INTRODUCTION: We present two cases of vitreous hemorrhage after micropulse cyclophotocoagulation one of which had concurrent hyphema. To the best of our knowledge, these are the first cases of vitreous hemorrhage due to micropulse CPC in the United States. CASE DESCRIPTION: The first case is an 82-year-old woman with bilateral severe primary open angle glaucoma. BCVA in the right eye was 20/25, and 10-2 Humphrey visual field showed severe peripheral defects. The patient underwent MPCPC of the right eye and at one week, a settled 2 mm hyphema and vitreous hemorrhage confirmed by B-scan were noted. At three months, the patient had a BCVA of 20/80 with an IOP of 12 and retina consultation deferred a PPV. The second case is of a patient with bilateral moderate stage POAG who underwent MPCPC in both eyes. His original VA was 20/200 bilaterally. At 2 weeks, RE VA was count fingers at one foot and LE was 20/150-1. At two months, a RE B scan revealed dense vitreous opacities. Retina consultation revealed vitreous hemorrhage but a PPV was deferred. CONCLUSION: Clinicians should be aware of the risks of bleeding and the potential need for additional surgical interventions after MPCPC.

An 82-year-old man with severe primary open-angle glaucoma on maximal medical therapy underwent an ab externo closed conjunctival Xen45 device insertion with mitomycin C. The surgery was uncomplicated, with a first postoperative day intraocular pressure of 4 mm Hg and visual acuity of 20/40 OD. Ten days later, the patient presented with an intraocular pressure of 5 mm Hg and a visual acuity of counting fingers at 5 feet. Examination showed Seidel negative bleb, shallow anterior chamber, and large nonappositional choroidal detachments. Medical therapy with steroids and cycloplegia was initiated. One week later, the serous choroidal detachments became appositional, and Xen explantation and surgical drainage of the choroidal detachment was performed. Postoperatively, the vision improved to 20/60. Significant choroidal detachments can occur after XEN45 implantation requiring surgical intervention.

IMPORTANCE: Children with a history of amblyopia, even if resolved, exhibit impaired visual-auditory integration and perceive speech differently. OBJECTIVE: To determine whether a history of amblyopia is associated with abnormal visual-auditory speech integration. DESIGN, SETTING, AND PARTICIPANTS: Retrospective observational study at an academic pediatric ophthalmologic clinic with an average of 4 years of follow-up. Participants were at least 3 years of age and without any history of neurologic or hearing disorders. Of 39 children originally in our study, 6 refused to participate. The remaining 33 participants completed the study. Twenty-four participants (mean [SD] age, 7.0 [1.5] years) had a history of amblyopia in 1 eye, with a visual acuity of at least 20/20 in the nonamblyopic eye. Nine controls (mean [SD] age, 8.0 [3.4] years) were recruited from referrals for visually insignificant etiologies or through preschool-screening eye examinations; all had 20/20 in both eyes. EXPOSURES: Participants were presented with a video demonstrating the McGurk effect (ie, a stimulus presenting an audio track playing the sound /pa/ and a separate video track of a person articulating /ka/). Normal visual-auditory integration produces the perception of hearing a fusion sound /ta/. Participants were asked to report which sound was perceived, /ka/, /pa/, or /ta/. MAIN OUTCOME AND MEASURE: Prevalence of perception of the fusion /ta/ sound. Prior to the study, amblyopic children were hypothesized to less frequently perceive /ta/. RESULTS: The McGurk effect was perceived by 11 of the 24 participants with amblyopia (45.8%) and all 9 controls (100%) (adjusted odds ratio, 22.3 [95% CI, 1.2-426.0]; P = .005). The McGurk effect was perceived by 100% of participants with amblyopia that was resolved by 5 years of age and by 100% of participants whose onset at amblyopia developed at or after 5 years of age. However, only 18.8% of participants with amblyopia that was unresolved by 5 years of age (n = 16) perceived the McGurk effect (adjusted odds ratio, 27.0 [95% CI, 1.1-654.0]; P = .02). CONCLUSIONS AND RELEVANCE: This pilot study suggests that children with a history of amblyopia have impaired visual-auditory speech perception. Early childhood appears to serve as an approximate time point for the development of successful visual-auditory fusion, by which time amblyopia must have either resolved or begun. Interventions to resolve amblyopia may not only influence visual acuity but may also influence the perception of sound.

