RESUMO

The authors examined 40 patients with malignant tumors of various histogenesis, sites and extent, as well as 5 patients with benign tumors and other non-tumorous diseases. They also studied their electroencephalography and peripheral blood lymphocytic and erythrocytic ultrastructure in metabolic alkalosis temporarily induced by intravenous sodium hydrogen carbonate. In cancer patients without late metastases, alkalosis caused a transient normalization of previously altered electroencephalography, erythrocyte disaggregation and substantially reduced the count of killer cells in small and middle lymphocytes. These findings suggest that patients with malignant neoplasms have a generalized intracellular acidosis which can be temporarily abolished by plasma alkalinization.

RESUMO

23 patients with malignant tumors of different location and histogenesis were investigated. There were no metastases in 9 cases. 10 patients had metastases in regional areas and 4--distant. The results were compared with those obtained in 4 patients with nonmalignant diseases. EEG, blood gases, plasma acid--base balance and ultrastructure of erythrocytes were explored before and after intravenous infusion of 4.2% sodium bicarbonate solution. The metabolic alkalosis induced amelioration of EEG, which was changed basically, the condense of pre-membrane layer disappeared or decreased in erythrocytes, and disaggregation of erythrocytes took place in cancer patients vs those with nonmalignant tumors. The results confirm the suggestion of generalized intracellular acidosis in malignant tumor patients. This acidosis can be temporarily avoided or diminished artificially by blood alkalosis.

RESUMO

A 3-5-min spontaneous hyperventilation caused normalization of initially altered electroencephalogram and inactivation of hypothalamic structures in 14 patients with malignant tumors, as well as negative dynamics of bioelectrical brain activity and activation of diencephalic area in 6 healthy subjects. In hyperventilation ACTH plasma concentration increased 13-fold on average (from 14.2 +/- 12.1, to 185 +/- 82 pg/ml) in normal subjects and 2.4-fold (from 45.5 +/- 19.8 to 110 +/- 17 pg/ml) in oncological patients. It is assumed that changes in hypothalamo-hypophyseal reactivity in patients with malignant neoplasias can be associated with generalized intracellular metabolic acidosis, partially, compensated by gas alkalosis in the plasma due to hyperventilation.

RESUMO

A parallel study of bioelectrical activity of the brain and urine excretion of 17-hydroxycorticosteroids was carried out in 107 patients with various malignant tumors of hemopoietic tissue before, during and after treatment. In all of them, electroencephalography showed excitation of the hypothalamus, the central component of the hypothalamo-pituitary-adrenal system. Moreover, the patients revealed a significant increase in the excretion of 17-hydroxycorticosteroids that pointed to the activation of the peripheral component--the adrenal cortex. Activation of both central and peripheral components suggests that its intermediate component--the pituitary--is activated in patients with hemopoietic system neoplasms, too.

Changes in bioelectric activity of the brain were observed in 56 patients with disseminated breast cancer which pointed to the activation of the hypothalamus. Hyperglycemia, with blood-sugar levels being 4-6 times the basal normal one, was induced in 16 patients by a single injection of glucose. As a result, a 50-80% transient improvement in electroencephalographic indexes was obtained, hypothalamic excitation subsiding and optimal indexes matching peaks of hyperglycemia. It is concluded that the said changes in the bioelectric activity of the brain in cancer patients are largely of compensatory nature and should be regarded as a response to tumor-induced hypoglycemia.
