INTRODUCTION AND AIMS: An anastomotic leak is one of the most dreaded complications in colorectal surgery because it increases postoperative morbidity and mortality. The aim of the present study was to identify whether indocyanine green fluorescence angiography (ICGFA) reduced the anastomotic dehiscence rate in colorectal surgery. MATERIAL AND METHODS: A retrospective study on patients that underwent colorectal surgery with colonic resection or low anterior resection and primary anastomosis, within the time frame of January 2019 and September 2021, was conducted. The patients were divided into the case group, in which ICGFA was performed for the intraoperative evaluation of blood perfusion at the anastomosis site, and the control group, in which ICGFA was not utilized. RESULTS: A total of 168 medical records were reviewed, resulting in 83 cases and 85 controls. Inadequate perfusion that required changing the surgical site of the anastomosis was identified in 4.8% of the case group (n = 4). A trend toward reducing the leak rate with ICGFA was identified (6% [n = 5] in the cases vs 7.1% in the controls [n = 6] [p = 0.999]). The patients that underwent anastomosis site change due to inadequate perfusion had a 0% leak rate. CONCLUSIONS: ICGFA as a method to evaluate intraoperative blood perfusion showed a trend toward reducing the incidence of anastomotic leak in colorectal surgery.

INTRODUCTION AND AIM: Anastomosis leak occurs in 1-19% of colorrectal surgeries. Our objective was to present the first Mexican case series on colorrectal surgery using indocyanine green fluorescence angiography to evaluate perfusion prior to carrying out the anastomosis. MATERIALS AND METHODS: A retrospective, analytic, descriptive study was conducted. We studied the case records of consecutive patients that underwent colorrectal surgery with indocyanine green angiography performed by the same group of colorrectal surgeons. RESULTS: Twenty-one case records were reviewed. Eleven (52.3%) of the patients were women, mean patient age was 57 years (38-82), and mean body mass index was 25 kg/m2 (17-34). Fifteen (71.4%) patients were diagnosed with malignant disease. Indocyanine green angiography changed our therapeutic decision in three (14.2%) patients. Two colorrectal anastomoses (14.2%) were performed at fewer than 5 cm from the anal verge and 13 (61.9%) were performed at more than 5 cm from the anal verge. Three of the anastomoses were ileocolic (14.2%), two were coloanal (9.5%), and one was ileoanal (4.7%). There were six (28.5%) complications, no cases of anastomotic leak, and no complications associated with the use of indocyanine green. The mortality rate was 0%. CONCLUSION: The present case series is the first on colorrectal surgery conducted in Mexico using indocyanine green fluorescence angiography, with excellent results.

OBJECTIVES: We aimed to report the first 54 cases of pregnant women infected by Zika virus (ZIKV) and their virologic and clinical outcomes, as well as their newborns' outcomes, in 2016, after the emergence of ZIKV in dengue-endemic areas of São Paulo, Brazil. METHODS: This descriptive study was performed from February to October 2016 on 54 quantitative real-time PCR ZIKV-positive pregnant women identified by the public health authority of São José do Rio Preto, São Paulo, Brazil. The women were followed and had clinical and epidemiologic data collected before and after birth. Adverse outcomes in newborns were analysed and reported. Urine or blood samples from newborns were collected to identify ZIKV infection by reverse transcription PCR (RT-PCR). RESULTS: A total of 216 acute Zika-suspected pregnant women were identified, and 54 had the diagnosis confirmed by RT-PCR. None of the 54 women miscarried. Among the 54 newborns, 15 exhibited adverse outcomes at birth. The highest number of ZIKV infections occurred during the second and third trimesters. No cases of microcephaly were reported, though a broad clinical spectrum of outcomes, including lenticulostriate vasculopathy, subependymal cysts, and auditory and ophthalmologic disorders, were identified. ZIKV RNA was detected in 18 of 51 newborns tested and in eight of 15 newborns with adverse outcomes. CONCLUSIONS: Although other studies have associated many newborn outcomes to ZIKV infection during pregnancy, these same adverse outcomes were rare or nonexistent in this study. The clinical presentation the newborns we studied was mild compared to other reports, suggesting that there is significant heterogeneity in congenital Zika infection.

