RESUMO

Xanthogranulomatous pyelonephritis is a rare disease that may mimic various renal lesions, especially renal tumors, in its focal form. We report a case of pseudoneoplastic xanthogranulomatous pyelonephritis in a young woman. This case is unusual, because it was diagnosed by a percutaneous renal puncture and furthermore total renal recovery was achieved by antibiotic treatment alone.

RESUMO

Surgical complications of renal transplantation, rejection and infectious diseases are factors contributing to poor renal graft survival. Factors directly concerning the donor can be involved in graft failure: age, medical history, causes of donor brain death. Urologic or arterial anatomic variations are often the source of difficult surgical conditions during renal transplantation. Technical errors during graft procurement must be avoided such as excess of traction or coagulation. Failure in perfusion preservation. As few renal grafts are available, it is thus essential to obtain optimal conditions to avoid failure in cadaver donor graft linked to technical errors during organ procurement.

RESUMO

The authors present a research carried out by the Universities of Essen, Tubingen (RFA) and Grenoble, France, on the use of magnetic resonance imaging (M.R.I.) in gynaecology. After clarifying our knowledge of the normal anatomy of the pelvis and of the tissular characteristics of MRI the principal indications are discussed and set out. The investigation is shown to be particularly valuable in working out the aetiology, the volume and the spread of a pelvic mass whether it is benign or malignant when it is important to be exact in finding the origin of the tumour. This research has been illustrated by analysing 35 case histories including 28 malignant tumours. A second study was carried out on 30 cases of cancer of the cervix. In this field MRI is the only test that can be carried out before therapy to give a tridimensional assessment of the size of the tumour and whether it has spread into the parametrium. The visual impressions obtained by MRI illustrate each chapter: of the gynaecological anatomy, of the pathology, in cancers of the cervix, in pelvic masses and particularly in ovarian tumours.

RESUMO

Three patients with straight azygos vein continuation of inferior vena cava (IVC) were studied by nuclear magnetic resonance (NMR) imaging. In all three cases the diagnosis had been established previously by ultrasound and/or computed tomography imaging, but NMR images provided better anatomical precision of the venous anomaly : visualization of the total trajectory of the major azygos vein, the caliber of which can exceed that of aorta, and absence of retrohepatic segment of IVC but presence of a short supradiaphragmatic segment towards which converge the suprahepatic veins; anastomotic etwork between IVC and major azygos vein contributing to ensure continuity of venous drainage superior to its renal segment. The advantages of NMR are described and the precise diagnostic role of this new method of imaging in the diagnosis of this type of anomaly discussed.

RESUMO

Results of nuclear magnetic resonance exploration in a patient with chronic thrombosis of main pulmonary arteries are used to outline an elementary semiology in agreement with current documented data. Signs observed relate to the thrombosis and showing of flow due to associated pulmonary artery hypertension. Cardiac synchronization is essential: obtaining 2 echos by the spin-echo technique allows differentiation of circulatory slowing phenomena, which provoke increased strength of 2nd echo, from the thrombus itself. Correlations established with V/Q scintigraphy, angiography and CT scan findings in this case provided preliminary evaluation of use of this imaging technique in this affection.