BACKGROUND: To determine the safety and efficacy of intravitreal ranibizumab therapy before and after Ahmed tube insertion for open-angle glaucoma as a means of optimizing postoperative intraocular pressure control. DESIGN: Randomized, controlled trial. PARTICIPANTS: Open-angle glaucoma patients scheduled for Ahmed tube insertion, randomized to ranibizumab or control groups. METHODS: Ranibizumab (0.5 mg in 0.05 mL) was administered intravitreally at three time points: 9 days prior to surgery, 1 month post-surgery and 2 months post-surgery. Control patients underwent the same procedure without ranibizumab. MAIN OUTCOME MEASURE: Success at 6 months postoperatively was defined as intraocular pressure <18 mmHg with no adjunctive medications or intraocular pressure <15 mmHg with one adjunctive medication. RESULTS: The study and control arms included six and five subjects, respectively, with four in each arm undergoing combined cataract surgery. In the ranibizumab arm, the preoperative and postoperative intraocular pressure/medication usage was 21.0 ± 6.7 mmHg on 3.2 ± 1.5 medications and 14.7 ± 1.9 mmHg on 0.5 ± 0.8 medications, respectively. In the control arm, preoperative and postoperative intraocular pressure/medication usage was 18.8 ± 3.8 mmHg on 2.8 ± 1.3 medications and 16.2 ± 3.6 mmHg with 1.8 ± 1.6 medications, respectively. Success was achieved in 83% of subjects in the ranibizumab group compared with 40% in the control group (two-tailed Fisher's exact test, P = 0.24). CONCLUSION: The findings from this small pilot comparative study suggest that intravitreal ranibizumab use may be a safe and potentially effective adjunctive treatment modality in improving success after Ahmed tube placement.

The authors describe a girl diagnosed as having Coats' disease, Turner syndrome (45X karyotype), and type 1 von Willebrand disease. She tested negative for the Norrie disease pseudoglioma (NDP) gene located on the X-chromosome, which has been suspected of contributing to Coats' disease.

PURPOSE: To determine the therapeutic concentrations of moxifloxacin achieved in an artificial anterior chamber by soaking the hydrophobic acrylic AcrySof™ SA60 (Alcon Inc.) intraocular lens (IOL) and the hydrophilic collamer Afinity™ CQ2015 (Staar Inc.) IOL in commercially available moxifloxacin 0.5% (Vigamox™; Alcon Inc.). METHODS: Forty IOLs (20 Acrysof SA60 and 20 Afinity CQ2015) were soaked in 1 mL of commercially available moxifloxacin 0.5%: 10 of each IOL for 1 min, and another 10 of each IOL for 10 min. The IOLs were placed on absorbent pads for 10 s on each side to dry excess liquid, and then placed in vials of 10 mL balanced salt solution (BSS™) for 30 min. Five milliliters of the balanced salt solution was removed and analyzed by high-pressure liquid chromatography to determine antibiotic levels. RESULTS: The moxifloxacin levels achieved after soaking the hydrophobic SA60 lens were 0.238 and 0.342 µg/mL for 1 and 10-min soaks, respectively. The moxifloxacin levels achieved after soaking the hydrophilic CQ2015 lens were 0.283 and 0.717 µg/mL for 1 and 10-min soaks, respectively. CONCLUSIONS: Both lenses were capable of delivering clinically significant antibiotic levels after a 1-min soak. Moxifloxacin concentrations reached at both 1 and 10-min soak times exceed the MIC(90) of the most common pathogens responsible for postoperative endophthalmitis. The antibiotic-soaked IOL has potential to become a clinically significant technique in the prevention of postoperative endophthalmitis.

RESUMO

The authors present a single-patient interventional case report of endogenous fungal endophthalmitis (EFE) in a patient with an implanted prosthetic device. A 74-year-old man underwent a double coronary artery bypass graft with a bioprosthetic pericardial valve and transvenous dual chamber pacemaker. Four months later, he presented with EFE. Despite aggressive local and systemic antifungal therapy, the patient died of septic shock due to a fungally infected pacemaker. Successful management of post-cardiac surgery EFE requires aggressive local and systemic antifungal therapy, but without surgical removal of implanted thoracic hardware these modalities alone may be insufficient for a cure.

