In poultry production, there is a lack of effective and convenient approaches to deliver bioactive compounds such as some essential oils, which have been proposed as alternatives to antibiotic growth promoters. The objective of this research was to develop a method for target delivery of essential oils in feed to the lower intestines of chickens. Carvacrol was used as a model essential oil, and 2 food-grade biopolymers, alginate and whey protein, were selected to encapsulate carvacrol in microparticles. The effects of a medium molecular weight alginate, a low molecular weight alginate (LBA), and whey protein concentrations on the properties of carvacrol-loaded microparticles were investigated using response surface methodology. The encapsulation efficiencies for all the tested formulations were ≥ 98% and carvacrol content in the dry microparticles was 72 ± 2% (wt/wt). The microparticles showed good gastric resistance and rapid intestinal release under simulated gastrointestinal conditions. Alginate concentrations had the strongest influence on the gastric resistance of microparticles, whereas whey protein was the dominant parameter in controlling the intestinal release. The concentration of LBA was found to be the critical factor affecting the mechanical strength of the microparticles. A predicted optimum formulation from in vitro optimization was tested in chickens. It was found that a negligible amount of carvacrol was detected in the intestines of chickens fed with unencapsulated carvacrol. Microparticles of predicted optimum formulation delivered a remarkably higher concentration of carvacrol to the jejunum and ileum regions. The high concentration was sustained for more than 3 h after oral administration. The in vivo release of carvacrol from the microparticles appeared faster than release from in vitro simulation. Nonetheless, the in vitro simulation provided good indications of the in vivo performance, and thus may serve as a useful tool for formula optimization. In conclusion, the current study indicates that alginate-whey protein microparticles could be used as a target delivery carrier in the feed to enhance the intestinal delivery of essential oils in poultry production.

OBJECTIVE: To investigate and diagnose the timing of asphyxial injury leading to cerebral cavitation with subsequent developing of neonatal porencephaly in the preterm VLBW infant. All newborns underwent careful neurodevelopmental outcome at 2 years of corrected age. METHODS: 250 consecutive VLBW infants (mean gestational age of 28 weeks and mean birthweight of 1150 g) have been study by means of weekly neonatal transfontanellae ultrasonography. Periventricular white matter necrosis was diagnosed when echolucencies were visible after day 3 from birth. RESULTS: Twelve cases of neonatal porencephaly were diagnosed by ultrasound. The timing of asphyxial insult leading to cerebral cavitation seems to have occurred in 33% of neonates during the antepartum period, in 42% during the peripartum period (antepartum + neonatal period) and 25% in the remaining neonatal period. Periventricular-intraventricular hemorrhage (PVH-IVH) was found in all cases and in 50% a severe IVH (grade III-IV) was diagnosed within 7 days neonatal period. Nine infants had evidence of cerebral palsy at 2 years neurological outcome. CONCLUSIONS: The ultrasound criteria of cerebral cavitation have been priorly selected in order to assure that the damage may have occurred before delivery. A comprehensive prenatal study of fetal brain, integrating ultrasound with high-velocity MRI, is also advocate. This will lead to a more detailed understanding of the underlying cerebral condition that is of critical importance for the clinician in planning the time and mode of delivery and have great deal with further medico-legal consideration.

Because of their anatomic location, cardiac sarcomas often interfere with cardiac function. Excision is considered to palliate the cardiac defect, but complete excision is often difficult owing to access, particularly in left atrial tumors. Incomplete resection results in tumor recurrence. To achieve complete resection of a large left atrial sarcoma, we used the technique of cardiac explantation, extracorporeal resection of the tumor with cardiac reconstruction, and cardiac autotransplantation.

RESUMO

Balloon mitral valvuloplasty (BMV) for mitral stenosis is a procedure that has evolved significantly since its introduction by Inoue et al. in 1984. This article reviews currently used techniques, advantages, and limitations as well as outcomes in comparison with surgical procedures. Included is a review of imaging techniques that facilitate BMV, such as transesophageal echocardiography and the recently developed tri-dimensional transthoracic echocardiography and intracardiac echocardiography. In a separate section, the application of BMV in specific clinical situations, such as in patients with multivalvular disease, during pregnancy, in children, in the presence of thrombi, and in patients with bioprostheses, is discussed.

