[Overview of the general management of acute leukemia for adults]. / Principes généraux de la prise en charge des leucémies aiguës de l'adulte.

Acute leukemias are a heterogeneous group of malignant hemopathies which are subdivided according to the cytological orientation of the pathological blast cell into lymphoblastic (ALL) and myeloblastic (AML) acute leukemias. Recent advances in the biological and genetic understanding of these diseases have led to improved treatments. Specific chemotherapy treatment or so-called «targeted¼ treatments, advances in bone marrow transplantation and better supportive care have gradually improved the prognosis. This review, focused on the adult patient, aims to describe recent progress in terms of diagnosis, prognostic markers and therapy.

Les leucémies aiguës sont un ensemble hétérogène d'hémopathies malignes qui se subdivisent, en fonction de l'orientation cytologique de la cellule blastique pathologique, en formes lymphoblastique (LLA) et myéloblastique (LMA). Les récents progrès dans la compréhension biologique et génétique de ces maladies ont permis d'améliorer les traitements. Le traitement spécifique de chimiothérapie ou traitements dits «ciblés¼, les progrès de la greffe de moelle et de meilleurs soins de support ont permis d'améliorer graduellement le pronostic. Cette revue, centrée sur le patient adulte, a pour objectif de décrire les progrès récents en termes de diagnostic, de marqueurs pronostiques ainsi que thérapeutiques.

[Diagnosis of anemia associated with alcoholic cirrhosis]. / Diagnostic différentiel de l'anémie dans la cirrhose éthylique.

We report here the case of a 62-year-old patient with Child-Pugh stage C ethylic cirrhosis associated with severe macrocytic anaemia, refractory to iterative transfusions and withdrawal. After a haemorrhagic, deficiency-related, or sideroblastic etiology was ruled out, haemolytic anaemia was suspected. A blood smear allowed diagnosis of haemolytic anaemia with acanthocytes. This offers the opportunity to discuss anaemia in patients with alcoholic cirrhosis, a frequent complication spanning a broad severity range and having the potential to be life-threatening. Its origin can be multifactorial : acute haemorrhage, dilution, haemolysis (here due to acanthocytosis), marrow insufficiency caused by direct alcohol toxicity, malnutrition, iron deficiency, vitamin B9 or B12 deficiency, chronic inflammation, splenic sequestration induced by portal hypertension...

Nous rapportons le cas d'une patiente de 62 ans atteinte d'une cirrhose éthylique de stade Child-Pugh C associée à une anémie macrocytaire sévère, réfractaire aux transfusions itératives et au sevrage. Après avoir exclu les étiologies hémorragiques, carentielles et sidéroblastiques, une anémie hémolytique (AH) est suspectée. La réalisation d'un frottis sanguin a permis le diagnostic d'une anémie hémolytique à acanthocytes. L'opportunité nous est donnée de discuter de l'anémie chez le patient cirrhotique alcoolique, complication fréquente recouvrant un large spectre de gravité et pouvant menacer la survie. Elle peut être multifactorielle : hémorragie aiguë, dilution, hémolyse (dans le cas particulier, liée à une acanthocytose), insuffisance médullaire par toxicité directe de l'alcool, malnutrition, carence martiale, déficit en vitamine B9 ou B12, inflammation chronique, séquestration splénique induite par l'hypertension portale….

Scurvy, a disease caused by a deficit in vitamin C, is again described in Europe nowadays. This condition is associated with poverty and malnutrition, as found in developing countries. However, denutrition may also occur with a "western" lifestyle that can lead to nutriment deficiencies. In the following report is described a case of scurvy in a person suffering from alcoholism.

Le scorbut, expression clinique du déficit en vitamine C, est une maladie en recrudescence dans nos sociétés souffrant d'une précarité grandissante et adoptant de façon systématique une alimentation «occidentale¼ carencée en fruits et légumes. Le cas présenté illustre ce problème chez une personne éthylique à risque.

[Miliary tuberculosis in a patient under adalimumab]. / Tuberculose miliaire chez une patiente sous adalimumab : cas clinique.

We describe a case of atypical miliary tuberculosis diagnosed by molecular testing in a patient suffering from psoriasis arthritis treated by adalimumab. Tuberculosis may have a non-classical presentation in patients under biological treatments. We briefly discuss the difficulties underlying the diagnosis and treatment of tuberculosis, especially in patients suffering from fever of unknown origin.

Nous rapportons un cas de tuberculose (TB) miliaire chez une patiente souffrant de polyarthrite psoriasique traitée par adalimumab. L'opportunité nous est ainsi donnée d'illustrer la possibilité de présentations atypiques, en particulier chez les patients traités par agents biologiques. Nous discutons brièvement, aussi, des difficultés inhérentes au diagnostic et au traitement de la tuberculose.
