RESUMO

Children with special health care needs face many barriers to oral care and are at high risk for oral disease. School nurses are in a unique position to promote oral wellness in this vulnerable population. Collaboration between school nurses and dental hygiene faculty resulted in the formation of a partnership between a university-based dental hygiene program and two special education districts in rural southern Illinois. Senior dental hygiene students participated in a school-based service-learning project that provided dental examinations, preventive services, and education to children with special health care needs. Evidence-based behavioral interventions were used to teach children to comply with oral procedures. School nurses mentored dental hygiene students in behavior management of children. Dental exams were provided to 234 children from four special education schools with the majority receiving cleanings and fluoride.

RESUMO

Child abuse and neglect have reached epidemic proportions. State and federal laws mandate that allied health professionals report suspected child abuse and neglect. Academic and continuing education programs could assist allied health professionals to be legally responsible, mandated reporters of child abuse and neglect. This study examined the training needs of allied health students to be legally responsible in reporting child abuse and neglect. A questionnaire was administered to 236 students enrolled in 5 allied health programs. The questionnaire addressed the knowledge base and perceptions of allied health students in regard to child abuse and neglect. Results revealed that most allied health students felt inadequately prepared academically in the topic of child abuse and neglect. Only half of the students knew they could be charged with a crime for failure to report suspicions of child abuse and neglect. Nursing students, female students, and students older than 25 years were those likely to report child abuse and neglect. Students appear to have insufficient preparation in the areas of knowing how to report child abuse and neglect as well as their responsibilities to do so as allied health professionals.

RESUMO

Local health departments can be resources for the public and health professionals to access educational materials on a variety of topics. Though the dental office is ideal, it should not be the only venue for obtaining oral cancer educational materials. As part of a cancer prevention and early detection project, this pilot study solicited printed educational materials concerning oral cancer from all local health departments (LHDs) in Illinois. Sixty of the 94 health departments responded, for a response rate of 63.8%. Only 32 had printed oral cancer educational materials for public and professional distribution, and most focused on tobacco. Forty-eight different samples were received. At least one risk factor was identified in all materials. The most common was tobacco usage, present in 100% of the materials. Twenty-nine (61%) identified tobacco use as the only risk factor. Alcohol consumption, the second most frequently identified risk factor, was mentioned in only 12 (26%) of the materials. Few materials comprehensively covered areas such as signs and symptoms, risk factors, and the importance of examinations for early detection. Only five materials (11%) acknowledged that early lesions are often asymptomatic. Local health departments are in an influential position to disseminate educational materials to the public and health professionals. Dental hygienists have the knowledge and expertise required to identify quality materials. They should be proactive in assisting LHDs with acquisition of appropriate printed materials and partner with them to advocate for prevention and early detection of oral cancer. In collaboration, the two can ensure that all LHDs have current and comprehensive printed materials about oral cancer to distribute to the public and other health professionals.

RESUMO

Few dentists in a rural Midwestern community participate in providing oral health care to public aid recipients. In response, faculty at a baccalaureate degree dental hygiene program located at Southern illinois University, Carbondale (SIUC) proposed, developed, and implemented the Heartland Dental Clinic to serve Medicaid participants. The unique program utilizes existing facilities, staff, and students to provide comprehensive oral health care to underserved populations. The state awarded a small grant to cover start-up costs. Two dental units were upgraded with fiber optics to allow restorative procedures. Dental hygiene students provide intake examinations and preventive care, while a staff dentist provides restorative care, dentures, and examinations. Dental technology students and faculty fabricate prostheses. A part-time clinic manager facilitates communication, patient scheduling, and billing. Two local Rotary Club members volunteer as receptionists for the clinic on the one evening per week that the clinic operates. The Rotary Club purchased educational pamphlets, a television/VCR, videotapes, and two signs for the clinic. By locating the clinic in the existing SIUC facility and utilizing dental hygiene students, a staff dentist, volunteer receptionists and dentists, student workers, and health care management interns, the clinic overhead costs have been kept to a minimum. The clinic provides a unique opportunity for dental hygiene students to experience firsthand scheduling, billing, and treating public aid patients while providing patients with an additional source for oral health care. The Heartland Dental Clinic model represents a cost effective method for increasing oral health access to underserved populations while also benefiting students in an educational program.
