RESUMO

PURPOSE: To analyze astigmatism axis changes after tropicamide and phenylephrine combined instillation. METHOD: One hundred and thirty-one eyes from 66 patients enrolled this cross-sectional study. An extensive ocular examination was carried out prior to tropicamide and phenylephrine instillation. Power and axis value from flat, steep, and mean keratometry were calculated using an Auto Kerato-Refractometer (AKR). Later, topography and tomography maps were evaluated with Pentacam HR® (Oculus, Wetzlar, Germany). Subsequently, a single drop of tropicamide 1% and phenylephrine hydrochloride 10% were instilled twice, with a five-minute gap between each instillation. After 30 minutes, the AKR and Pentacam HR® tests were repeated. RESULTS: Incyclotorsion was found in 59 eyes (45.1%) and mean absolute incyclotorsion change was 3.91 ± 3.62 degrees (0.10 to 14.20). Excyclotorsion was found in 72 eyes (54.9%) and mean excyclotorsion change was 4.99 ± 5.94 degrees (0.20 to 36.20). We observed that 74.6% and 68.1% of eyes experienced incyclotorsion and excyclotorsion within 0 to 5 degrees, respectively. Fewer patients experienced incyclotorsion and excyclotorsion changes within 5 to 10 degrees, precisely 11.8% and 19.4%, respectively. Eyes that experienced over 10 degrees of incyclotorsion and excyclotorsion were 13.6% and 12.5%, respectively. CONCLUSION: Astigmatism axis could change after combined tropicamide and phenylephrine instillation. Reference axis marking in astigmatism correction surgery should be performed under the same circumstances as the astigmatism axis has been measured.

RESUMO

This work focuses on obtaining realistic human hand models that are suitable for manipulation tasks. A 24 degrees of freedom (DoF) kinematic model of the human hand is defined. The model reasonably satisfies realism requirements in simulation and movement. To achieve realism, intra- and inter-finger constraints are obtained. The design of the hand model with 24 DoF is based upon a morphological, physiological and anatomical study of the human hand. The model is used to develop a gesture recognition procedure that uses principal components analysis (PCA) and discriminant functions. Two simplified hand descriptions (nine and six DoF) have been developed in accordance with the constraints obtained previously. The accuracy of the simplified models is almost 5% for the nine DoF hand description and 10% for the six DoF hand description. Finally, some criteria are defined by which to select the hand description best suited to the features of the manipulation task.

Se describe el caso clínico de una paciente desnutrida de la República de Djibouti, de 24 años de edad y procedencia rural, atendida en el Servicio de Urgencia de Cirugía por presentar dolor en hipogastrio, fijo, mantenido y punzante, que se intensificaba progresivamente, además de náuseas y vómitos biliosos. En la exploración bimanual se palpó una masa anexial en el lado derecho, muy dolorosa, de casi 8 cm. Los exámenes realizados confirmaron la presencia de un quiste de ovario torcido y posible tuberculosis peritoneal, lo cual se comprobó con el diagnóstico peroperatorio. La joven evolucionó satisfactoriamente y fue trasladada al Servicio de Medicina para tratamiento antibacilar(AU)


The clinical case of a undernourished 24-year-old patient of the Republic of Djibouti who lived in a rural region is described. She was attended in the Emergency Department of Surgery due to a fixed, continuous and stabbing hypogastric pain, which was intensified progressively besides nauseas and bilious vomiting. A very painful adnexal mass of almost 8 cm was palpated in the right side with a bimanual exploration. Examinations confirmed the presence of a bent ovaric cyst and a possible peritoneal tuberculosis, which was proven with intraoperative diagnosis. She made a good progress and was transferred to the Medicine Service for antibacillary treatment(AU)

RESUMO

Se revisa la literatura en algunos aspectos (etiología, anatomía patológica, manifestaciones clínicas y diagnóstico). Se presenta un caso diagnosticado como feocromocitoma de la glándula suprarrenal derecha, por medio del ultrasonido y retroneumoperitoneo y confirmado en la intervención quirúrgica y el estudio anatomopatológico. Se destacan desde el punto de vista radiológico los valores de las diferentes investigaciones y se comparan con otros autores; se comprueba el alto valor diagnóstico del método, con la característica de ser inocuo y no invasivo(AU)
