RESUMO

Skeletal dysplasia in achondroplasia can affect all body joints - including the glenohumeral joint - and is prone to develop to degenerative osteoarthritis (OA). This may cause pain and mobility problems at young age. Surgical treatment is challenging due to the dysplastic anatomy of the shoulder joint - with a dysplastic deformed short humerus, a small, hypoplastic medialized glenoid and lateralized acromion - and the long life expectancy of these patients. The indications for reverse shoulder arthroplasty (RSA) evolved during years with rotator cuff tears and rotator cuff arthropathy in combination with or without glenohumeral OA as the main indicator, with good short to mid-term results. Long term results of RSA are rarely found in literature, especially in young patients. The use of a RSA in glenohumeral OA with an intact rotator cuff has rarely been reported. In this case report we present the ten-year clinical and radiographic results of a RSA for the treatment of degenerative OA with glenohumeral dysplasia in a young patient with achondroplasia.

Internal impingement is a commonly described cause of shoulder pain in the overhead athlete, particularly in tennis players. Three shoulder dysfunctions, often correlated with internal impingement symptoms, require attention in the rehabilitation strategy of internal impingement in the tennis player: (1) acquired glenohumeral anterior instability, (2) loss of internal rotation range of motion, and (3) lack of retraction strength. Based on recent literature, the following guidelines are proposed in the rehabilitation of the tennis player with internal impingement symptoms: (1) shoulder rehabilitation should be integrated into kinetic chain training, not only in the advanced phases of the athlete's rehabilitation, but from the initial phases; (2) both angular and translational mobilisations can be used in the treatment of acquired loss of glenohumeral internal rotation range of motion to stretch the posterior structures of the glenohumeral joint; and 3) in the rehabilitation of scapular dyskinesis, the therapist should focus on restoration of intramuscular trapezius muscle balance in the scapular exercises, with special attention to strength training of the retractors.

We compared normalized trapezius muscle activity and intramuscular balance ratios between overhead athletes with impingement symptoms and non-injured athletes during isokinetic abduction and external rotation movements. Thirty-nine overhead athletes with chronic impingement symptoms and 30 non-injured athletes participated in the study. Electromyographic activity of upper, middle, and lower trapezius was measured during isokinetic abduction and external rotation, using surface electrodes. The results show a significant increase of upper trapezius activity during both movements in the patient group, with decreased activity in the lower trapezius during abduction, and in the middle trapezius during external rotation. Analysis of the intramuscular activity ratios revealed muscle imbalance on the injured side of the patient group for upper/middle trapezius and upper/lower trapezius during abduction, and for all three muscle activity ratios during external rotation. These results confirm the presence of scapular muscle imbalances in patients with impingement symptoms and emphasize the relevance of restoration of scapular muscle balance in shoulder rehabilitation.

OBJECTIVES: To determine if the muscle force and electromyographic activity in the scapular rotators of overhead athletes with impingement symptoms showed differences between the injured and non-injured sides. METHODS: Isokinetic peak force was evaluated during protraction and retraction of the shoulder girdle, with simultaneous recording of electromyographic activity of the three trapezius muscles and the serratus anterior muscle, in 19 overhead athletes with impingement symptoms. RESULTS: Paired t tests showed significantly lower peak force during isokinetic protraction at high velocity (p<0.05), a significantly lower protraction/retraction ratio (p<0.01), and significantly lower electromyographic activity in the lower trapezius muscle during isokinetic retraction on the injured side than on the non-injured side (p<0.05). CONCLUSION: These results confirm that patients with impingement symptoms show abnormal muscle performance at the scapulothoracic joint.

OBJECTIVE: The first aim of this study was an approach to quantify the 3D kinematics of the glenohumeral joint referred to the joint surfaces. The method was used to study the glenohumeral patho-arthrokinematics related to minor anterior instability at the end of the late preparatory phase of throwing. STUDY DESIGN: Using a finite helical axis approach, arthrokinematics focused on: (i) the rotations and shift of the humeral head on the glenoid cavity, and (ii) the migration of contact of the articular surfaces. BACKGROUND: Controversy still exists whether the clinical syndrome called 'minor anterior glenohumeral instability' can be validly termed as an instability. METHODS: Helical CT-data of discrete shoulder positions were three-dimensionally reconstructed. Based on humeral and scapular sets of skeletal landmarks, rotation matrices and translation vectors were estimated and processed in glenohumeral finite helical axes. The finite helical axis parameters of rotation, shift and direction were related to a co-ordinate system embedded on the glenoid, whereas the position of the finite helical axis was related to the articulating surface of the humeral head. RESULTS: From 90 degrees abduction and 90 degrees external rotation to full cocking (90 degrees abduction with full external rotation and horizontal extension), the humeral head in the normal shoulders did not externally/internally rotate on the glenoid. In contrast, a large external rotation component was found in the minor unstable shoulders. The geometrical centre of the humeral head of the normal shoulders translated into a posteriorized position on the glenoid, whereas in minor anterior instability it translated centrally on the glenoid. CONCLUSIONS: Compared with in vitro biomechanical research which states that towards full cocking the anterior part of the inferior glenohumeral ligament limits anterior translation and external rotation of the humeral head on the glenoid, the results suggest in minor anterior instability a dysfunction of the anterior part of the inferior glenohumeral ligament. RELEVANCE: The results indicate that the so-called 'minor anterior glenohumeral instability syndrome' can validly be stated as an instability problem. The results also indicate that the glenohumeral joint does not move consistently as a ball-and-socket joint, meaning that the concave-convex rules for glenohumeral joint mobilization need 'evidence-based' adjustments.

