Ten own observations of chyloperitoneum for the 1984-2000 yrs period were analyzed. Algorithm of steps of surgeon in revealing chyloperitoneum was substantiated.

[Aspects of clinical course and treatment of the gastroduodenal ulcer perforation in combination with hemorrhage]. / Osobennosti klinicheskogo techeniia i lecheniia perforatsii gastroduodenal'noi iazvy v sochetanii s krovotecheniiem.

The results of treatment of 51 patients with gastroduodenal ulcer perforation (GDUP) in combination with hemorrhage were analyzed. Frequency of this complication in the gastroduodenal perforative ulcers structure had constituted 2.5%. The GDUP clinical variants in combination with hemorrhage were delineated depending on the time of the particular complication occurrence, the peculiarities of their clinical course were depicted. Necessity of an early radical operative intervention was substantiated as well as application of active surgical tactics while postoperative hemorrhage occurrence. Postoperative mortality was 29.4%.

Results of treatment of the thyroid gland cancer in 251 patients were analyzed.

[Relaparotomy: clinical, strategic and organizational aspects]. / Relaparotomiia: klinicheskie, takticheskie i organizatsionnye aspekty.

The results of relaparotomy (RL) performance in 118 patients were analyzed. Mortality was 16.1%. In 50.8% of patients RL was done with delay. There were proposed for timely diagnosis of postoperative complications to use the "alarm" symptoms group and during RL performance--to follow definite technical methods and rules. The RL performance necessity in 62.7% patients was caused by surgical technique failures and nonrational method of the first operation choice.

[Surgical treatment of putrid neck phlegmon]. / Khirurgicheskoe lechenie gnilostnoi flegmony shei.

The experience of treatment of 236 patients with cervical phlegmon, in 7 of whom with putrefactive phlegmon, was summarized. The treatment difficulties are due to rapid infection spreading, the possibility absence of timely laboratory confirmation of diagnosis. Operative intervention was conducted during two hour period of time after the hospitalization moment. An adequate detoxication therapy, hyperbaric oxygenation, antibiotic therapy was conducted to all of the patients, metrogyl was administered without fail. Two patients died because of mediastinitis (1) and the pulmonary artery thromboembolism (1).

[Rheologic blood disorders in ulcer diseases complicated with posthemorrhagic syndrome]. / Nerusheniia reologicheskikh svoistv krovi pri oslozhnennoi iazvennoibolezni s postgemorragicheskim sindromom.

Of the 229 patients with ulcerous bleeding operated on, 27 (11.8%) died. In 19 of them, the death was caused by hemorrhagic syndrome with severe disorders in rheologic blood properties. Considerable decrease in capability of erythrocytes for deformation is, possibly, a pathogenetic link of the posthemorrhagic syndrome development.

[Aspects of surgical treatment of peptic ulcer complicated by hemorrhage]. / Aspekty khirurgicheskogo lecheniia iazvennoi bolezni, oslozhnennoi krovotecheniem.

The article presents materials of clinical examination of 407 patients with ulcer disease of the stomach and duodenum complicated by bleeding. The nearest and long-term results of vagotomy and resection of the stomach are shown in comparison. Totally, organ-preserving interventions give more promising results than resection of the stomach especially by the Billroth-II method. After vagotomy, however, recurrent disease was observed in 8.4% and dyspeptic symptoms took place in 17% of the patients examined. By the data of medical labour examination 3-5 years after operation, advantages of vagotomy become increasingly notable, social labour characteristics of vagotomy as a method of treatment is considerably more preferable than resection of the stomach.

[The effect of 5-fluorouracil on lipid metabolism in patients with acute pancreatitis]. / Vliianie 5-ftoruratsila na lipidnyi obmen u bol'nykh ostrym pankreatitom.

The state of lipid metabolism was investigated in 72 patients with acute pancreatitis using the method of thin-layer chromatography on "Silufol" plates. It was established that the level of general phospholipids underwent a reduction in destructive pancreatitis. The level of triglycerids reduced sharply after treatment with 5-fluoruracil. The revealed changes were the basis for instituting fatty emulsions and lipid mixtures in the treatment of these patients.

[Closed injuries of the spleen]. / Zakrytye povrezhdeniia selezenki.

The 10-year experience with diagnosis and treatment of closed injuries to the spleen in 84 patients is presented. In 13 patients, the two-instant splenic rupture, in 5--spontaneous was noted. Seventy (83.3%) patients had isolated injuries. In 17 patients, blood reinfusion with total volume of 22,800 ml was performed at operation. Post-operative lethality was 4.8%, total--5.9%. One patient died without operation.
