study in 1,024 mentally handicapped patients (231 with Down's syndrome [DS] and 793 affected with other mental disturbances [OMD]), educated in seven institutions of the Madrid region. Those patients with negative HBsAg and anti-HBc markers were considered vaccination susceptible individuals. This group was constituted by 546 individuals. The regime of vaccination used was 0-1-6 m, and the dose, 20 micrograms i.m. The present study makes an evaluation of the postvaccine serologic results one month after the third dose. It was considered the age, sex and if he or she suffers from Down's syndrome or not. Corresponding with these variables the seroconversion rate (percentage of patients with greater than or equal to 10 IU/l of anti-HBs) was determined as well as the geometric mean titer (GMT) of antibodies. The seroconversion rate was 97.1% in the group affected with DS and 95.5% in the group with OMD, not being this difference statistically significant. There is not also significant association with regard to the sex, being the seroconversion rate in males (94.7%) and females (97.6%) very similar. However, it is significant difference between the mean age of the "responders" to the vaccine (24.6 years of age) and that of the "non responders" (33.3 years of age) (p less than 0.001). Thus, the age exerts a negative influence in the response to the vaccination. The titer of anti-HBs antibody, expressed using the GMT, has significant association with the sex (p less than 0.001) but not with the type of mental disturbance.(ABSTRACT TRUNCATED AT 250 WORDS)