Free-floating idiopathic vitreous cysts are rare entities. For symptomatic patients, treatment options include laser photocystotomy or pars plana vitrectomy (PPV) with cyst excision. Historically, size of the cyst impacted treatment approaches. Laser treatments have been used for smaller cysts ranging 3 to 5 mm in diameter. PPV has been utilized for cysts larger than 4 mm. Successful laser photocystotomy has been reported, utilizing both argon and Nd: YAG lasers. Herein, we present a case of successful argon laser photocystotomy of a 7.1 mm diameter vitreous cyst, the largest documented cyst ever treated. Treatment should be individualized based on patient age, ability to cooperate in an office setting, phakic status, status of the posterior hyaloid, cyst morphology, and location. In our case, because of pigment on the anterior cyst surface, and the presence of an attached posterior hyaloid, the argon green laser was used.

BACKGROUND AND OBJECTIVE: To evaluate the safety and efficacy of residents performing glaucoma valve surgery at a metropolitan county hospital. PATIENTS AND METHODS: A retrospective analysis was performed of consecutive resident-performed Ahmed valve (New World Medical, Inc., Rancho Cucamonga, CA) glaucoma surgeries from 1993 to 2005. Data of 50 eyes of 48 patients were evaluated for intraocular pressure (IOP) and best-corrected visual acuity. RESULTS: The mean follow-up period was 34.7 +/- 30.5 months, with an IOP decrease from 30.8 +/- 11.3 mm Hg preoperatively to 15.3 +/- 11.6 mm Hg at last visit. Success, defined by a postoperative IOP of greater than 5 mm Hg and at most 21 mm Hg or less or a decreased postoperative IOP of at least 25% if the preoperative IOP was already 21 mm Hg or less, occurred in 78% of eyes. Best-corrected visual acuity decreased in 58% of eyes. Four eyes (8%) required subsequent penetrating glaucoma procedures. CONCLUSION: This retrospective study suggests that placement of an Ahmed valve can be a safe and effective procedure in the hands of residents under appropriate attending physician supervision.

Neural larva migrans (NLM) with eosinophilic meningoencephalitis secondary to raccoon roundworm (Baylisascaris procyonis) infection has been reported in rural and suburban areas of North America and Europe with extant raccoon populations. Most cases have occurred in infants less than two years of age exposed to areas of raccoon fecal contamination. Here, we present a case of Baylisascaris-induced NLM from the densely populated borough of Brooklyn in New York City and alert urban pediatricians to consider this cause of clinical neurologic disease even in areas not typically thought to be associated with endemic risk factors. Infected raccoons also occur in urban settings, and urban children may be exposed to environmental areas or materials contaminated with their feces and the parasite's eggs.

Diffuse unilateral subacute neuroretinitis (DUSN) secondary to raccoon roundworm (Baylisascaris procyonis) infection has been reported in rural and suburban areas of North America and Europe with extant raccoon populations. Here, we present a case of Baylisascaris-induced DUSN from the densely populated borough of Brooklyn in New York City and alert urban ophthalmologists to consider this etiology even in areas not typically thought to be associated with endemic risk factors. Infected raccoons also occur in urban settings, and urban patients may be exposed in surrounding areas. Most patients with Baylisascaris ocular larva migrans-DUSN will not have concomitant neurologic disease; this fact and larval neurotropism are both misconceptions regarding this infection.

PURPOSE: To compare optical coherence tomography with fluorescein angiography in 11 patients (21 eyes) with central foveal damage from solar retinopathy. METHODS: Retrospective, observational case series of 11 patients with solar retinopathy. Dilated funduscopic examination was performed as well as photographic, fluorescein angiography, optical coherence tomography, and Humphrey visual field assessment. RESULTS: Significant foveal pathology was identified in each of the 21 eyes (11 patients). Visual acuity in affected eyes ranged from 20/25 to 20/200. Optical coherence tomography demonstrated foveal atrophy associated with a characteristic defect at the level of the inner and outer segment junction of the photoreceptors in all 21 affected eyes, whereas fluorescein angiography identified classic window defects in 19 eyes (10 of 11 patients). There was a modest correlation between foveal thickness and visual acuity. CONCLUSION: Fluorescein angiography did not detect lesions characteristic of solar retinopathy in all patients with a definitive history of sungazing and visual loss. Conversely, optical coherence tomography did detect significant foveal atrophy in all affected eyes and a characteristic defect at the photoreceptor-retinal pigment epithelium junction. Optical coherence tomography improves the diagnosis and assessment of the degree and nature of foveal damage in patients with solar retinopathy and may be an important tool in identifying foveal damage not detected by standard fluorescein angiography.

