RESUMO

BACKGROUND: Evaluate the effect of a community pharmaceutical intervention on the control of blood pressure in hypertensive patients treated pharmacologically. METHODS: A cluster-randomized clinical trial of 6 months was carried out. It was conducted in the Autonomous Community of Castilla-La Mancha (Spain). Sixty-three community pharmacies and 347 patients completed the study. Intervention patients received the community pharmaceutical intervention based on a protocol that addresses the individual needs of each patient related to the control of their blood pressure, which included Health Education, Pharmacotherapy Follow-up and 24 h Ambulatory Blood Pressure Measurement. Control patients received usual care in the community pharmacy. RESULTS: The pharmaceutical intervention resulted in better control of blood pressure (85.8% vs. 66.3% p < 0.001), lower use of emergencies (p = 0.002) and improvement trends in the physical components of quality of life, measured by SF-36 questionnaire, after 6 months of pharmaceutical intervention. No significant changes were observed for any of these variables in the control group. There were also detected 354 negative medication-related outcomes that were satisfactorily resolved in a 74.9% of the cases and 330 healthcare education interventions and 29 Ambulatory Blood Pressure Monitorings were performed in order to increase adherence to pharmacological treatment and minimize Negative Outcomes associated with Medication and prevent medication-related problems. CONCLUSIONS: Community pharmaceutical intervention can increase hypertensive patients with controlled blood pressure, after 6 months, compared with usual care.

La farmacia y las ciencias farmacéuticas abarcan una serie de disciplinas diferentes. La Farmacia Asistencial se ha definido como “la disciplina científica que estudia los diferentes aspectos de la práctica de la farmacia y su impacto en los sistemas de atención de la salud, el uso de medicamentos y la atención al paciente”. Por lo tanto, los estudios de Farmacia Asistencial abarcan tantos elementos de farmacia clínica como de farmacia social. Como cualquier otra disciplina científica, la práctica de la farmacia clínica y social difunde los resultados de la investigación utilizando revistas científicas. Los editores de revistas de farmacia clínica y farmacia social tienen un papel en la promoción de la disciplina al mejorar la calidad de los artículos publicados. Como ha ocurrido en otras áreas del cuidado de la salud (es decir, medicina y enfermería), un grupo de editores de revistas de práctica farmacéutica clínica y social se reunió en Granada, España, para discutir cómo las revistas pueden contribuir a fortalecer la práctica farmacéutica como disciplina. El resultado de esa reunión se compiló en estas Declaraciones de Granada, que comprenden 18 recomendaciones reunidas en seis temas: el uso apropiado de la terminología, los resúmenes con impacto, la necesidad de la revisión por pares, la dispersión de revistas, el uso más eficaz y más inteligente de los indicadores bibliométricos y la selección por parte de los autores de la revista de práctica farmacéutica más adecuada para presentar su trabajo. Presentamos el caso de una mujer de 92 años, que sufre un problema de salud agudo y acude a los servicios de urgencias de 2 centros sanitarios. La falta de acceso de los profesionales sanitarios a la historia clínica/farmacológica de la paciente, origi-na cambios en la medicación y una prescripción en cascada que tiene como consecuencia el deterioro de la salud en una paciente estable.Cuando un paciente acude a distintos profesionales para resolver un problema de salud, la falta de un sistema común de repositorio de esa información con acceso directo de los profesionales lastra la resolución de los problemas de salud (AU)


We present the case of a 92 years old woman that suffers an acute health problem and goes to the emergency services of 2 health care centers. The lack of access of the health professionals to the clinical and pharmacological record of the patient, produces changes in the medication and a big quantity of prescriptions that have as a conse-quence the worsening of health of a stable patient.When a patient goes to different health profession-als to solve a health problem, the lack of a common repository system with the information and with direct access for the professionals affects health problems resolution (AU)