PURPOSE: We examined the effects of intracoronary irradiation delivered at a high dose rate on neointimal hyperplasia after injury induced by two methods: balloon overstretch injury, and stent implantation in a porcine model of coronary restenosis. METHODS AND MATERIALS: In 34 Hanford miniature swine, a segment of each coronary artery was targeted for injury and treatment. The artery segments were treated with 192Ir at doses of 10 Gy over 4 min (eight animals), 15 Gy over 6 min (nine animals), 25 Gy over 10 min (nine animals) or control (simulation wire only; eight animals). The treated segments were subjected to stent implantation (left anterior descending and right coronary artery) or balloon overstretch (circumflex) injury. Twenty-eight days later, repeat coronary angiography and sacrifice were done. Quantitative coronary angiography, morphometry, and extensive histopathologic analyses were carried out in a blinded fashion. RESULTS: The change in minimal lumen diameter from postinjury to presacrifice in the stent-injured left anterior descending was -0.79 +/- 0.34 (mean: +/- SD) mm in the control group, compared to -0.43 +/- 0.35 mm in the 15 Gy (p = 0.04) and -0.21 +/- 0.50 mm in the 25 Gy (p = 0.01) groups; and in the balloon-injured circumflex was -0.31 +/- 0.22 mm in the control group compared to -0.03 +/- 0.18 mm in the 10 Gy (p = 0.05) and 0.00 +/- 0.33 in the 15 Gy (p = 0.01) groups. Percent area stenosis in the left anterior descending was 36 +/- 9% in the control group compared to 18 +/- 12% in the 15 Gy (p = 0.003) and 11 +/- 11% in the 25 Gy (p < 0.001) groups; and in the circumflex was 16 +/- 10% in the control groups, compared to 5 +/- 5% in the 15 Gy (p = 0.02) and 2 +/- 2% in the 25 Gy (p = 0.009) groups. Histopathology showed a striking reduction in the amount of neointima in the irradiated arteries compared with control vessels. Other radiation effects were stromal fibrin exudate, thinning of the media, and adventitial fibrosis and leukocyte infiltration in the radiated arterial segments. CONCLUSIONS: High dose rate intracoronary irradiation with 192Ir effectively inhibits intimal proliferation after stent-induced as well as balloon-overstretch injury. This shorter treatment time (4 to 10 min) may provide a clinically practical approach to the prevention of restenosis after angioplasty.

A 37-year-old man with systemic lupus erythematosus, who underwent an aortic valve replacement with a Carpentier-Edwards porcine valve for severe aortic insufficiency, was admitted to the hospital with pulmonary edema. Transesophageal echocardiography revealed severe aortic insufficiency arising from partial dehiscence of the valve sewing ring, as well as centrally from the valve cusp. In addition, marked thickening of the mitral valve was observed with severe eccentric regurgitation. At surgery, valvulitis of the native mitral and bioprosthetic aortic valves was demonstrated, with a perforation of the porcine valve cusp. After replacement of both valves, the patient had a stormy postoperative course with recurrent communications between the left ventricle and atrium requiring multiple surgeries and eventually died. This case illustrates the severity of valvulopathy and ensuing complications that can affect patients with systemic lupus erythematosus and demonstrates that the valvulopathy can affect bioprosthetic valves, a finding that has significant implications as to the type of valve replacement in these patients.

RESUMO

A 53-yr-old active man with angina but no angiographically significant obstructive coronary artery disease developed refractory spasm following ergonovine provocation. This resulted in thrombus formation and myocardial infarction. As such, this is the first case in which thrombosis developed at the site of intense coronary artery spasm induced by ergonovine, all of which were documented angiographically. Despite the notable safety of this test, this case re-emphasizes the potential of ergonovine to produce intractable myocardial ischemia and infarction.

RESUMO

Papillary fibroelastoma is a rare cardiac tumor arising most frequently from valvular endocardium. These tumors are clinically important because of their tendency to embolize. Emphasis is therefore placed on early detection and surgical excision. We describe a case of a papillary fibroelastoma arising from the right atrium. To our knowledge, only 2 such cases have been reported in the English literature. Our case illustrates the discovery of a rare, but potentially fatal cardiac tumor, successfully removed by surgery.

