INTRODUCTION: Childhood abuse is linked to poorer well-being, yet some survivors show no prolonged effect, suggesting multifinality. Men and women also differ in the experience and sequelae of abuse supporting gender-specific analyses. To assist in circumventing poor outcomes associated with child abuse, this study examined fear of self (FoS) and self-compassion (SC) as risk and protective factors between child abuse and well-being. METHOD: Australian women (N = 1302, Mage = 47.28 [13.63]) completed an online assessment of childhood abuse, psychological distress, perceived quality of life, SC, FoS. Moderated-moderation examined the different abuses as predictors of psychological distress and quality of life (QoL) and FoS and SC as primary and secondary moderators, respectively. RESULTS: Physical abuse did not predict psychological distress or QoL. FoS and SC were independent predictors of distress and QoL. Similarly, childhood sexual abuse was not found to predict distress or QoL. FoS and SC were independent predictors of distress and QoL. There was also an interaction between childhood sexual abuse and SC in predicting QoL. A moderated-moderation was found for the relationship between child sexual abuse and distress. Psychological abuse predicted distress and QoL, and FoS and SC both moderated this relationship. Moreover, a moderated-moderation was observed for the relationship between psychological abuse and QoL. CONCLUSIONS: Greater FoS is associated with poorer outcomes for child abuse survivors; SC may mitigate this impact. Psychological abuse was the strongest predictor of well-being. Interventions that increase SC and raise awareness of psychological abuse and its impacts are needed.

BACKGROUND: Current literature reports widely different rates of distant metastasis at presentation for squamous cell carcinoma of the head and neck (SCCHN). We used the Surveillance Epidemiology and End Results (SEER) database to determine the rate of and risk factors for distant metastasis. METHODS: We identified patients with SCCHN diagnosed between 1988 and 2003. The rate of distant metastasis was determined and then stratified by tumor site, size, and nodal status. To evaluate the contribution of each individual factor, both univariate and multivariate analyses were performed. RESULTS: Among 73,247 patients identified, 2066 patients (2.82%, 95% confidence interval [CI] 2.70-2.94%) had distant metastasis. Independent risk factors for distant metastasis included primary site, nodal status, tumor size, age, and race. CONCLUSIONS: In SCCHN, there is a relationship between the risk of distant metastasis and tumor site, size, and nodal status. This information can be used to guide the evaluation and treatment of SCCHN.

Community has historically provided an essential psycho-spiritual framework for nursing. Changes in communication technology pose challenges for nurses internationally who create communities across borders. This article discusses The Bamboo Bridge online community, a project responding to the global call for nursing education about the complementarity of nursing and healing traditions. The project explores how technologies such as Centra and Ning promote community building and encourage belonging in members from 5 continents and 10 countries. This article includes detailed accounts of the project design, examples of cultural diplomacy as the emerging theoretical framework, and an African member's perspective of online community.

Tracheoesophageal puncture prostheses (TEPPs) are an integral aspect of speech rehabilitation for many patients who have undergone total laryngectomy. Because one flange of the prosthesis sits in the trachea and the other in the esophagus, these devices can be aspirated or swallowed if dislodged. Five cases of prosthesis aspiration that occurred in 4 veterans within a 16-month period are described. The 5 aspirated TEPPs resulted in highly variable clinical presentations ranging from complaints of "lost" TEPPs in asymptomatic patients to near-asphyxiation. Furthermore, the aspirated TEPPs were not reliably demonstrated on chest radiographs, often leading to delayed diagnosis. Aspiration of TEPPs may be more common than formerly recognized, and chest computed tomography or bronchoscopy is indicated in cases of missing TEPPs not demonstrated on plain films.