UNLABELLED: This study examined the efficacy of trazodone 200 mg/day in patients with erectile dysfunction (ED) without major organic findings. PATIENTS AND METHODS: Thirty-four patients with ED without major organic findings entered a double-blind, placebo-controlled, randomized two-centre trial comparing placebo and trazodone 200 mg/day. After a 2 week treatment-free, baseline period eligible patients entered a 4 week treatment-phase. Nocturnal erectile activity was measured before and after treatment by Rigiscan Plus. Sexual desire, quality of partner relation and depressive symptomatology were assessed with the derogatis sexual functioning inventory (DSFI), the dyadic adjustment scale (DAS) and the Hamilton depression rating scale (HDRS), respectively. Sexual function during the study period was assessed with patient diaries. Compliance was measured by trazodone serum concentrations. There were no significant differences between trazodone and placebo in total erection duration, change in sexual desire, report of morning erections or psychometric scales, except for the HDRS. Trazodone serum levels indicated that half of the patients were not compliant. In conclusion, Trazodone 200 mg/day has no effect on sexual function in men with ED without major organic findings and non-compliance was an important issue in the present study.

RESUMO

Arthroscopic subacromial decompression is traditionally performed through a posterolateral viewing portal and a lateral working portal. We describe the same procedure by using a posterolateral viewing portal and a posteromedial working portal. Because this portal is in the same sagittal plane as the ipsilateral acromioclavicular joint, it allows performing an arthroscopic excision arthroplasty of this joint.

RESUMO

Arthroscopy of the shoulder has become much more common in the past decade as surgeons have developed proficiency with the arthroscope in the knee and appropriate instrumentation has been developed. In recent years arthroscopic techniques adapted to the shoulder have continued to evolve from a diagnostic to a treatment-oriented modality. It is now recognized and accepted as both a diagnostic and therapeutic technique in orthopedic surgery. A thorough knowledge of the anatomy, disorders, arthroscopic variations and pathological findings is essential to successfully perform the procedure. This paper discusses the operating room set-up, the portal placement and the indications for arthroscopy of the shoulder.

Arthroscopic transglenoid suture of Bankart lesions was performed in 31 patients from 1988 to 1992. The diagnosis in all patients was recurrent traumatic anterior luxation, and a Bankart lesion was found in all cases. Mean time for clinical follow-up was 43 months (ranging from 25 to 76 months). A telephone review of all cases was obtained two years later. Five patients experienced postoperative wound problems posteriorly, where the sutures were tied over the fascia of the infraspinatus. One transient suprascapular nerve palsy was seen. There was a recurrence of complete dislocation in eight patients, while six patients had had repeated subluxations (total failure rate of 45.1%). Sixteen patients (51.6%) were assessed as having good to excellent results according to the Rowe scoring system. A slight loss of external rotation was found in six cases. Seventeen patients (54.8%) were able to return to their pre-injury level of athletic activity. Due to the high failure rate, we do not recommend arthroscopic transglenoid suture of Bankart lesions in patients with recurrent traumatic anterior dislocations.

RESUMO

A 37-year-old sports teacher suffering from idiopathic haemochromatosis with arthropathy of the MP joints has been treated and followed-up by us for 4 years. Three out of four affected MP joints were treated with arthroscopic operations; one of these had been treated elsewhere previously by arthrotomy. This article presents a brief review of the condition and its treatment by arthroscopic surgery with detailed technique. We believe that MP joint arthroscopy in certain cases is an alternative to open surgery and gives excellent results. No specific instruments are needed apart from a standard small joint arthroscopy set.

An analysis of the follow-up results of 53 patients treated by an arthroscopic decompression (AD) and 53 patients treated by an open decompression (OD) is presented. Patients were evaluated pre- and postoperatively with the UCLA Shoulder Rating Scale, which includes an assessment of pain, function, range of motion, strength, and patient satisfaction. After an average of 20.1 months for the AD group and 27.3 months for the OD group, good or excellent results were found in 83.1% in the AD group and in 81.1% in the OD group. Patient satisfaction was 88.3% (AD) compared with 94.3% (OD). Results of decompression in both groups were not influenced by associated pathologies (acromioclavicular degeneration, frozen shoulder, calcified tendinitis, rotator cuff lesions).

An analysis of the results of 53 patients treated by an open Neer decompression for chronic impingement of the shoulder is presented. Patients were evaluated pre- and post-operatively on the UCLA Shoulder Rating Scale, which includes an assessment of pain, function, range of motion, strength and patient satisfaction. After an average period of 27.3 months, good or excellent results were found in 81.1%. Satisfactory results (according to Neer) were found in 94.3%. Differential diagnosis of acromioclavicular pathology and cervical syndromes is important.

Congenital contracture of the infraspinatus muscle has been reported by K. Kitano et al. (1988). We describe a patient with an acquired contracture of the infraspinatus muscle causing posterior subluxation of the glenohumeral joint.

A derivative of luciferin, 4-methyl-D-luciferin, was synthesized in low yield, and characterized by its ultraviolet and fluorescence spectra. The spectra of luciferin and its derivative are very similar. 4-Methyl-D-luciferin is a substrate of Photinus pyralis luciferase, and it causes light emission in luminometric assays.

A multicenter study including 10 outpatient private laboratories (hospital laboratories excluded) was carried out in France. 1,611 urines samples from patients with UTI were collected during the forth trimester of 1987. The most frequently recovered pathogens were: E. coli (71%), Proteus mirabilis (9%), Staphylococcus coagulase (6%), Klebsiella (6%), Enterobacter (2%). Other sorts (Streptococcus D, Proteus sp, Pseudomonas aeruginosa, Enterobacter sp) were infrequent (less than 1%). The sensitivity of the aerobic Gram-negative bacteria to ampicillin, clavulanic acid-amoxicillin, cephalothin, gentamicin, pipemidic acid, norfloxacin and co-trimoxazole was tested.