This study describes the short- and long-term outcome of 44 consecutive percutaneous balloon aortic valvuloplasty procedures performed in 42 elderly patients (age 77.8 +/- 7 years) with calcific aortic stenosis. The initial success rate was 95%, with the peak aortic valve pressure gradient declining from a mean of 82 +/- 32 mm Hg to 44 +/- 23 mm Hg and aortic valve area increasing from a mean of 0.59 +/- 0.15 cm2 to 0.83 +/- 0.40 cm2. One procedure-related death occurred and an additional three patients died less than or equal to 30 days after balloon aortic valvuloplasty. These patients all had New York Heart Association (NYHA) class IV heart failure symptoms prior to the procedure and their mean left ventricular ejection fraction (LVEF) (28 +/- 7%) was lower than that of hospital survivors (52 +/- 13%) (as was their ratio of left ventricular [LV] wall thickness-to-cavity ratio [0.50 +/- 0.10 versus 0.70 +/- 0.15]). At the time of hospital discharge after valvuloplasty, 76% of patients were asymptomatic or markedly improved (NYHA class I or II). After a mean follow-up of 15.5 months (range 2 to 26 months), however, 10 patients had died and 15 had undergone aortic valve replacement for recurrence of NYHA class III or IV symptoms. The adjusted 1- and 2-year survivals were 0.68 and 0.62, respectively, and adjusted 2-year event-free survival was 0.25. Proportional hazard regression analysis indicated that LVEF less than 40% was the only variable affecting survival (p less than 0.01) and was a possible indicator of event-free survival (p = 0.07).(ABSTRACT TRUNCATED AT 250 WORDS)

Two instruments were developed to mechanize the handling of anaerobic microorganisms for microbial mutant isolation. The instruments automatically dispense liquid or agar medium to large and small 96-well platesand petridishes. Protocols were developed for inoculating different microorganisms, and calibration curves of number of areas or wells inoculated versus cell concentration were prepared for bacteria, yeast and fungi (spores). Experiments with yeast auxotrophic mutants and fungal spores showed that microbe inoculation follows Poisson statistics in distributing a single microorganism per inoculation point. The isolation and identification of Yarrowia lipolytica auxotrophic, morphological, and temperature-sensitive or tolerant mutants demonstrated the use of the instruments for microbial screening.

Mitral valve area (MVA) determined at cardiac catheterization was compared with M mode echocardiographic measurements in 44 patients with mitral stenosis and no substantial mitral regurgitation. Despite statistically significant correlations, measurements of anterior leaflet motion, including rate of diastolic closure (EF slope) were not useful in predicting severity of stenosis. In contrast, maximal diastolic separation of anterior and posterior leaflets (SEP) was more closely correlated with MVA and appears to have some predictive value. Narrow separation was associated with severe mitral stenosis. Wide separation was associated with relatively mild stenosis. Intermediate values in 16 of 44 patients (36%) were not of predictive value. Recognizing this limitation, measurement of maximal diastolic mitral leaflet separation from M mode echocardiograms is proposed as a simple and useful method for assessing severity of mitral stenosis.

Three months after receiving a Bjork--Shiley mitral valve prosthesis, a patient experienced acute gastronintestinal bleeding associated with heavy alcohol intake and poor compliance with oral anticoagulation therapy. As a result of the gastrointestinal bleeding, treatment with warfarin was discontinued. Ten months later, the patient presented with symptoms of transient cerebral ischemia. Despite demonstrated loss by phonocardiography of the previously well-recorded opening click and echocardiographic suggestion of a thrombosed prosthetic mitral valve, no thrombosis or dysfunction of the prosthetic mitral valve was revealed at reoperation.

In 44 consecutive patients with angina at rest associated with transient S-T segment elevation, clinical features were correlated with angiographic coronary anatomy. Patients were divided into three groups depending on the number of major vessels having greater than or equal to 70 per cent luminal narrowing: Group I = no or minimal disease (six patients); group II = single vessel disease (13 patients); and group III = multiple vessel disease (25 patients). The following features did not differ significantly among groups I, II or III: age, sex, risk factors, time from onset of episodes of pain at rest to study or arrhythmias during ischemic episodes. Patients in group III were more likely to have angina on effort (p less than 0.001) and an abnormal base line electrocardiogram (p less than 0.001) than patients in groups I or II. However, the absence of these features did not separate patients in group I from those in group II. In patients with angina at rest associated with transient S-T segment elevation, clinical features identify patients with multiple vessel disease but do not allow differentiation of patients with no or minimal coronary disease from patients with single vessel disease.
