RESUMO

Results of surgical treatment of 28 adolescent girls with vaginal aplasia and functioning uterus are analyzed. The uterus had to be removed in all the patients 1.5 to 6 months after abdominal colpopoiesis with preservation of functioning uterus; after sigmoidal colpopoiesis the uterus was preserved in 85% of patients. Analysis of the contents of the uterine cavity showed endometritis, which was evidently caused by the absence of the cervix uteri, a natural barrier preventing the penetration of intestinal flora of neovaginal mucosa into the uterine cavity. The studies permitted a new interpretation of the results of abdominal and sigmoidal colpopoiesis with preservation of functioning uterus in adolescent girls with vaginal aplasia.

RESUMO

Ultrasonic device Toshiba 140 A/G was used to examine 118 women at pregnancy terms 10 to 30 weeks. Umbilical artery, intraplacental arteries, and retroplacental blood flow were assessed. All the women were divided into two groups by the results of retrospective analysis of dopplerograms. Group 1 were 47 women whose intraplacental blood flow values were higher than umbilical artery blood stream parameters. Group 2 were 71 women with the umbilical artery pulsation index higher than in intraplacental vessels. In group 2 blood clotting disorders were more incident (p < 0.01) presenting as platelet hyperaggregation and/or hypercoagulation and sometimes as increased plasma concentrations of fibrin and fibrinogen degradation products. Study of intraplacental and retroplacental blood flow helps differentiate between preclinical and compensated stages of placental insufficiency and thus timely administer the needed drug therapy with due consideration for the time of discirculatory abnormalities, predominantly uteroplacental or fetoplacental.

RESUMO

The results of surgical treatment of 102 patients with destructive tuberculosis of the lungs and liver pathology are presented. It was shown possible to widen the operability ranges in the patients by using the up-to-date treatment including hemosorption and laser technique. After hemosorption there was a significant improvement of liver function. Data on efficiency of the surgical treatment (90.2 per cent) in the group of the patients with pulmonary tuberculosis at high surgical risk are indicated.

RESUMO

The article analyses experience in the use of beams of various types of lasers in treatment of 152 patients with different forms of pyothorax of tuberculous and nontuberculous origin. Semiconductor and carbon dioxide laser devices were used in the stages of surgical treatment. Methods for the use of lasers in the pre- and postoperative periods and during the operation are described. With the use of laser beams in the stages of surgical treatment the traumatic character of the intervention is lessened, blood loss during the operation reduced, the walls of the empyema are sterilized effectively, the incidence of postoperative complications diminishes, and the clinical efficacy of surgical management of this severe contingent of patients is improved.

RESUMO

On the basis of the analysis of the results of surgical treatment of 114 patients, the authors show that in liver pathology, continued aggravation of destructive tuberculosis occurs. To reduce the risk of operation, it is suggested to perform intensive preoperative preparation with inclusion of hemosorption and laser irradiation.
