The authors analyse a series of 35 Rippen System cementless hip joint prostheses implanted in 32 patients from May 1987 to September 1988. Certain radiographic parameters were studied in an attempt to find a correlation with the clinical results. Special attention was paid to the high incidence of thigh pain, attempting to identify the cause through radiographic study of the bone-implant interface. Even when analyzed using Kappa statistics, the correlation between the clinical and radiographic findings was purely incidental. In agreement with the literature, the authors identify some elements which should be investigated further such as primary temporary fixation of the stem, reduction of micromotion, and biocompatibility of the materials.

Based on the authors' clinical experience and a review of the literature, the methods used for the diagnosis of loosening in hip arthroprostheses are considered. Clinical and radiographic monitoring are essential but the authors believe that in suspected loosening it is particularly important to perform bone scintigraphy using Tc 99 M.D.P., Ga 67, and Indium 111. Arthrography is advised only in selected cases. The criteria of evaluation in the various tests are reported and a procedure for diagnosis is proposed.

Post-operative infection is still a serious problem in orthopaedic surgery. Technological advances aimed at achieving aseptic conditions in the operating theatre, and the prophylactic use of antibiotics during operation have considerably reduced this complication during the last decade. Two indispensable factors in monitoring the effectiveness of these preventive measures are the determination of the incidence of post-operative infection and monitoring of the microbacterial population of the hospital.

The authors report their results in 17 hips treated by distal transposition of the great trochanter in patients suffering from coxa vara resulting either from congenital dislocation of the hip or Perthes disease. The results, evaluated both clinically and radiologically, were excellent. We would emphasize that distal transposition inevitably implies lateralization, which restores the normal musculo-skeletal relationship. The "hyperlordosis" sign is described and its mechanism explained. This simple physical sign is a useful test in evaluating the indications for this type of operation.
