In four patients with alkaline-encrusted cystitis, Corynebacterium group D2 was isolated from consecutive urine cultures and stones. Encrusted cystitis occurred in bladders harboring inflammatory or tumorous lesions in patients with chronic or recurrent urinary tract infections appearing after surgery or instrumentation. The urease activity of Corynebacterium group D2 and the neutralization of this enzyme by acetohydroxamic acid are shown. Clinical improvement, disappearance of struvite crystals, and decrease of the urine pH were obtained when these bacteria were eliminated from urine samples. Corynebacterium group D2 strains were highly resistant to many antimicrobial agents but were highly susceptible to norfloxacin and vancomycin when tested at two pHs (7.4 and 8.5).

In 3,000 calculi, monosodium urate was detected in 50 cases (1.66%). Infrared frequencies of monosodium urate are reported. In thin sections it appeared as long and thin fan-shaped or feather-like arranged crystals. Six calculi had monosodium urate as the sole component but all other cases were of mixed composition, with whewellite as the main secondary substance. The relative scarcity of monosodium urate cannot be explained by its high aqueous solubility because in urine the solubility decreases with increasing sodium concentration. Monosodium urate should be suspected in radiolucent calculi with neutral or light alkaline reaction and high uric acid and sodium concentrations in the patient's urine.

In the group of 1635 patients with nephorolithiasis the authors found 16% with idiopathic hypercalciuria. They administered thiazide diuretics for 8 days as a test for the detection of masked hyperparathyroidism in idiopathic hypercalciuria. In 6 cases which were diagnosed in this manner, the existence of a parathyroidadenoma could be demonstrated by surgical operation. The hypercalciuria persisted after exstirpation of the tumor, in some cases the nephrolithiasis also remained active. the various pathogenetic mechanisms connected with these common diseases are discussed in relation to these results.
