RESUMO

Prophylaxis or treatment of tumor-associated seizures is adaily concern in neurosurgical practice but is often guided by the surgeon's habits rather than evidence from clinical trials, which is lacking. The present study reviews the literature on the incidence, clinical aspects and treatment of epilepsy and epileptic seizures in patients undergoing surgery for meningioma. Based on the published data, we then performed a French nationwide survey of neurosurgeons' practices regarding perioperative management of meningioma-related epilepsy and epileptic seizures.

BACKGROUND/AIMS: The treatment of common bile duct stones diagnosed during videolaparoscopic cholecystectomy is still under debate. In cases of suspected common bile duct stones, a double approach with endoscopic retrograde cholangiopancreatography either prior to, or following videolaparoscopic cholecystectomy is the current routine in many centers. An intraoperative endoscopic retrograde cholangiopancreatography with endoscopic papillosphincterotomy and stone extraction has recently been proposed. METHODOLOGY: We compared the approaches for suspected common bile duct stones in 21 cases of combined intervention endoscopic retrograde cholangiopancreatography during videolaparoscopic cholecystectomy to 17 cases of sequential intervention (endoscopic retrograde cholangiopancreatography prior to videolaparoscopic cholecystectomy). Complications and postoperative monitoring are discussed and reported on the basis of hospital stay. RESULTS: Although the efficacy and the complications are similar, patients treated with the sequential approach stayed in the hospital longer because of the double monitoring period during both after endoscopic retrograde cholangiopancreatography and after videolaparoscopic cholecystectomy. CONCLUSIONS: A combined approach to suspected common bile duct stones during videolaparoscopic cholecystectomy could be an effective and a financially worthwhile treatment.

In paediatric age there may occur the various types of cystic renal tumor also of different clinical meaning; we can count among them shapes openly or potentially deadly, among which Wilms tumor cystic owing to haemorrhages and tumoral necrosis, congenital mesoblastic cystic nephroma, clear cell carcinoma, cystic, partially differentiated nephroblastoma (CPDN) and benign forms such as multilocular cystic nephroma (MLCN), cystic localized dysplasia of the kidney. In some cases, the differential diagnosis is difficult and it becomes only by histology: particularly the differential diagnosis between MLCN and CPDN, indistinguishable on the basis of the imaging and the macroscopic aspect, it is possible only by histology. It is described a case of multicystic lesion in a baby, male, aged twenty-two months, with an indolent mass of left side, screened by US and TC. The US examination has put into evidence the presence of renal with clean outlines of eight cms, formed by multiple small cysts, divided by thin regular septa . These aspects have been confirmed by TC. The histological examination has diagnosed a multilocular cystic nephroma, without atypias. Therefore, the multilocular cystic lesions of the kidney have to be considered potentially malignant and submitted to surgical treatment.

RESUMO

A case of aggressive angiomyxoma (AAM) of the pelvis and perineum is reported, whose clinical and pathological findings are consistent with the literature. This is a clinicopathological entity recently described by Rosai and colleagues (1983) who stressed its locally infiltrative nature and its tendency to recur. Its differential diagnosis includes a series of benign and malignant soft tissue tumours.

RESUMO

A personal series of colo-rectal tumours from 1979 to 1987 has been examined so as to check the validity of CEA as a marker in the diagnosis of these tumours. The data confirmed what has already been reported, namely its poor diagnostic sensitivity even though its utility for prognosis and in the follow-up of these patients remains confirmed, as is shown by the literature.

RESUMO

550 patients given abdominal or extra-abdominal surgery in the first six months of 1980 were examined to provide data on the incidence of delayed postoperative canalisation. Both general (sex, age, physical constitution) and surgical (extent of surgery, whether the peritoneal sac was opened) factors were considered. The data collected provide information on the prophylaxis and therapy of delayed postoperative canalisation.