BACKGROUND: The quantitative effect of strong electrolytes, pCO2 , and plasma protein concentration in determining plasma pH and bicarbonate concentrations can be demonstrated with the physicochemical approach. Plasma anion gap (AG) and strong ion gap (SIG) are used to assess the presence or absence of unmeasured anions. HYPOTHESES: The physicochemical approach is useful for detection and explanation of acid-base disorders in horses with colitis. AG and SIG accurately predict hyperlactatemia in horses with colitis. ANIMALS: Fifty-four horses with acute colitis and diarrhea. METHODS: Retrospective study. Physicochemical variables were calculated for each patient. ROC curves were generated to analyze sensitivity and specificity of AG and SIG for predicting hyperlactatemia. RESULTS: Physicochemical interpretation of acid-base events indicated that strong ion metabolic acidosis was present in 39 (72%) horses. Mixed strong ion acidosis and decreased weak acid (hypoproteinemia) alkalosis was concomitantly present in 17 (30%) patients. The sensitivity and specificity of AG and SIG to predict hyperlactatemia (L-lactate > 5 mEq/L) were 100% (95% CI, 66.4-100; P < .0001) and 84.4% (95% CI, 70.5-93.5 P < .0001). Area under the ROC curve for AG and SIG for predicting hyperlactatemia was 0.95 (95% CI, 0.86-0.99) and 0.93 (95% CI, 0.83-0.99), respectively. CONCLUSION AND CLINICAL RELEVANCE: These results emphasize the importance of strong ions and proteins in the maintenance of the acid-base equilibria. AG and SIG were considered good predictors of clinically relevant hyperlactatemia.

The presence of some dental abnormalities as a predisposal factor to installation of localized periodontal alterations. The present study was to evaluate the frequency of development abnormalities present on a sample of molars that may predispose to occurrence and progression of periodontal disease. Two hundred and seventy seven molars (141 maxillary and 136 mandibular molars) were selected for this study. The evaluations were performed by two examiners together who was trained and calibrated for the study. All measurements were made by direct visualization with a magnifying glass. The following clinical parameters were measured: Enamel cervical projections; Root trunk: in the three different extensions (<3 mm, >3 mm and <6 mm, >6 mm); Enamel pearls: presence (1) or absence (0). The anatomic observations were made on four surfaces of the tooth: mesial, distal, vestibular and palatal/lingual. Descriptive statistics and percentage distribution were performed. A total of 130 molars showed at least some degree of enamel cervical projection and the most frequent score was the degree 1, according to Masters and Hoskins classification. Fifteen teeth presented enamel pearls and the root trunk was more evident on the group with variation from 3 to 6 mm of extension, observed in 64 teeth. It can be concluded that the presence of development abnormalities is a frequent finding in molars and it has to be taken into consideration during periodontal examination and therapy.

La respuesta de estres ha surgido en la naturaleza como un mecanismo para proteger al individuo retornándolo a su homeostasis. Como parte de dicha actividad se involucran mecanismos celulares y humorales que en forma de cascadas interreguladas pueden lograr el objetivo primordial. En ocasiones las lesiones primarias desencadenan respuestas sistemicas severas que ponen en peligro la vida del sujeto. La capacidad de respuesta oy control del estrés está determinada geneticamente, lo que afecta tanto el metabolismo intermediario como el de sintesis y la capacidad de fosforilacion mitocondrial. Los diferentes cambios morfologicos en los organelos celulares determinaron si hay reparacion o si se supera el punto de no retorno. Como parte de las ayudas terapeuticas se tienen: la optimizacion de la volemia; el logro de la atenuacion y el pago temprano de la deuda de oxigeno; el aporte de los nutrientes y cofactores que preferencialmente utilice el organismo para obtener energia; y finalmente, debe continuarse el análisis de las condiciones experimentales y clinicas en las que se utilizan moduladores de las respuestas primarias o antagonistas, especificos de las respuestas antibacterianas.
