RESUMO

El síndrome Ramsay-Hunt asocia parálisis facial periférica acompañada de una erupción vesicular en pabellón auricular o cavidad oral, causada por la afectación del ganglio geniculado por el virus varicela-zóster. La afectación facial en el zóster ótico es inicialmente más severa y con peor pronóstico de recuperación que la parálisis de Bell. El tratamiento precoz de esta enfermedad suele incluir aciclovir intravenoso u oral, analgésicos, corticoides y medidas de protección ocular; encaminado todo ello a mejorar el pronóstico de curación. Presentamos un caso de síndrome de Ramsay-Hunt en una mujer de 65 años con mala evolución (AU)


Ramsay-Hunt syndrome is peripheral facial palsy combined with a vesicular rash in the pinna or oral cavity. It is caused by the varicella zoster virus infection of the geniculate ganglion. Facial condition in the Zoster Oticus initially has a more severe and worse prognosis of recovery from Bell's palsy. Early treatment of this pathology usually consists of intravenous treatment or oral acyclovir, analgesics, corticosteroids and eye protection measures, in order to improve the probability of cure. We present a case of Ramsay-Hunt syndrome with a poor outcome in a 65 year-old woman (AU)

RESUMO

El conocimiento del inglés avanza a pasos de gigante entre la población mundial, y muy especialmente entre la europea, una tendencia de la que los profesionales sanitarios ni debemos ni podemos quedar al margen, porque lograr una comunicación fluida y eficaz con los pacientes es una de las competencias básicas de nuestra profesión. Lo que pretendemos con este artículo es facilitar con un guión de conversación bilingüe (español-inglés), la elaboración de una historia clínica en urgencias graves rurales. El conocimiento del inglés para cualquier profesional en el área de la salud constituye una necesidad evidente en un mundo donde la lengua de comunicación por excelencia es la inglesa. El guión bilingüe contribuye a la realización de un diagnóstico sindrómico inicial en la atención urgente de un paciente extranjero y al mismo tiempo mediante el guión facilitamos la cumplimentación del registro de pacientes extranjeros atendidos en nuestra zona básica de salud (AU)


English language skills are growing fast among the world's population, and especially in Europe. Health professionals cannot and should not be excluded from this group, as achieving fluent and effective communication with foreign patients is a basic skill in our profession. The basic purpose of this paper is to provide a simple bilingual script that may help to prepare a medical record in emergency medical situations in rural areas. The knowledge of English for any professional in the area of Health is an obvious need in a world where the language of communication is English. The bilingual script contributes to the achievement of an initial syndromic diagnoses in the urgent care of a foreign patient, and with this script, we make it easier to register the foreign patients treated in our health area (AU)

RESUMO

A 39-years-old male patient with chronic venous insufficiency, deep venous thrombosis and recurrent pulmonary embolism in the past medical history. After syncopal event was diagnosed of bilateral chronic pulmonary embolism, pulmonary hypertension and right ventricular failure. Fibrinolytic treatment was no effective therapeutic modality. Under cardiopulmonary bypass, bilateral pulmonary thromboendarterectomy with extension into lobe arteries, plus insertion of caval filter was performed. We present our experience with this case and a review of the literature.

RESUMO

A unique case of transitional cell carcinoma of the bladder with the primary being in the anus is reported. The patient presented with acute urinary retention from a large bladder tumor, which was thought to be primary to the bladder. Biopsy of the bladder revealed transitional cell carcinoma, microscopically indistinguishable from the primary cloacogenic carcinoma diagnosed seven years previously.

RESUMO

We studied 73 patients with transitional cell carcinoma of the bladder seen from 1967 to 1973. The red cell antigen test was done in a controlled blind study to determine if tumor behavior can be predicted by this test. Our results do not substantiate the findings of other investigators who have reported excellent correlation. We did not find statistical evidence that loss of antigen could be equated to a poor prognosis. We noted that patients in group O gave the weakest reaction and that this test had to be done in a modified fashion in tumors from these patients. It also was noted that as individuals age they tend to lose the red cell antigen. There is no doubt that this test, as reported in the literature, is a great advance in the study of bladder cancer. Further refinement in technique and more data are needed to substantiate its accuracy in predicting the behavior of bladder cancer.
