Abstract Antineutrophil cytoplasmic antibodies (ANCAs) are associated with small vessel vasculitis but their prevalence is not rare in other immune diseases. In lupus nephritis (LN), their pathological role and clinical relevance have been the target of controversial views. We present a case of acute kidney injury and nephrotic syndrome in a young woman with diffuse global proliferative and membranous nephritis on her kidney biopsy, showing a full-house immunofluorescence pattern, very allusive of class IV + V LN, but lacking associated clinical criteria and laboratory findings to support the diagnosis of systemic lupus erythematosus (SLE). Furthermore, the patient presented with high titers of ANCA, steadily decreasing alongside the renal function and proteinuria improvements, with mycophenolate mofetil (MMF) and steroid treatment. The authors believe this is a case of lupus-like nephritis, in which ANCAs are immunological markers, although they are not directly involved in the pathogenesis.
Resumo Os anticorpos anticitoplasma de neutrófilos (ANCAs) estão associados à vasculite de pequenos vasos, no entanto, a sua prevalência não é rara em outras doenças imunológicas. Na nefrite lúpica (LN), o seu papel patológico e relevância clínica têm sido alvo de pontos de vista controversos. Apresentamos um caso de lesão renal aguda e síndrome nefrótica em uma jovem com nefrite proliferativa difusa e membranosa em sua biópsia renal, muito alusivo a NL classe IV + V, com um padrão full house na imunofluorescência, mas sem critérios clínicos e achados laboratoriais para corroborar o diagnóstico de lúpus eritematoso sistêmico (LES). Não obstante, a paciente apresentou títulos elevados de ANCA, que diminuiram progressivamente com a melhoria da função renal e da proteinúria, após tratamento com micofenolato de mofetil (MMF) e esteróide. Os autores acreditam que se trata de um caso de nefrite semelhante à nefrite lúpica, em que os ANCAs são marcadores imunológicos, embora não estejam diretamente envolvidos na patogênese.

Fast and accurate taxonomic identification of invasive trans-located ladybird beetle species is essential to prevent significant impacts on biological communities, ecosystem functions, and agricultural business economics. Therefore, in this work we propose a two-step automatic detector for ladybird beetles in random environment images as the first stage towards an automated classification system. First, an image processing module composed of a saliency map representation, simple linear iterative clustering superpixels segmentation, and active contour methods allowed us to generate bounding boxes with possible ladybird beetles locations within an image. Subsequently, a deep convolutional neural network-based classifier selects only the bounding boxes with ladybird beetles as the final output. This method was validated on a 2, 300 ladybird beetle image data set from Ecuador and Colombia obtained from the iNaturalist project. The proposed approach achieved an accuracy score of 92% and an area under the receiver operating characteristic curve of 0.977 for the bounding box generation and classification tasks. These successful results enable the proposed detector as a valuable tool for helping specialists in the ladybird beetle detection problem.

Auscultation is a routine exam and the first line of screening in heart pathologies. The objective of this study was to assess if using a new data collection system, the DigiScope Collector, with a guided and automatic annotation of heart auscultation, different levels of expertise/experience users could collect similar digital auscultations. Data were collected within the Heart Caravan Initiative (Paraíba, Brasil). Patients were divided into two study groups: Group 1 evaluated by a third year medical student (User 1), and an experienced nurse (User 2); Group 2 evaluated by User 2 and an Information Technology professional (User 3). Patients were auscultated sequentially by the two users, according to the randomization. Features extracted from each data set included the length (HR) of the audio files, the number of repetitions per auscultation area, heart rate, first (S1) and second (S2) heart sound amplitudes, S2/S1, and aortic (A2) and pulmonary (P2) components of the second heart sound and relative amplitudes (P2/A2). Features extracted were compared between users using paired-sample test Wilcoxon test, and Spearman correlations (P<;0.05 considered significant). Twenty-seven patients were included in the study (13 Group 1, and 14 Group 2). No statistical significant differences were found between groups, except in the time of auscultation (User 2 consistently presented longer auscultation time). Correlation analysis showed significant correlations between extracted features from both groups: S2/S1 in Group 1, and S1, S2, A2, P2, P2/A2 amplitudes, and HR in Group 2. Using the DigiScope Collector, we were able to collect similar digital auscultations, according to the features evaluated. This may indicate that in sites with limited access to specialized clinical care, auscultation files may be acquired and used in telemedicine for an expert evaluation.
