RESUMO

AIM: This study examined the role of job dimensions, job satisfaction, psychological stress and job burnout in predicting turnover intention at one and five years' time among professional nurses in the Philippines. METHODS: A cross-sectional design was utilized in the study using a convenience sample of 549 registered nurses from six hospitals in the Central Philippines during the period of October 2018 to January 2019. Five self-report scales were used in this study. RESULTS: Overall, 46.1% (n = 253) and 78.9% (n = 433) of nurses reported planning to leave the organization at one and five years' time, respectively. The type of hospital, job satisfaction and job burnout explained variances for turnover intention at one and five years' time. Separately, hospital bed capacity and job stress predicted turnover intention at five years' time. None of the job dimensions accurately predicted turnover intention in nurses. CONCLUSION: Higher turnover intention results at one and five years' time were dependent on nurses' psychological stress, job burnout and job satisfaction. IMPLICATIONS FOR NURSING PRACTICE AND POLICY: Organizational measures aimed at reducing stress and burnout and increasing job satisfaction in nurses are critically important to enhance the retention of nurses and ensure continued quality of nursing care.

RESUMO

INTRODUCTION: A lot of evidence has demonstrated the importance of different cytokines in acute renal rejection. Previous studies have examined the presence or absence of interleukin (IL)-10 in related immunopathologic rejection grafts as well as other interleukins. Studies in human transplantation show elevated levels of IL-10 and gamma interferon (INF-γ) in inflammation and rejection. OBJECTIVE: The objective of this study was to demonstrate the lack of association of elevated urinary levels of IL-10 and IFN in the presence of active inflammation. METHODS: An observational, descriptive, cross-sectional study conducted in transplant recipients at 12 months of follow-up after renal transplantation. In those who were held biopsy after renal transplantation at one year follow-up, or allograft dysfunction, we also measured IL-10 and INF-γ in the urine. The following were considered as variables: age, body mass index (BMI), gender, transplant type, creatinine, chronic kidney disease epidemiology collaboration equation, (CKD-EPI), modification of diet in renal disease study equation (MDRD), Banff classification, and levels of IL-10 and INF-γ. Statistical analysis was performed calculating a sample size of 25 patients, with an alpha bias of 0.05%, yielding measures of central tendency and determining no association between levels of IL-10 and INF-γ with the presence of rejection using SPSS 21.0 program. RESULTS: A total of 50 patients, 34 (68%) males, 16 (32%) females, with an average 31.7 ± 9.9 years, weight of 64.91 ± 13.84 kg, size 1.60 ± 0.10 m and 24.97 ± 4.07 BMI were included,39 (78%) living donor and 11 (22%) cadaveric. Twenty-six (52%) showed inflammation in the biopsy and 24 (48%) showed none. Mean creatinine was 1.81 ± 1.5, and the estimated glomerular filtration rate (eGFR) was 55.27 ± 22.46, 65.76 ± 26.7. (MDRD and CKD-EPI, respectively). No statistical difference was found in the levels of IL-10 and IFN-γ using analysis of variance. (ANOVA; P = .467 and P = .063, respectively) Based on Banff, the inflammation on biopsy score was 2.78 ± 2.84. There was statistical significance (P < .05) with respect to the Cr and eGFR by different equations. There were no significant interactions between cytokine levels and more than 1 factor. (as indicated by P < .2). DISCUSSION AND CONCLUSIONS: No significant differences were observed in the level of interleukins in patients with and without inflammation, denoting an adequate immunosuppression in most of these patients. Determination of inflammatory cytokines in urine could be used as a determinant of a good immunosuppression status, rather than as an early marker of rejection.

RESUMO

This study compared the effects of different bedding depths on the chemical and microbiological characteristics of the bedding material used to raise pigs during growing and finishing. The experiment was conducted in two pens housing 5 pigs from 60 to 145 days of age, with rice husk beddings 0.50 or 0.25 m deep. Four lots of pigs (replicates) were raised over time in each bedding depth: each bedding was used by two consecutive lots. Bedding samples were collected quarterly to determine the most probable number (MPN) of thermophilic and mesophilic bacteria, fungi and actinomycetes. Contents of N, P, K, C, organic, mineral and dry matter, C:N ratio and pH were also determined. The MPN of thermophilic bacteria was higher for the 0.50 m than for the 0.25 m bedding (p<0.05). The compost of 0.25 m deep bedding had a higher N, P and K content than that from the 0.50 m bedding (p<0.05). Thus, the use of the 0.25 m deep bedding would be recommended due to its greater agronomical value in comparison with the deeper bedding.

RESUMO

1. The effect on feed efficiency of two probiotics, one prepared with Saccharomyces boulardii and the other with Bacillus cereus var. toyoii, was tested in broilers infected with Salmonella enteritidis. 2. One-day-old chicks were divided at random into three groups and fed commercial feed devoid of antibiotics: group 1 was fed with non-supplemented feed, group 2 was supplemented with S. boulardii and group 3 with B. cereus. At 14 d of age the animals were challenged by the oral route with 1 x 10(7) viable S. enteritidis. 3. At d 47, average live weights were: group 1, 1.77 kg, group 2, 1.89 kg and group 3, 2.06 kg, and were significantly different. Feed conversion rates were 2.61 for group 1, 2.35 for group 2 and 2.30 for group 3. 4. We conclude that both probiotics improved feed efficiency in broilers.

RESUMO

Male breast cancer accounts for less than 1% of all breast cancers in the United States. The low incidence of this disease prevents therapeutic questions from being addressed in prospective randomized trials. As such, treatment algorithms have generally been extrapolated from those used in female breast cancer. Similar to female breast cancer, lymph node involvement and the size of the primary tumor are the most important factors in predicting disease-free and overall survival. Modified radical mastectomy remains the standard local therapeutic approach, with radiation therapy indicated for patients who have clinicopathologic features associated with a high risk for postmastectomy chest wall or lymph node recurrence. Similar to female breast cancer, adjuvant chemotherapy or hormonal therapy is indicated in the majority of men with breast cancer, specifically those with tumors larger than 1 cm or those with lymph node-positive disease.

RESUMO

PURPOSE: This study reports 21 patients with Stage I-III low-grade non-Hodgkin's lymphoma who were treated with comprehensive lymphatic irradiation (CLI) at the University of Florida between 1966 and 1992. METHODS AND MATERIALS: Sites clinically involved with disease were treated with 30 Gy, whereas clinically uninvolved sites were treated with 25 Gy. Median follow-up for the group was 14 years (24.5 years for Stage III patients). RESULTS: Overall absolute survival rates at 5, 10, and 15 years were 84%, 68%, and 34%. Cause-specific survival rates at 5, 10, and 15 years were 84%, 68%, and 56%. Freedom-from-relapse rates at 5, 10, and 15 years were 75%, 58%, and 58%, with no relapses noted after 10 years. Bulky disease (>6 cm) was a significant indicator of poor prognosis for cause-specific survival (p = .01). CONCLUSION: These data support findings from other institutions suggesting a role for CLI as potentially curative therapy with acceptable toxicity and a short treatment time for patients with Stages I and II and limited Stage III disease.