PURPOSE: The purpose of this study was to report the first case of Streptococcus bovis causing a perforating corneal ulcer in a healthy adult. METHODS: Observational case report. RESULTS: A healthy 77-year-old man presented with rapidly progressing corneal ulcer, resulting in perforation within 4 days. Preoperative and operative cultures isolated the same organism, S. bovis, susceptible only to vancomycin and ceftriaxone. The patient underwent a successful corneal transplantation, with histopathology revealing full-thickness necrosis and scattered bacteria. CONCLUSIONS: Although a rare pathogen in ocular infections, we report a case of devastating corneal infection caused by S. bovis and highlight consideration of this pathogen in rapidly progressing corneal ulcers.

PURPOSE: To systematically study potential adverse events associated with the use of intraocular bevacizumab at a single medical center. METHODS: Retrospective study of all consecutive patients receiving intraocular bevacizumab injections at the Stanford University Department of Ophthalmology between November 15, 2005 and July 14, 2006. Bevacizumab was given for exudative age-related macular degeneration, retinal vascular occlusion, diabetic macular edema, neovascular glaucoma, and five other indications. RESULTS: We analyzed medical records of 186 subjects (203 eyes) who received a total of 578 injections of 1.25 mg of bevacizumab. The average follow-up was approximately 6 months. Five eyes with exudative age-related macular degeneration developed retinal pigment epithelial (RPE) tears, all with preexisting RPE detachments. These five eyes represented 2.9% of all age-related macular degeneration eyes treated and 7% of the age-related macular degeneration eyes with preexisting RPE detachments at initiation of treatment. Other adverse events were rare and included retinal ischemia, subretinal hemorrhage, vitreous hemorrhage, ocular irritation or pain, worsened hypertension, and headache. No death or thromboembolic events were observed. CONCLUSION: Intraocular bevacizumab appears to be well tolerated for the treatment of a variety of retinal and choroidal vascular diseases. RPE tears may occur when treating choroidal neovascularization, particularly in patients with preexisting RPE detachment.

PURPOSE: To prospectively evaluate the safety, efficacy, predictability, and long-term stability of hyperopic laser in situ keratomileusis (LASIK) using the Star S2 excimer laser (Visx). SETTING: Department of Ophthalmology, Stanford University School of Medicine, Stanford, California, USA. METHODS: This study evaluated 41 eyes of 27 patients who had LASIK for the correction of spherical hyperopia. Patients were divided into 3 groups based on preoperative cycloplegic refraction: low hyperopia (less than +2.00 diopters [D]), medium hyperopia (+2.00 to +4.00 D), and high hyperopia (more than +4.00 D). RESULTS: In all eyes, the mean preoperative spherical equivalent (SE) was +2.53 D (range +0.75 to +5.25 D) and the mean attempted SE was +2.58 D (range +0.75 to +5.25 D). At the last visit (mean 5.29 years), the mean SE was +0.44 D +/- 0.69 (SD) in the low hyperopia group, +0.58 +/- 0.56 D in the medium hyperopia group, and +0.59 +/- 1.18 D in the high hyperopia group. The percentage of eyes within +/-1.00 D of emmetropia was 82.4%, 75.0%, and 66.7%, respectively (68.9% overall). The uncorrected visual acuity was 20/40 or better in 100% of eyes in the low hyperopia group, 91.7% in the medium hyperopia group, and 66.7% in the high hyperopia group (87.8% overall). The hyperopic shift between 1 year and the last visit was +0.18 D, +0.30 D, and +0.55 D, respectively (+0.40 D overall; range -1.25 to +2.63 D). CONCLUSIONS: Hyperopic LASIK using the Star S2 excimer laser was safe, effective, and predictable for hyperopia up to +4.10 D (mean of high group). A mild regression of the refractive effect was seen over the 5-year follow-up.
