RESUMO

Machine learning and deep learning advancements have boosted Brain-Computer Interface (BCI) performance, but their wide-scale applicability is limited due to factors like individual health, hardware variations, and cultural differences affecting neural data. Studies often focus on uniform single-site experiments in uniform settings, leading to high performance that may not translate well to real-world diversity. Deep learning models aim to enhance BCI classification accuracy, and transfer learning has been suggested to adapt models to individual neural patterns using a base model trained on others' data. This approach promises better generalizability and reduced overfitting, yet challenges remain in handling diverse and imbalanced datasets from different equipment, subjects, multiple centres in different countries, and both healthy and patient populations for effective model transfer and tuning. In a setting characterized by maximal heterogeneity, we proposed P300 wave detection in BCIs employing a convolutional neural network fitted with adaptive transfer learning based on Poison Sampling Disk (PDS) called Active Sampling (AS), which flexibly adjusts the transition from source data to the target domain. Our results reported for subject adaptive with 40% of adaptive fine-tuning that the averaged classification accuracy improved by 5.36% and standard deviation reduced by 12.22% using two distinct, internationally replicated datasets. These results outperformed in classification accuracy, computational time, and training efficiency, mainly due to the proposed Active Sampling (AS) method for transfer learning.

RESUMO

This survey reviews advances in 3D object detection approaches for autonomous driving. A brief introduction to 2D object detection is first discussed and drawbacks of the existing methodologies are identified for highly dynamic environments. Subsequently, this paper reviews the state-of-the-art 3D object detection techniques that utilizes monocular and stereo vision for reliable detection in urban settings. Based on depth inference basis, learning schemes, and internal representation, this work presents a method taxonomy of three classes: model-based and geometrically constrained approaches, end-to-end learning methodologies, and hybrid methods. There is highlighted segment for current trend of multi-view detectors as end-to-end methods due to their boosted robustness. Detectors from the last two kinds were specially selected to exploit the autonomous driving context in terms of geometry, scene content and instances distribution. To prove the effectiveness of each method, 3D object detection datasets for autonomous vehicles are described with their unique features, e. g., varying weather conditions, multi-modality, multi camera perspective and their respective metrics associated to different difficulty categories. In addition, we included multi-modal visual datasets, i. e., V2X that may tackle the problems of single-view occlusion. Finally, the current research trends in object detection are summarized, followed by a discussion on possible scope for future research in this domain.

RESUMEN Introducción: La elaboración de colgajos ha representado un cambio en reconstrucción de defectos resultantes de exéresis de tumoraciones de base de cráneo. No siempre indispensables, existen circunstancias, planificados o no, donde debemos emplearlos. Preservando los pedículos de la mucosa, damos cobertura a urgencias intraquirúrgicas, como fístula LCR no sospechada, así como reintervenciones. Objetivo: Presentar nuestra experiencia en pacientes, a quienes hemos realizado colgajo tipo rescue flap. Material y método: Se diseña este colgajo, sin comprometer su pedículo. En caso de fístula, sospecha de ésta o herniación del diafragma selar, se extiende, cubriendo el defecto. Resultados: De 34 pacientes intervenidos de patología hipofisaria endoscópicamente, en 18 diseñamos colgajo tipo rescue flap. 12 casos se elaboraron, no utilizándolos. En 4 pacientes con extenso tumores lo empleamos preventivamente. En 1 caso, de reintervención, previamente con colgajo Hadad izquierdo, realizamos colgajo de mucosa contralateral. En otro, diseñamos un rescue flap derecho, al objetivar salida de LCR, sellamos con este colgajo. No evidenciamos fístulas. Discusión: Esta técnica consiste en levantar parcialmente mucosa del potencial colgajo, preservando su pedículo, pudiendo utilizarse en casos de fístula LCR no programada, o reintervenciones. Sin realizar colgajos innecesariamente. Conclusión: La técnica rescue flap favorece un corredor quirúrgico, menos invasivo, manteniendo mucosa para eventuales reintervenciones.


