RESUMO

Vitamin D deficiency (VDD) is a global problem, however, there were no Ukrainian guidelines devoted to its screening, prevention, and treatment, which became the reason for the Consensus creation. This article aimed to present the Consensus of Ukrainian experts devoted to VDD management. Following the creation of the multidisciplinary Consensus group, consent on the formation process, drafting and fine-tuning of key recommendations, and two rounds of voting, 14 final recommendations were successfully voted upon. Despite a recent decrease in VDD prevalence in Ukraine, we recommend raising awareness regarding VDD's importance and improving the strategies for its decline. We recommend screening the serum 25-hydroxyvitamin D (25(OH)D) level in risk groups while maintaining a target concentration of 75-125 nmol/L (30-50 ng/mL). We recommend prophylactic cholecalciferol supplementation (800-2000 IU/d for youthful healthy subjects, and 3000-5000 IU/d for subjects from the risk groups). For a VDD treatment, we recommend a short-term administration of increased doses of cholecalciferol (4000-10,000 IU/d) with 25(OH)D levels monitored after 4-12 weeks of treatment, followed by the use of maintenance doses. Additionally, we recommend assessing serum 25(OH)D levels before antiosteoporotic treatment and providing vitamin D and calcium supplementation throughout the full course of the antiosteoporotic therapy.

OBJECTIVE: The aim: To study the osseointegrative properties of titanium and tantalum implants with different surface structures in animal experiments. PATIENTS AND METHODS: Materials and methods: The histological and morphometric study was carried out on 60 male white rats after titanium implants with different surface structures made by 3D printed technology were inserted in the distal femur bone: presented by the multilayered layers of interlacing pores of 300 microns (series 1); rough (> 2 microns) (series 2); and tantalum implants with 300 microns pores and 80% porosity (series 3) as control material. RESULTS: Results: On the 30 days we found statistically significant differences in the bone-implant contact rate between the 2nd experiment series (44.77 ± 1.86)% and 1st (59.91 ± 2.86)% (p=0.000047) and 3rd (53.89 ± 2.11)% (р=0.000065), on the 90 days between the 2nd experiment series (51.26 ± 2.7)% and 1st (66.84 ± 2.63)% (p=0.000187) and 3rd (70.35 ± 4.32)% (p=0.000349). There was a difference between the indices of the bone-implant volume at day 90 between the 1st (48.43 ± 2.2)% and 2nd (36.88 ± 2.56)% series (p=0.000919), between the 2nd and 3rd series (51.2 ± 3.06)% (p=0.000107). There were no significant differences between the studied indices in the 1st and 3rd series of the experiment. CONCLUSION: Conclusions: Titanium implants with multilayered interlaced pore layers of 300 microns and tantalum with 300 microns pore size and 80% porosity may be promising. Rough-surface titanium also has osseointegrative qualities, but they are lower compared to other materials.

OBJECTIVE: The aim: Study of the dynamics of morphological rearrangement of bone under conditions of immediate occlusive functional load and the effect of splinting of implants with temporary orthopedic structures with the analysis of the coefficient of stability of implants during immediate implantation in the experiment. PATIENTS AND METHODS: Materials and methods: A series of experiments was performed on 6 male Duroc pigs at the age of 6 months and weighing 40-60 kg. In the course of recent advances, the following methods have been used: the clinical protocol of immediate - implantation of time-consuming clothes, the definition of COEFICIENT, morphometry and light microscopy of the slides, statistical analysis. RESULTS: Results: By morphometric examination after 3 months the BIC in the series with splinting was 1.68 times higher compared to 1 month. studies, in a series of experiments without splinting - 1.9 times, after 3 months. the difference between implantation experiments with splinting components and without splinting is 1.6 times. During the functional study of the resonant - frequency analyzer, there is an increase in the ISQ in the second and third months after surgery, but this figure is higher in the study using the splint component. CONCLUSION: Conclusions: Stagnation of the shingle component in the case of intrinsic intraoperative functional juvenile implantation accelerates the dynamics of osteointegration, so that high indicators of the efficiency of the implant stability can be achieved.

BACKGROUND: There is a lack of comparative data on osseointegration (BIC) of acetabular cup surfaces in hip arthroplasty in both normal bone mineral density and in conditions of osteoporosis. AIM: To compare osseointegration of acetabular implants with various types of surfaces in an animal model with normal and osteoporotic bone tissue. MATERIAL AND METHODS: The study was performed on 60 rats. To simulate osteoporosis ovariectomy was performed in 30 animals. Thirty healthy rats served as controls. In standardised defects of the distal metadiaphysis of the femur we implanted: porous tantalum Trabecular Metal (A), Trabecular Titanium (B), Titanium with Gription coating (C), Stiktite (D), and Tritanum (E). Bone apposition (osseointegration) was defined as all areas of direct "bone-to-implant contact" (BIC). RESULTS: No qualitative morphological differences in the evaluation of BIC around different implant types was seen in normal rats and rats with osteoporosis. Connective tissue areas around implants were larger in rats with osteoporosis. Morphometric studies showed that the highest BIC were seen in implants A and B, both in healthy animals ([72.00 ± 3.48]% and [67.46 ± 1.69]%) and ones with osteoporosis ([59.19 ± 2.10]% and [53.36 ± 2.57]%). BIC was (60.10 ± 2.05)%, (60.26 ± 2.36)%, and (61.78 ± 2.27)% around implants C, D and E in healthy rats, respectively. BIC in osteoporosis was (45.39 ± 2.37)%, (47.81 ± 2.41)% and (42.10 ± 1.44)%, respectively. CONCLUSION: Our study showed that the evaluated implants have good BIC features. Furthermore, based on histomorthometry and histology, Porous tantalum Trabecular Metal (A) and Trabecular Titanium (B) implants exhibit higher BIC with bone tissue.
