RESUMO

In a multi-institution study, 18 knees with complete quadriceps tendon ruptures (QTRs) were reviewed to analyze the cause of failure in radiographic diagnosis. Only 6 of the 18 knees were diagnosed correctly initially. This occurred even though almost all knee radiographs showed more than three abnormalities compatible with QTR (obliteration of quadriceps tendon, suprapatellar mass, suprapatellar calcific density, or patella baja). Of the 12 diagnostic failures, only 3 were considered to be complete failure. Nine primary reports (50%) were classified as incomplete diagnosis and could be correctly diagnosed if the attention of radiologists was focused to the quadriceps mechanism. We conclude that better understanding of radiographic abnormalities in QTR is necessary for the improvement of primary radiological diagnosis.

OBJECTIVE: To clarify the efficacy of MRI for the evaluation of rotator cuff impingement beneath the coracoacromial arch, MRI findings were correlated with confirmed full-thickness rotator cuff tears (FTRCTs). MATERIALS AND METHODS: Thirty-four shoulders with FTRCT and 49 control shoulders without FTRCT were selected for this study. Direct measurement of the subacromial space (SAS) and MRI grading of SAS and subacromioclavicular space (SACS) were correlated with FTRCT. In addition, the MRI impingement score (sum of SAS grade and SACS grade) was correlated with age and FTRCT. RESULTS: Direct measurement of the SAS showed poor correlation with FTRCT. The SAS grade showed significant correlation with FTRCT; however, SACS grade showed poor correlation. Shoulders with higher MRI impingement scores were associated with FTRCT in patients over 40 years of age. CONCLUSION: The MRI sign of rotator cuff impingement is fairly reliable and helpful in discriminating patients with FTRCT, especially older patients.

To identify any correlation between the distribution of occult bone lesions and meniscoligamentous injuries, magnetic resonance images of 333 patients with traumatic knee joint disease were reviewed. Bone lesions of the knee were commonly associated with medial meniscal injuries and/or anterior cruciate ligament injuries. While knees with bone lesions showed a higher incidence (P < 0.05) of anterior cruciate ligament injury than knees without bone lesions, the presence of a lateral femoral condylar lesion resulted in a significantly higher incidence of anterior cruciate ligament injuries (P < 0.01). However, no significant positive correlation was found between other occult bone lesions and meniscoligamentous injuries.

RESUMO

Giant cell tumor of the tendon sheath with characteristic magnetic resonance imaging findings is reported. The tumor was low intensity on T1-weighted and T2-weighted images, and showed strong homogeneous enhancement. These findings were considered to represent the pathological features of fine hemosiderin granules and collagenization.
