RESUMO

Resumen El liquen mixedematoso (LM) representa un grupo de enfermedades cutáneas raras, el cual se encuentra dentro de las mucinosis crónicas. Anteriormente descrita como escleromixedema localizado, sin embargo, a diferencia de éste, por lo general no tiene compromiso sistémico. Dentro de los subtipos, se encuentra el LM atípico, el cual es infrecuente y hay pocos casos reportados asociados a mieloma múltiple (MM). Se presenta el caso de un paciente masculino con MM positivo para cadenas lambda, con cuadro clínico de inicio agudo, en quien se realizó diagnóstico de LM atípico; recibió manejo con corticoide tópico con mejoría de las lesiones al mes de tratamiento.


Abstract Lichen myxedematous (LM) represents a group of rare skin diseases, which is found within the chronic mucinoses. Previously described as localized scleromyxedema, however, unlike localized scleromyxedema, it usually does not have systemic involvement. Among the subtypes, there is atypical LM, which is infrequent and there are few reported cases associated with multiple myeloma (MM). We present the case of a male patient with MM positive for lambda chains, with acute onset clinical picture, who was diagnosed with atypical LM; he received management with topical corticosteroid with improvement of the lesions after one month of treatment.

BACKGROUND: Associations between mental health and dermatology have generated a new branch of study called psychodermatology, which includes relevant conditions to the adolescent population. However, there is limited research focusing on this field and this specific population. OBJECTIVE: To estimate the prevalence of dermatological pathologies in the Colombian adolescent population and their possible associations with mental health conditions. METHODS: We performed a secondary analysis of the Colombian National Mental Health Survey results. A representative sample of the adolescent Colombian population answered questions regarding dermatological conditions and mental health disorders. We estimated the prevalence and evaluated associations using the chi squared independence test. RESULTS: Of 1753 adolescents, 8.3% (CI95%=[6.8-10.1]) had dermatitis or skin allergies and 4.5% (CI95%=[3.3-6.1]) acne. For dermatitis, we found associations with age, education, and family dysfunction (p-Values=0.024; <0.001; 0.046 respectively). Acne was associated with age, sex, educational level, the number of social groups involved in, alcohol consumption, psychoactive substances use and previous violent experiences (respective p-Values=0.007; 0.004; 0.005; 0.036; 0.002; 0.003; 0.044). Regarding mental health, dermatological conditions were associated with depression, affective disorders, suicide attempt and suicide ideation. CONCLUSION: Our results are the first to describe the prevalence of these disorders in Colombia. This is an exploratory study; nonetheless, it is of great value since it is the first to describe these associations in adolescents in a middle-income country, which should be considered during clinical examinations. Further longitudinal studies evaluating possible causal relationship between psychiatric and dermatological conditions are fundamental to establish causal links.

El artículo describe el caso de un paciente de 28 años de edad con antecedente de VIH, quien presentaba múltiples lesiones en la piel y las mucosas, producto de sífilis secundaria, y quien mostraba un amplio espectro de manifestaciones cutáneas características de esta enfermedad.


We describe the case of a 28-year-old patient with HIV history who has multiple skin and mucous lesions that showing a broad spectrum of cutaneous manifestations of secondary syphilis.

RESUMO

La mayoría de hormonas tiene influencia directa en la piel lo cual permite la regulación de los procesos biológicos normales que mantienen la homeostasis necesaria para que la barrera cutánea establezca mecanismos que permitan diferenciar las noxas, externas o internas, de las señales relevantes y, de esta manera, cuantifique y traslade dichas señales a respuestas definidas no sólo locales, sino muchas veces sistémicas. La complejidad del sistema neuroinmuno-endocrino-cutáneo, representado por cada uno de los componentes de la piel a través de sus unidades anatomofuncionales, permite reconocer en este órgano un sinnúmero de posibles mecanismos fisiológicos y muchas veces implicados en patologías de expresión cutánea que pueden ser reconocidos de manera oportuna por el clínico. Uno de estos hallazgos clínicos en la piel son los producidos por la disfunción tiroidea