ABSTRACT Introduction: The development of flaps has represented a change in reconstruction of defects resulting from excision of skull base tumors. It not always indispensable, there are circumstances, planned or not, where we must use them. Preserving the pedicles of the mucosa, we cover intraoperative emergencies, such as unsuspected CSF fistula, as well as reinterventions. Aim: We present our experience where we performed rescue flap. Material and method: This flap is designed without compromising its pedicle. In case of fistula, suspicion of this or herniation of the selar diaphragm, it extends, covering the defect. Results: Of 34 patients who underwent endoscopic surgery for pituitary pathology, in 18 we designed a rescue flap. 12 cases were made, not using them. In 4 patients with extensive tumors we used it preventively. In 1 case, of reintervention previously with left Hadad flap, we performed contralateral mucosa flap. In another, we designed a right rescue flap, when we observed LCR output, we seal with this flap. We did not show fistulas. Discussion: This technique consists in partially lifting the mucosa of the potential flap, preserving its pedicle, and may be used in cases of unscheduled CSF fistula, or reinterventions. Without flapping unnecessarily. Conclusions: The Rescue Flap technique favors a less invasive surgical corridor, maintaining mucosa for posible reinterventions.

Genetic variability related to the host immune system has been proposed as one of the most influential factors in the development of diseases caused by HPV. CLINICAL CASE: We report the case of a 5-year-old child in whom chronic laryngeal papillomatosis, probably acquired vertically during labor, was detected. The diagnosis of laryngeal papillomatosis was confirmed with a biopsy after a first surgery to remove the papillomas. The Derkay classification system was used to assess the severity of papillomatosis. Biopsy genotyping was performed by demonstrating HPV-6. Later, HLA-DQA1 * 0505, -DQB1 * 0301, -DRB1 * 1101 alleles were homozygous for HLA allele typing. CONCLUSIONS: Further studies are needed to identify the most prevalent HLA alleles in the Latino population and their potential association with genetic susceptibility in Recurrent Respiratory Papillomatosis.

La variabilidad genética relacionada al sistema inmune del huésped ha sido propuesta como uno de los factores más influyentes en el desarrollo de enfermedades causadas por HPV. Caso clínico: Reportamos el caso de un niño de 5 años en cuyo estudio por disfonía crónica se encuentra papilomatosis laríngea probablemente adquirida por vía vertical durante el parto. El diagnóstico de papilomatosis laríngea se confirmó con una biopsia tras una primera cirugía orientada a remover los papilomas. Se utilizó el sistema de clasificación Derkay para evaluar la severidad de la papilomatosis. Se realizó genotipificación en biopsia demostrándose HPV-6. Posteriormente mediante tipificación de alelos HLA se demostró homocigosis para los alelos HLA-DQA1*0505, -DQB1*0301, -DRB1*1101. Conclusiones: Se necesitan estudios adicionales que permitan identificar los alelos HLA más prevalentes en población latina y su potencial asociación con la susceptibilidad genética en Papilomatosis Respiratoria Recurrente.

Antecedentes: los buenos resultados motvaron el uso del polipropileno a partr de los 90. La infección de la malla es escasamente publicada, por lo que su manejo es motvo de controversia. Objetvo: señalar la incidencia y la complejidad del tratamiento no conservador de las prótesis infectadas. Lugar de aplicación: hospital público.Diseño: estudio retrospectvo observacional de historias clínicas. Población: Desde enero de 2005 hasta diciembre de 2012, 411 pacientes con patología de la pared abdominal se operaron utlizando prótesis. De ellos, se infectaron 6 y son el motvo de esta presentación. Método: se consideraron dos grupos: Grupo A prótesis de 400 cm², 71 pacientes (17,3%) y Grupo B prótesis entre 40 y 70 cm², 340 pacientes (82,75%). Se evaluó el tempo de aparición de la infección, el germen involucrado, la interface huésped-prótesis, las comorbilidades y el tratamiento. Resultados: se infectaron 4 pacientes del Grupo A (5,63%) y 2 pacientes del Grupo B (0,58%). La fis-tula crónica con secreción purulenta luego de los 20 meses fue el motvo de consulta en las prótesis grandes. Staphylococcus aureus es el germen más frecuente, y un bacilo de Koch se diagnosticó en la interface huésped-prótesis de una hernioplasta. Fueron infructuosos los tratamientos conservadores, por lo cual la explantación de las prótesis del Grupo A se realizó en forma temprana a través de cirugías complejas e iteratvas. La internación superó los 10 días y la reinserción laboral los 3 meses. Conclusión: las prótesis grandes deben ser removidas en su mayoría cuando se presentan con infección crónica, con trayectos fistulosos y secreción purulenta.
