A problem attracting considerable attention in Sweden today is the substantial regional differences in sickness absence. The aim of the study was to investigate and compare how people, from a random sample of the population in both a rural area in the north of Sweden and the Swedish capital Stockholm, perceive their health, and what their attitudes are to work, leisure time and social welfare systems. Results showed that a larger proportion of those answering in Stockholm considered their health status to be "very good", compared with those in the rural area (p<0.0001). A majority in the rural area compared to the city of Stockholm reported a high or very high level of aches/pain (p<0.0001) and that work causes them physical problems p<0.0001). The population in both Stockholm and the rural area is of the opinion that the increase in sickness absence is mainly due to deterioration in the work environment. Almost half of the individuals in both the rural area and in Stockholm are of the opinion that many of those sick-listed are not actually ill. It may be that in the rural area in north Sweden people are more inclined to put their opinions to practice than those in Stockholm are.

One important aspect of the nurse-patient relationship is nurses' attitudes towards their patients. Nurses' attitudes towards people with dementia have been studied from a wide range of approaches, but few authors have focused on the structure of these attitudes. This study aimed to identify a structure in licensed practical nurses' attitudes towards people with dementia. Twenty-one group dwelling units for people with dementia at 11 nursing homes participated in the study. A total of 1577 assessments of 178 patients were sent out to 181 respondents and 1237 answers were returned. The semantic differential technique was used. The scale had 57 bipolar pairs of adjectives that estimate an unknown number of dimensions of nurses' attitudes towards an identified patient. The assessments were analysed using entropy-based measures of association combined with structural plots. The analysis revealed four dimensions, which related to licensed practical nurses' opinions of the patients: an ethical and aesthetic dimension; an ability to understand; an ability to experience; and an ability for social interaction. The results of the study indicated that, on the positive to negative attitude continuum, the nurses' attitudes fell at the positive to neutral end. This is an important finding owing to the personhood perspective, from which it is reasonable to assume that, with a more positive attitude to people with dementia, the prerequisites for person-centred care will improve.

Nursing care can figuratively be described as a gift that is given by the nurse in the nurse-patient relationship where attitudes play an important role for the gift's appearance. Sometimes patients are unwilling to or incapable of accepting the gift and this can lead to situations in which nurses are not able to handle their situation in a professional manner. This research survey aimed to investigate nurses' attitudes and find a structure in nurses' attitudes towards their patients. Six psychiatric group dwellings and six acute psychiatric hospital wards participated in the study. In all, 2700 assessments of 163 patients were sent out to 160 respondents and 2436 answers were returned. The semantic differential technique was used. The scale has 57 bipolar pairs of adjective, which estimate an unknown number of dimensions of nurses' attitudes towards an identified patient. The respondents' answers were analysed through factor analysis rotated using the Varimax method and etropy-based measures of association combined with structural plots were also used. Both the factor analysis and the entropy revealed three factors, which were interpreted as being of evaluative type. Factor 1 was interpreted as describing nurses' answers mainly as a combination of an ethical and aesthetic evaluation of the patients. Factor 2 was interpreted as being of an empathetic type and factor 3 as nurses' experiences of the patients' cognitive capacity. The study indicates that the dominant aspect of nurses' attitudes in a psychiatric context is the ethical/aesthetic dimension, which is an important finding for the understanding of nurses' attitudes and actions towards their patients.

Among personnel in primary healthcare and psychiatric care, the ability to detect early signs of psychosis was examined and compared. Differences due to sex, age, profession or time in the profession were explored. Three different vignettes concerning patients with mental illness were presented to the respondents, and every respondent was to mark what they thought were early signs of psychosis in the vignettes. The findings were compared with a pre-set standard of symptoms and signs. Healthcare personnel from two different administrative districts, Umeå and Ornsköldsvik in northern Sweden, participated in the study. The response rate was 77%, but for district nurses in the administrative district of Umeå it was only 25%, so therefore complementary interviews with nine district nurses in that district, representing the drop-outs, were performed. There was no difference whatsoever in ability to detect early signs of psychosis between personnel in primary healthcare and psychiatric care; nor were there any significant differences due to sex, age, profession or time in their profession. Detecting early signs of psychosis requires training and the ability to communicate with patients, as well as a special sensitivity to psychiatric conditions. This can be achieved by enhancing accessibility and continuity of primary healthcare for these patients. Furthermore, a greater sensitivity to early signs of psychosis must be created among all healthcare personnel. The co-operation between primary healthcare and psychiatric care must increase. Primary care physicians need opportunities to see patients over time in order to correctly interpret early signs and symptoms of psychiatric illness.

It is known that proliferation and survival of neural stem/progenitor cells in vitro not only depend on exogenous factors, but also on autocrine factors secreted into the conditioned medium. It is also well known that the identification of bioactive proteins secreted into the conditioned medium poses a substantial challenge. Recently, neural stem/progenitor cells were shown to secrete a survival factor, cystatin C, into the conditioned medium. Here, we demonstrate an approach to identify other low molecular weight proteins in conditioned medium from cultured adult rat hippocampal progenitor cells. A combination of preparative two-dimensional gel electrophoresis (2-DE) and mass spectrometry was utilized in the analysis. We were able to identify a number of proteins, which include Rho-guanine nucleotide dissociation inhibitor 1, phosphatidylethanolamine binding protein (PEBP), also termed Raf-1 kinase interacting protein, polyubiquitin, immunophilin FK506 binding protein 12 (FKBP12) and cystatin C. The presence of PEBP and FKBP12 in conditioned medium was confirmed immunologically. All nestin-positive progenitor cells showed immunoreactivity for antibodies against PEBP and FKBP12. To our knowledge we are the first to use this preparative proteomic approach to search for stem cell factors in conditioned medium. The method could be used to identify novel bioactive proteins secreted by stem/progenitor cells in vitro. Identification of bioactive proteins in vitro is of potential importance for the understanding of the regulatory mechanisms of the cells in vivo.
